[10 years of functional reconstructive surgery in exstrophy of the urinary bladder--a preliminary report (author's transl)].

Functional reconstructive surgery is performed in bladder exstrophy at the University Children's Hospital of Zurich since 1971. The surgical procedure is usually done in three stages--bilateral iliac osteotomy--turn-in-operation wih preliminary reconstruction of the bladder neck and antireflux surgery--closure of the epispadia, if necessary combined with continence plasty. Out of 26 patients with bladder exstrophy 23 underwent reconstruction. 3 patients died postoperatively. One was lost for follow-up. The preliminary long-term results in 19 patients are presented. The assessment was based on four criteria: last intravenous pyelography, urinary function, frequency and severity of urinary infections and degree of attained continence. The results were classified as good in 5 patients, fair in another 5 patients and insufficient in 6. 3 patients were too young to be assessed. The results are compared with other series reported in literature.

[Heterotopic autotransplantation of splenic tissue after traumatic rupture of the spleen - a solution after unavoidable splenectomy? (author's transl)].

Considering the increased incidence of infections and the danger of postsplenectomy sepsis (OPSI) a preservation of the ruptured spleen by suturing etc. is usually attempted today. When splenectomy is unavoidable autotransplantation of splenic tissue may preserve some degree of splenic function. We report our first successful case. 6 months after autotransplantation splenic tissue could be visualized by technetium 99 scintigraphy. Howell-Jolly bodies were absent in the peripheral blood and the immunoglobulins were normal.

[Clinical aspects of neuronal intestinal dysplasia (author's transl)].

Neuronal intestinal dysplasia is histologically and histochemically well defined. The resection of the neuronal dysplastic segment is usually recommended in the literature as being curative. Whether a resection is necessary in every case remains to be debated. The case history of a male patient with Hirschsprung's disease and proximal intestinal neuronal dysplasia of the entire colon is presented. After manometrical, radiological and functional examination the aganglionic portion of the gut only has been resected and the neuronal dysplastic part has been left in situ. Several months later the patient is well. It seems possible to avoid in some selected cases a subtotal or total colectomy.

[Epiphyseal fractures of the distal tibia. Treatment and results].

Epiphyseal fractures of the distal end of the tibia generally require operative treatment. A conservative treatment can be recommended only in minimally displaced fractures with undisturbed ankle joint congruity. Among 48 patients treated (40 operatively, 8 conservatively) 6 developed angular deformities, 3 of which required a corrective osteotomy. After the initial treatment follow-up is necessary until bone growth is complete.

[Epiphysiolysis of the distal tibia. Treatment and results].

23 patients with traumatic epiphyseolysis of the distal tibia with and without additional metaphysical fragment have been seen at the Surgical Department of the University Children's Hospital, Zurich. 21 were treated conservatively, only 2 were operated upon. 21 of the 23 children were found for follow-up 2 to 11 years, mean 6 years after trauma. 15 were well and without any clinical symptoms, 6 had occasional slight pains with changing weather etc. Clinical examination was normal in all patients. 4 had axial malposition of more than 8 degrees at X-ray control. One patient with additional periostal interposition had an early epiphyseal closure and needed therefore an epiphyseodesis of the contralateral distal tibia. None of the 21 patients had more than 10 mm difference in length compared with the non-injured leg. Our late results confirm that in cases of traumatic epiphyseolysis, the conservative way is usually the treatment of choice.

Pediatric surgical patients with severe anaerobic infection: report of 16 T-antigen positive cases and possible hazards of blood transfusion.

