Uricosuric effect of different doses of irtemazole in normouricaemic subjects.

Irtemazole 12.5 to 50 mg in 6 healthy, normouricaemic subjects caused a maximal decrease in plasma uric acid (after 8 to 12 h) of 46.5%. The uricosuric effect began during the first 60 min after drug administration and it lasted for 7 to 24 h. Renal uric acid excretion returned to its base line value after 8 to 16 h and uric acid clearance after 10.0 to 12.0 h. Doses of irtemazole between 12.5 and 37.5 mg produced a dose-related rise in the uricosuric effect. There was no essential difference between the uricosuric effect of 37.5 mg and 50 mg irtemazole. The D50 dose (that producing a half-maximal effect) was between 16.3 mg and 34.2 mg, (average 24.7 mg). The value of irtemazole in the management of hyperuricaemia and gout remains to be determined.

Nephrolithiasis in twins with APRT-deficiency. Stones as a marker of an inborn error of metabolism.

We describe the first case of twins with complete APRT-deficiency, indicating homozygosity, and 2.8-dihydroxyadenine stones in both kidneys. The stones were discovered accidentally during a routine abdominal ultrasound examination, and thus the diagnosis of an inborn error of metabolism, was established. In nephrolithiasis caused by an inborn error of metabolism, early diagnosis and therapy are of great importance for prognosis and quality of life.

[Comparative measurement of skinfold thickness of the back of the hand with caliper and ultrasound--initial results of a prospective study].

In 110 persons the thickness of the skin on the back of the hand was measured by ultrasound (B-mode) and caliper, and the results were compared. The thickness of the skin measured by ultrasound was statistically lower than measured by caliper. This seems to be due to the better distinguishing of cutis from subcutaneous fat by ultrasound. Both methods revealed reproducible results. Caliper measurement is advantageous because of its simple handling. 10 MHz transducer is also useful for measuring the thickness of the skin.

Is ultrasound superior to carcinoembryonic antigen (CEA) measurement in recurrent tumor screening of patients with colorectal carcinoma?

Assuming that in the postoperative recurrent tumor screening CEA levels are as important as imaging techniques in elucidating the spread or the recurrence of a tumor, we examined 61 patients with colonic carcinoma. Those patients were operated curatively and were regularly controlled by clinical examination, laboratory, sonographic, endoscopic or radiological methods. In 3 patients there was a local recurrent, in 3 other patients there was a local recurrent and metastases. 8 patients showed sonographic livermetastases only, proved by computed tomography. In 6 patients CEA-levels rose. In 2 patients with sonographically confirmed metastases of the liver, there were neither clinical nor laboratory hints for metastases and the CEA-levels were normal. No one had elevated CEA-levels before detecting metastases. Our results indicate that ultrasound of the liver can detect metastases of colonic tumors earlier than clinical or laboratory sings do. Because of its greater diagnostic security, ultrasound is superior to CEA-measurement in recurrent tumor screening.

[Angiomyolipoma of the kidney--value of imaging procedures].

The angiomyolipoma of the kidney is composed by blood vessels, smooth muscle tissue, and fat. It is important to differentiate this benign tumor from a malignant one. Two cases demonstrate the diagnostic value of the different imaging procedures. In 'typical' cases computed tomography shows the density of fat; histological examination is not necessary and follow up control with ultrasonography is sufficient. In other cases with a low part of fat histological examination cannot be missed. Angiography is not helpful for the differentiation from a malignant tumor.

Rapid onset of uricosuria after oral administration of irtemazole, an uricosuric substance.

A single 50 mg dose of irtemazole was given orally to ten healthy male volunteers. The onset of effects was tested. Plasma uric acid began to decrease 15 to 25 min after irtemazole was administrated. Renal uric acid excretion and uric acid clearance increased 10 to 20 min after drug administration. Maximal renal uric acid excretion (mean 197.4 mg/h) and maximal uric acid clearance (mean 78.4 ml/min) were reached after 15 to 55 min. No side effects were observed. The effects of irtemazole occur earlier than those of benzbromarone. The therapeutic effects of long-term therapy remain to be investigated.

