[Visual rating of T2'-blood-oxygen-level-dependent magnetic resonance imaging in acute stroke patients--a pilot study].

PURPOSE: Delineation of brain tissue that is at risk but not yet infarcted (penumbra) continues to be a major challenge for stroke imaging. Metabolic characterization of the penumbra might be able to be achieved using blood-oxygen-level-dependent (BOLD) imaging. MATERIALS AND METHODS: We analyzed MRI data from 20 patients within the first 6 hours after stroke onset and after 5-8 days. Among other sequences, the MRI protocol consisted of diffusion-weighted (DWI/ADC = apparent diffusion coefficient) and quantitative T2 and T2* imaging (qT2, qT2*). BOLD images (T2') were calculated using 1/T2' = 1/qT2* - 1/qT2. BOLD lesions were rated by two blinded observers. RESULTS: Based on the primary blinded reading of the BOLD images, the lesion side was rated correctly by observers 1 and 2 in 80/50 % of the cases, incorrectly in 5/40 % of the cases, and rated as not visible in 15/10 % of the cases. After unblinding the observers, the visibility was rated in 45/45 % of the cases as good, in 35/40 % of the cases as reasonable, and in 20/15 % of the cases as insufficient for diagnostic purposes. The sensitivity for subsequent infarct growth was 0.88 (95 % confidence interval, CI 0.47 to 0.99), the specificity was 0.33 (95 % CI 0.07 to 0.70), the positive predictive value (PPV) was 0.54 (95 % CI 0.25 to 0.81), and the negative predictive value (NPV) was 0.75 (95 % CI 0.19 to 0.99). The odds ratio for subsequent infarct growth was 3.5 (95 % CI 0.20 to 115.53). CONCLUSION: Hypo-intense lesions in BOLD imaging were visible and exceeded the lesion in diffusion-weighted imaging in most of the stroke patients. The encouraging results justify further testing of the hypothesis that BOLD lesions, when larger than DWI lesions, are associated with infarct growth from initial DWI to final infarct.

Surgical treatment of a basilar perforator aneurysm not accessible to endovascular treatment.

Aneurysms originating from perforatoring branches of the midbasilar artery are extremely rare. Rupture of such an aneurysm resulted in a subarachnoid hemorrhage with a prepontine clot in a 44 year old male who presented with an acute confusional state. After coil embolization had failed, the partially thrombosed aneurysm was wrapped and coagulated via a combined supra-/infratentorial subtemporal presigmoid approach in prone position. The postoperative course was complicated by a tension pneumatocephalus and liquorrhea. Additional aneurysms of the anterior communicating artery and right middle cerebral artery were clipped several months later. The patient recovered well, and except for slight gait ataxia no other deficit remained.

[Duplex ultrasound of external carotid artery branches for the detection of dural arteriovenous fistulae].

PURPOSE: To evaluate duplex ultrasonography of external carotid artery branches for the non-invasive diagnosis of dural AV fistulae (DAVFs). MATERIALS AND METHODS: A total of 126 patients suffering from pulsatile tinnitus underwent duplex ultrasonography of the cervicocephalic vasculature including external carotid artery branches. DAVFs were identified by increased flow velocity and reduced pulsatility. Patients with ultrasound findings of DAVFs were considered for selective angiography. RESULTS: Of the 23 patients that fulfilled the ultrasound criteria of AV fistula, 5 did not undergo angiography and were excluded from the study. The ultrasound findings of AV fistula were confirmed in all remaining patients (17 DAVFs, 1 extracranial AV fistula). Duplex ultrasound detected a DAVF of the common carotid artery in 29 %, of the external carotid artery in 65 % and of the external carotid artery branches in 100 % of the cases. CONCLUSION: Duplex ultrasonography of external carotid artery branches improves the non-invasive detection of DAVFs. For the evaluation of the venous drainage, however, selective angiography is necessary.

[Clinical relevance of cranial CT under emergency conditions--basic neuroradiologic investigations].

