[Origin of shoulder pain after "neck dissection". Importance of the cervical plexus].

BACKGROUND: The role of sensitive parts of the cervical plexus for the development of neuropathic pain is not yet clear. Our study investigated the correlation between shoulder pain and cervical plexus damage after different types of neck dissection (ND). MATERIAL AND METHODS: The sensitivity for warm/cold and sharp/blunt was tested in the dermatomes of C2, C3, C4, and the minor occipital nerve. Shoulder pain was measured semiquantitatively by a rating scale. RESULTS: Motion-dependent shoulder pain was observed 6 months postoperatively in 50% after resection and in 29.2% after preservation of these structures. Pain occurred more frequently following radical ND than after modified radical types 1 and 3 ND. DISCUSSION: Our investigations showed that the superficial cervical plexus function is assessable by cutaneous sensitivity tests. The minor occipital nerve seemed to be less affected. Fewer pain symptoms in cases with preserved cervical plexus could be demonstrated. We can conclude that preservation of the superficial cervical plexus is important to diminish postoperative shoulder pain.

Immunocytochemical localization of nitric oxide synthase in the brain of the chicken.

The distribution of neuronal nitric oxide synthase (NOS) in the chicken brain was investigated by immunocytochemistry. Strongly stained neurones were concentrated in the paleostriatum augmentatum, lobus parolfactorius, ventral pallidum, olfactory tubercle, parts of the neostriatum, mesencephalic reticular formation and locus coeruleus. Cells in these areas have previously been shown to stain for NADPH-diaphorase, a histochemical activity associated with NOS. However, various structures which NADPH-diaphorase staining has suggested to contain NOS were not immunoreactive: these included the glomeruli of the olfactory bulb, magnocellular preoptic neurones, the median eminence, subcommissural organ and mesencephalic trigeminal neurones. NOS was sparsely present in the hyperstriatum ventrale, providing evidence against the involvement of nitric oxide in certain forms of learning and memory processes known to occur in this region.

[The Family System Test: a 3-dimensional method for analyzing social relation structures].

The Family System Test (FAST) is a clinically-derived figure placement technique designed for a three-dimensional representation of cohesion and power in the family. The FAST measures individual and group perceptions in typical, ideal and conflict situations. The flexibility of family structures is assessed by comparing representations of these situations. Scoring procedures as well as validity and reliability of the FAST are described. The relevance of the FAST is discussed from a systemic, developmental and clinical perspective.