Red blood cells become polyagglutinable when the normally latent T-antigens of the red blood cell membrane are exposed. Unmasking of T-antigens results from removal of N-acetyl-neuraminic acid by neuraminidase, an enzyme commonly produced by a variety of bacteria. Red blood cells altered in this way are said to be T-activated. T-activated red blood cells can be agglutinated by anti-T, an antibody normally present in human serum, so that severe transfusion reactions may occur and have occurred, if T-antigen positive patients are transfused with normal whole blood or plasma. This can be avoided by transfusing only packed or washed red blood cells. From October 1978 to October 1980 we found T-activation in 16 pediatric surgical patients aged 3 days to 14 yr with severe anaerobic infections. This included patients with necrotizing enterocolitis, perforated appendicitis, megacolon, infected anal atresia and gas gangrene. The isolate neuraminidase-producing bacteria were Clostridium perfringens and Bacteroides fragilis. Clinical data of these 16 patients are briefly reviewed and the importance of T-antigen positivity for their management is discussed.

[Interposition of periosteum in joint fractures in adolescents; comparison of operative and conservative treatment (author's transl)].

Interposition of periosteum in joint fractures in adolescents may lead to delayed consolidation of the fracture or even malposition (varus-valgus deformities etc.) by early partial closure of the epiphyseal line. In severe cases, early arthrosis may be sequelar of inappropriate treatment of these patients. 14 cases of joint fractures with interposition of periosteum in adolescents are presented. The results of 5 operated cases are compared with 9 conservatively treated patients. The need for operative treatment is stressed.

[Long-term results in intestinal atresias and stenoses distal to the ligament of Treitz (author's transl)].

From 1945 to 1975 61 cases of atresias and stenoses of the small intestine distal to the ligament of Treitz and atresias and stenoses of the colon were treated at the University Children's Hospital Zurich. The long-term results of 51 well documented cases are presented and the influence of improvement of operative techniques the establishment of an intensive care unit and the introduction of long-term parenteral nutrition is analyzed. Cases treated during the earlier period (1945-1965) are compared with those from 1966 to 1975. The overall-all mortality was high. Out of 51 patients there were only 19 long-term survivors. During the first period 71% died, during the second 52%. The reason for the still high mortality is especially the high number of intraoperative additional complicating findings (meconium peritonitis, volvulus etc.). Additional malformations in other organ systems and low birth weight were not important factors for survival in this series. None of the 19 long-term survivors had to be reoperated after the first discharge from the hospital. At late control they were healthy, and most of them without abdominal symptoms. Only three complained of occasional diarrhoea. All but one were within normal percentiles for weight and height.

Comparison of late results in patients with corrected intestinal malrotation with and without fixation of the mesentery.

Between 1940 and 1975, 86 patients with malrotation of the intestine leading to intestinal obstruction or volvulus were treated at the University Children's Hospital of Zürich. All case histories and radiograms have been examined and a questionnaire has been sent to all surviving patients. Forty-four of them were reexamined personally 3 to 37 yr later. The study was undertaken to find out whether at the time of correction of intestinal malrotation simultaneous additional fixation of the mesentery had any value. Six of the 86 patients were successfully treated nonoperatively and 3 died before surgery of other severe malformations. Of the remaining 77 children who underwent surgery, additional fixation was done in 28 of the earlier cases, but not done in 49 of the later cases. The percentage of early survivors was 85.7% in the group with intestinal fixation and 83.7% in the group without fixation. The number of reoperations was slightly lower in the group without fixation (10.4%) when compared to the group with fixation (16.6%). 25% of the children with intestinal fixation and 21% of the children without fixation had occasionally slight abdominal symptoms at a later time. These results demonstrate that intestinal fixation neither increased the number of early survivors nor reduced the number of reoperations, nor the percentage of children with abdominal complaints. It was therefore concluded that additional fixation after correction of intestinal malrotations was of no benefit and unnecessary.

[Eventration of the diaphragm in infancy and childhood (author's transl)].