PURPOSE: To determine the diagnostic accuracy of cranial CT under emergency conditions. MATERIAL AND METHODS: Four hundred cranial CT examinations (cerebrum, petrosal bone, fascial bone, sinuses, orbita) were evaluated retrospectively and correlated with the initial neurologic examination, the discharge and surgical diagnoses, and the results of other imaging modalities (conventional X-Ray, MRI, angiography). The average age was 49 (2 to 89) years, with 42 % male and 58 % female patients. The discharge diagnosis was defined as the gold standard, assuming that clinical examinations and laboratory and imaging studies were maximized to establish the diagnosis, together with any possible surgery. RESULTS: Overall sensitivity, specificity and diagnostic accuracy was 85 %, 70 % and 82 % for cranial CT, 66 %, 45 % and 61 % for the initial neurologic examination, 79 % and 29 % for acute cerebral infarction, and 65% and 13% for intracranial bleeding. Correlation of the CT diagnoses to the discharge and surgical diagnoses was high (82 % and 80 %, respectively; p < 0.01), to the diagnosis of conventional x-ray moderate and to the diagnoses of angiography and MRI unsatisfactory (74 % and 68 % and 60 %, respectively; p > 0.05). Craniotomy was performed in 47 of 400 (12%) patients after cranial CT, within 12 hours in 29, in 12 to 24 hours in 8, and after 24 hours in 10 patients. The risk for intracranial bleeding in patients with a skull fracture was increased by a factor of 3.3. CONCLUSION: The high diagnostic accuracy of cranial CT in establishing a precise, timely and reproducible diagnosis confirms the important role of CT as primary diagnostic method under emergency conditions.

Collateral circulation is an independent radiological predictor of outcome after thrombolysis in acute ischaemic stroke.

We tested the hypothesis that the type of vascular occlusion, recanalisation and collateralisation are predictive of outcome after thrombolytic therapy in acute ischaemic stroke. We carried out angiography and local intra-arterial (97) or systemic (14) thrombolysis within 6 h of the onset in patients with an ischaemic stroke in the territory of the internal carotid artery. Early ischaemic signs (EIS) on pretreatment CT and angiographic findings were classified and analysed in relation to clinical outcome at 3 months. A favourable outcome (Barthel index [BI]>/= 90) was found in 40% of patients with an occlusion of the middle cerebral artery trunk whereas intracranial occlusion of the internal carotid artery ("carotid T occlusion") was followed by death or severe disability (BI<50) in 87%. Significant univariate predictors of favourable outcome were occlusion type ( P<0.01), recanalisation ( P<0.01) and collateralisation ( P<0.01). However, multivariate analysis revealed a significant relationship only between collateralisation and favourable outcome (odds ratio 5.9, 95% confidence interval 1.3-26.7, P=0.02). EIS were not predictive in either case. Occlusion type and recanalisation, are related to outcome only if adequate collateralisation prevents infarction until recanalisation occurs.

Diffuse arachnoidal enhancement of a well differentiated choroid plexus papilloma.

INTRODUCTION: The case of a 38-year old man with a histologically benign choroid plexus papilloma arising within the fourth ventricle with en plaque growth around the brain stem and medulla is described in detail. Up to this point this particular growth pattern has not been published and is a rare presentation for this tumour. CLINICAL PRESENTATION: The patient presented with a 1.5 year history of headache, nausea, and vomiting in the morning. Additional symptoms like blurred vision and gait ataxia lead to hospital admission. MRI demonstrated a homogeneously contrast-enhancing tumour completely filling the fourth ventricle and subsequent obstructive hydrocephalus. In addition Gd enhancement encasing the brain stem, the lower aspect of the medulla and the conus medullaris was seen suggesting a disseminated ependymoma or medulloblastoma. INTERVENTION: An extensive resection of the tumour in the fourth ventricle and CP angle was performed. Infiltrative growth into the structures of the left CP angle and into the rhomboid fossa hampered complete removal. Surprisingly histological examination revealed a well-differentiated papillary choroid plexus papilloma without signs of anaplasia. On follow up imaging the Gd enhancement encasing the pons vanished completely. A growing cyst adjacent to a small tumour residuum left behind on the floor of the fourth ventricle led to re-operation after 8 months with complete removal. DISCUSSION: This case presents several biological, neuroradiological and surgical aspects which make it noteworthy and we hope that the informations provided add to the understanding of these tumours, expand the differential diagnostic thinking of lesions which present with diffuse arachnoid Gd enhancement upon first presentation.

Cavernous sinus sampling in selected cases of Cushing's disease.