Uricosuric effect of irtemazole in healthy subjects.

Following a single dose of 50 mg irtemazole per os, plasma uric acid levels decreased after 1 h and fell to 53.5% of the original value within 6-12 h. Renal uric acid excretion increased up to 66 mg/h 30 min after drug application and reached its maximum of 151 mg/h 30 min later. Uric acid clearance also increased after 30 min and reached its maximum of 56 ml/min after 60 min. The response of the kidney to irtemazole is faster than to benzbromarone or probenecid. Lowering of plasma uric acid has a shorter-lasting effect than benzbromarone or probenecid. At 24 h after the application of 50 mg irtemazole the decrease of the plasma uric acid was between 15.4% and 30.0%, or 24.7% on average. Three days after the application the basic plasma uric acid levels were reached again.

[Sweet syndrome in chronic myeloid leukemia].

A 31-year-old woman with chronic myeloid leukemia developed bullous skin changes not responding to antibiotic therapy. A biopsy showed a sterile predominantly neutrophilic infiltrate consistent with Sweet's syndrome. These skin changes responded well to methylprednisolon. Seven months later the patient died in a blast crisis.

[Actinomycosis of the skin in HIV infection].

A 29-year-old woman with HIV-infection developed disseminated abscesses of the skin, identified as an infection with Actinomyces israelii. To our knowledge, actinomycosis associated with HIV-infection (AIDS-related complex) has been reported only once previously.

Pharmacokinetics and pharmacodynamics of different doses of Irtemazole in repeated application.

Irtemazole, a new uricosuric substance, was tested in 12 normouricemic subjects in doses between 6.25-37.5 mg applied twice daily for 7 days per os. The plasma uric acid level decreased in all subjects, to a constant level within 1-2 days with application of the higher doses, and within 2-3 days with application of the lower doses, and was maintained as long as irtemazole was given. The average decrease of plasma uric acid was 20.4% at a dose of 6.25 mg irtemazole, 22.7% at 12.5 mg, 42.0% at 25 mg, and 45.7% at 37.5 mg. The renal uric acid excretion and the uric acid clearance increased to a constant level within 1 day of irtemazole application. The initial levels of plasma uric acid, uric acid excretion, and uric acid clearance were reached after 1-2 days after cessation of irtemazole. Within 1 to 2 days after ingestion of irtemazole a constant plasma irtemazole level and renal irtemazole excretion were observed in all doses throughout the application period. One to 2 days after discontinuation of irtemazole it was no longer detectable in plasma and urine. No side effects were found.

Uricosuric effect of irtemazole in hyperuricemic patients without and with renal insufficiency.

A single oral dose of 50 mg irtemazole exhibited a clear cut uricosuric effect in five patients with hyperuricemia and normal renal function. The average maximal decrease of plasma uric acid was 46.1% compared to the initial value; it was reached between 6 h and 12 h after drug administration. Extent and course of plasma uric acid showed no differences between normouricemic (5) and hyperuricemic subjects. In four patients suffering from hyperuricemia and renal insufficiency only a minor effect of irtemazole was seen. The average maximal decrease of plasma uric acid reached 12.8% compared to the initial value. It occurred between 8 h and 12 h after drug ingestion. In both normouricemic and hyperuricemic subjects irtemazole acts more quickly than benzbromarone and probenecid. The duration of its uricosuric effect is shorter. Like benzbromarone, irtemazole exhibits only a minor effect in patients with reduced creatinine clearance.

[Renal malacoplakia--a rare diagnosis. Imaging with sonography and computerized tomography].

A 61-year-old man with septic enophthalmitis and cramps in the upper abdominal region, fever and leukocytosis was admitted to hospital. Ultrasound examination revealed an inhomogenous enlargement of the left kidney with normal conturation and swollen, in part almost transonic, renal parenchyma. In the area of the left kidney CT demonstrated an enlarged tumor with inhomogenous and destroyed structures. The left kidney was removed. Histology confirmed the diagnosis of renal parenchymal malakoplakia.