31 children with eventration of the diaphragm were observed at the surgical department of the University Children's Hospital in Zürich from 1960 to 1975. 27 of the 31 children were less than one year old. In 14 of the cases the eventration was caused by a congenital hypoplasia of the diaphragm, in 12 by a traumatic paralysis of the phrenic nerve and in 3 cases it occurred postoperatively. 2 children had acquired the eventration later on in childhood. 18 of the 31 children were operated upon, 13 were treated conservatively. All except one survived and were examined again 3 to 15 years later. The various pathological anatomical forms, the clinical picture, signs and symptoms and the indication for conservative or operative treatment are discussed and illustrated by typical cases of the present series.

Transplantation of the small intestine. An experimental study in young minipigs.

An experimental study on small intestinal transplantation on 82 young minipigs is reported. The selected experimental model allows for early recognition of complications in the graft. A monitoring system, based on multiple sequential mucosal suction biopsies is presented, Transplanted segments up to 120 cm were rejected in similar way as are other organs. There was no evidence of a graft versus host reaction.

The risks of small intestinal transplantation for the recipient: experimental results in young minipigs.

In a previous series of 36 small intestinal transplants in young minipigs, a monitoring system, based on a special transplantation model, clinical and histologic criteria has been worked out. The spontaneous evolution was observed and no attempt was made to save the animal when complications on the graft occurred. Most of the animals died. In the present series of 32 small intestinal transplants the monitoring system and explanation criteria are now tested. Twenty-three primarily successfully transplanted animals had complications of the graft, i.e., hemorrhagic infarction, deficient arterial circulation, or rejection. Only 1 of the 23 died before explanation could be done. Twenty-two were explanted according to the previously established criteria. Two died of complications not directly related to the graft, the other 20 animals survived for 1-5 mo. All 22 explanted grafts showed irreversible damages both macroscopically and on histologic examination. The present series confirms that early recognition of complications of the graft and timely removal with survival of the recipient animal is possible. This could be an important step forward to the clinical application of small intestinal transplantation in man.

[The milk curd syndrome].

The milk curd syndrome is an as yet little known cause of intestinal obstruction in neonates. Two personal cases are reported, and 37 others are collected from the world literature. Infants with milk curd syndrome have a normal discharge of meconium. The intestinal obstruction appears usually between the 2nd and 16th day after birth. In most of the cases, the milk curd was found in the distal part of the ileum. The aetiology of the milk curd obstruction syndrome remains still unclear. The composition of artificial milk feeding formula in connexion with transitory absorption deficiency of amino acids, and the possibility of a temporarily insufficient excretion of bile acids are discussed.

Indications for operative treatment of fractures in childhood.

Fractures in children heal more quickly than in adults. Stiffness of joints hardly ever occurs in children, even after prolonged immobilization. Pseudoarthrosis and Sudeck's atrophy are extremely rare. Axial, lateral and longitudinal malpositioning can be largely compensated by increased growth. Only rotation malpositions remain permanent in children. By and large, the treatment of fractures in childhood is therefore still conservative. Operative treatment is recommended for some types of fractures near joints, for fractures of the neck of the femur, and for some types of the fractures around the elbow. In all other types of fractures surgery should only be undertaken when the conservative methods fail. An exception may be in adolescents over 12 to 14 years of age and children with multiple trauma, especially cerebral trauma with consecutive spasticity. In these patients, conservative treatment with extension may be difficult and an osteosynthesis may facilitate general care.

Oesophageal atresia: triumph and tragedy.

An enormous amount has been written about oesophageal atresia during the last 30 years. This is not surprising because it is not so long ago that the condition was uniformly fatal, and even today, a generation after the first successful operations, many problems associated with its management have not been completely solved. This lecture discusses past and present management, past and present results and future prospects of infants suffering from this malformation.

[The Göttinger minipig as a laboratory animal (author's transl)].

Blood pictures, serumelectrolytes, acid-base- and bloodgas-values of the arterial blood were determined from two juvenile and one adulte collectives of minipigs (Gottingen strain), which were kept under SPF-conditions. The results were compared with the corresponding values of human blood and should be regarded as base- or orienting values for further investigations.