The classical trias in Cushing's disease, normal or moderately elevated plasma ACTH, significant suppression of cortisol in the high-dose dexamethasone test, and stimulation of ACTH and cortisol in the CRH test, confirms the diagnosis in most cases. However, as a referral centre for complex Cushing's disease cases, we are confronted with problems in the differentiation of pituitary and ectopic Cushing's syndrome and with problems in the localisation of minute ACTH-secreting pituitary adenomas. In this study, cavernous sinus sampling (CSS) was evaluated as a diagnostic tool in complex Cushing's disease cases. Thirty-five patients were transferred to our unit for the treatment of Cushing's disease between January 1999 and August 2000. Of those, 17 patients (including five children) had the combination of equivocal results in endocrinological testing and negative MRI prior to admission. In these cases, CSS was performed preoperatively to confirm the diagnosis and to obtain further information about the localisation of pituitary microadenomas. Twelve of these 17 patients showed the classical trias for Cushing's disease after equivocal tests were repeated. A central-peripheral gradient was also found using CSS. In ten of the 11 patients, where CSS could be successfully performed, the lateralisation of the ACTH-adenoma was correctly predicted (91%). In the eleventh case, a right ACTH-gradient was found in a stalk tumour. In one patient (a child), the catheterisation of the cavernous sinus failed due to anatomical reasons. Eleven of these 12 patients successfully underwent transnasal adenomectomy. In two of the 17 patients, active Cushing's syndrome was excluded. In the remaining three cases, ectopic Cushing's syndrome was suspected based on CSS results. Therefore, these patients did not undergo pituitary exploration. In complex cases, we recommend CSS for diagnostic purposes, especially for the localisation of ACTH-secreting microadenomas within the pituitary. In our experience, CSS has a higher accuracy than inferior petrosal sinus sampling (IPSS) in the localisation of ACTH-adenomas.

Operative or endovascular treatment of ruptured intracranial vertebral artery aneurysms?

Our purpose was to evaluate the surgical and endovascular treatment outcomes of ruptured intracranial vertebral artery aneurysms (RIVAA). The outcomes of 44 patients with RIVAA treated between 1983 and 1998 surgically (26), endovascularly (20) or both (2) were evaluated. The aneurysms were clipped in 24 patients, and clipped and wrapped in two. We treated 20 by the endovascular approach, 12 with Guglielmi detachable coils (GDC), and eight by parent-vessel occlusion using detachable balloons. Three patients had endovascular treatment after a failed or inadequate surgical attempt. Posttreatment follow-up was 17-183 months (mean 101 months) for surgically treated patients. For the GDC-treated group angiographic follow-up was carried at 8-49 months (mean 19 months). The condition of seven (27 %) of the surgically treated patients worsened due to procedure-related complications, compared with 10% in the endovascular treatment group. Of the patients initially presenting with Hunt and Hess grade IV or V, three of five (60%) died who were treated surgically and two of eight (25%) who were treated endovascularly. A good outcome was achieved in 17 surgically treated patients (85% of the survivors) and in 16 of the endovascular group (89% of the survivors). This present "same-site" report on treatment of a specific abnormality, RIVAA, treated surgically or by an endovascular approach indicates that especially in poorer Hunt and Hess grade patients, the latter may offer a clinical outcome as good as that of surgery, although long-term efficacy of GDC treatment is still to be determined.

Clinical significance of pedicle aneurysms on feeding vessels, especially those located in infratentorial arteriovenous malformations.

OBJECT: The diminishing threshold for the application of neuroimaging leads to an increasingly frequent diagnosis of previously asymptomatic arteriovenous malformations (AVMs). In such a context, it is warranted to define the criteria that make a lesion potentially hazardous so that neurosurgeons and patients reach a decision concerning how to manage the AVM. In addition to the proposed grading system for AVMs, which assesses the risk of an actual treatment procedure, several studies have been concerned with the evaluation of angioarchitectural features. The goal of the present study is to demonstrate the significance of feeding vessel pedicle aneurysms, especially those found in infratentorial AVMs. METHODS: To determine the incidence of associated aneurysms, the authors reviewed an unselected cohort of 242 consecutive patients with AVMs managed between 1989 and 1999. Within this group were 240 patients who were treated by surgery, endovascular techniques, or a combination of both. Of these patients, 216 harbored a supratentorial and 24 an infratentorial AVM. Two additional patients with supratentorial AVMs underwent treatment of ruptured aneurysms without treatment of the AVMs. In six of the patients with supratentorial AVMs, proximal flow-related aneurysms were found on major feeding arteries, only one of which had caused hemorrhage. In only one patient were there additional distal feeding vessel pedicle aneurysms near the AVM, one of which had caused a major hemorrhage. In contrast, four of 24 patients with infratentorial AVMs had distal feeding artery pedicle aneurysms. Three of these aneurysms had caused hemorrhage. CONCLUSIONS: Pedicle aneurysms on feeding vessels are frequently associated with hemorrhage (four of five cases in this series). In our cohort of 242 treated patients (240 treated for AVM and two for an aneurysm), feeding vessel pedicle aneurysms appear to occur more frequently in conjunction with infratentorial AVMs, which justifies aggressive management to prevent incidences of morbidity associated with rupture of the aneurysm.

[Spontaneous vertebral arteriovenous fistula. Detection and treatment follow-up with color-coded duplex ultrasound].

Spontaneous or traumatic arteriovenous fistulae between vertebral artery and the surrounding venous plexus may cause vertebrobasilar hypoperfusion by steal effects. We report on a 71-year-old man presenting with vertigo. Duplex sonography revealed a vertebral arteriovenous fistula at the C4/5 level with the typical perivascular color Doppler artifact and hyperperfusion in the supplying arteries and draining veins. Angiography confirmed the findings; the consequently performed endovascular embolization using platin coils and silicon balloon removed the symptoms immediately. Ultrasonographic follow-up examinations within 5 months demonstrated the success of therapy showing only low-flow fistula yet. This case demonstrates that early detection of a vertebral arteriovenous fistula by duplex sonography is highly beneficial because efficient treatment modalities are available.

Two-step presurgical endovascular treatment of five arteriovenous malformations partially fed by single vessels en passage.

PURPOSE: To describe a method for the preoperative embolization of arteriovenous malformations (AVM) containing vessels en passage (VeP). First, before embolization of the primary AVM, the distal portion of the VeP beyond the AVM, which supplies the parenchymal compartment, is blocked through placement of an endovascular ligature (fibered coils). This protects the post lesional parenchymal tissue and isolates malformational compartments before embolization. Thus the proximal AVM-supplying segment of the VeP can be safely embolized. PATIENTS AND METHODS: Five of 204 AVM patients admitted for preoperative embolization between 1989 and 1997 fulfilled the following treatment criteria for the placement of an endovascular ligature in a VeP before embolization: 1. The diameter of the distal portion of the VeP behind the AVM was large whereas the parenchymal blush was poor; 2. The VeP fed a large portion of the AVM; 3. The VeP was judged to be accessible only late in the surgical procedure; 4. The VeP and its off branches were an integral part of the AVM periphery and thus not suitable for microdissection. RESULTS: In all five cases the leptomeningeal collateral perfusion (the arterial supply to parenchymal brain areas) served to supply brain areas distal to the AVM after primary blockage of a VeP by endovascular ligature with fibered coils. Embolization and complete surgical dissection of the AVM was then achieved in all cases. No neurological deficits occurred. CONCLUSION: Experience with our five cases indicates that a preparatory endovascular ligature of a VeP between parenchyma and the malformational compartment followed by embolization of the AVM can serve as an alternative to open surgical dissection of a vessel en passage and that it safely allows effective preoperative embolization.

Outcome after endovascular treatment of Hunt and Hess grade IV or V aneurysms: comparison of anterior versus posterior circulation.

BACKGROUND AND PURPOSE: The most common cause of poor treatment outcome in patients suffering aneurysmal subarachnoid hemorrhage is cerebral vasospasm, especially in cases of poor Hunt and Hess grades (IV and V). A further prognostic factor in surgically treated patients is aneurysm localization. The aim of the present retrospective study is to compare the endovascular treatment outcome in such poor-grade patients according to aneurysm localization in either the anterior (AC) or posterior (PC) circulation. METHODS: Forty poor-grade patients admitted between 1993 and July 1998 were treated by endovascular approach within 23 days after aneurysm rupture. Eighteen had aneurysms in the AC, 22 in the PC. Mean treatment delay was 4 days after rupture and median, 2 days. One patient showed multiple aneurysms. In 36 cases, aneurysms were occluded by Guglielmi detachable coils; in 4 cases, by parent vessel balloon occlusion. RESULTS: The incidence of delayed ischemic neurological dysfunction or cerebral infarct due to vasospasm did not differ significantly between the AC and PC groups. Two procedure-related complications with clinical effect were observed in each group. At 6 months' follow-up, the result was good in 5 patients and poor in 13 in the AC group and good in 11 patients and poor in 11 in the PC group. CONCLUSION: Given comparable incidence of vasospasm in poor-grade patients, a tendency toward better treatment outcome was found in patients with aneurysms in the posterior circulation (chi(2)=2.04; P=0.15) than in the anterior circulation. Endovascular therapy for poor-grade patients is recommended, as are further studies to determine treatment differences.

Preoperative embolization of intracranial meningiomas with a fibrin glue preparation.

BACKGROUND AND PURPOSE: Preoperative embolization expands the spectrum of meningioma that can be operated on safely. Our goal was to achieve the distalmost loading of the vascular bed and confluent tumor necrosis with a fibrin glue preparation in the preoperative embolization of meningiomas. METHODS: Between 1992 and 1997, 80 patients with a meningioma had diagnostic angiography with a standard transfemoral Seldinger technique, performed with a 6F guiding catheter and digital subtraction angiography. Preoperative embolization was carried out in the same session with an additional microcatheter system. Fibrin glue was the only component used. In all cases, CT was performed immediately after embolization; in nine patients, MR imaging was also performed. RESULTS: Angiography verified the elimination of tumor blush in all patients. The high-density areas seen on postembolization CT scans, caused by the fibrin glue dispersed in the embolized supply area, were found to be necrotic at surgery and were easily removed by suction. Two (2.5%) of the 80 patients had complications associated with embolization that resulted in neurologic deficits. CONCLUSION: The most effective preoperative embolization of tumors requires a distalmost loading of the vascular bed. Fibrin glue, which is easy to use and safe to handle, causes confluent tumor necrosis within the injected vascular territory.

The predictive value of early CT and angiography for fatal hemispheric swelling in acute stroke.

PURPOSE: Our goal was to analyze the predictive value of early CT and arteriographic morphologic criteria to achieve a more reliable prediction of fatal outcome in patients undergoing fibrinolytic stroke treatment. METHODS: In 74 patients with acute carotid artery stroke, early signs of cerebral ischemia were determined by CT. The site of vascular occlusion was identified by digital subtraction angiography (DSA). The patients were subsequently treated by intraarterial (n = 68) or intravenous (n = 6) fibrinolysis by means of recombinant tissue plasminogen activator (rt-PA), urokinase, or rt-PA combined with lys-plasminogen and followed-up for a period of 3 months. CT and DSA data were compared with the clinical course, with special emphasis on signs of early fatal deterioration (ie, death by intracranial mass effect) as determined by corresponding CT and clinical observations, occurring within 7 days after stroke. RESULTS: Seventeen patients died, all of intracranial mass effect, and all within a week after stroke. In nine of these fatalities, DSA revealed carotid "T" occlusion (CTO), which affected 19 patients. In five of the fatalities, a major early sign of ischemia (MESI, referring to cortical hypodensity in more than a third of the territory of the middle cerebral artery, as seen in 14 patients) was recognizable on the initial CT scan. This led to a higher predictive value and sensitivity of CTO relative to MESI for estimating early fatality. CONCLUSION: CTO as determined by DSA is a substantially better predictor of fatal outcome in patients undergoing intraarterial thrombolytic therapy than is MESI as determined by CT.

[Lateral cranial dural fistula. Detection with Doppler and duplex ultrasound].

Cranial dural fistulae are rare; when they occur, it is usually difficult to detect them at an early stage. With a view to the question of possible progress in diagnosis we now report on seven patients with lateral dural fistulae fed by branches of the external carotid artery. The examination was carried out before selective arteriography using cw-Doppler sonography and colour coded duplex sonography in combination. Sonographic criteria for detection of hyperperfusion take account of flow velocity as well as pulsatility. In all cases hyperperfusion of the external carotid artery was detected. In most of these cases pathologic findings were also observed at the occipital artery, and more rarely in the contralateral external carotid artery or the ipsilateral vertebral artery, in addition. A possible source of error arising from confusion of blood vessels was present with the cw-Doppler sonography, but not for colour coded duplex sonography. Therefore, cranial dural fistulae characterized by a high shunt volume can be diagnosed correctly by indirect Doppler sonographic criteria using cw-Doppler and duplex sonography. Direct visualization of the fistula and its nidus requires additional selective arteriography, in the course of which endovascular embolization may be performed.

[Asymptomatic cerebral aneurysms--surgical and endovascular therapy options].

Aneurysmal subarachnoid hemorrhage carries a risk of 50% mortality. Therefore it is recommended to also treat asymptomatic, previously unruptured aneurysms. Deciding whether to treat or to observe depends on the consideration of risk of hemorrhage and the surgical risk. Such decision could be facilitated if predisposing factors affecting the outcome were known. 15 Patients with 19 unruptured aneurysms managed in a period when both, surgical and endovascular treatment options were available, were selected from a group of 47 asymptomatic patients documented since 1984. The patients were either operated (n = 9) or were treated by endovascular occlusion with GDC coils (n = 6). In most cases, a long history of headaches eventually lead to a CT-scan or MRI in which a suspicious abnormality was found. Upon subsequent angiography, 8 middle cerebral, 6 carotid, 3 basilar and 1 pericallosal, and ophthalmic artery aneurysm(s), respectively, were found. The aneurysms varied from 4 to 30 mm in diameter. The patients were followed between 9 months and 2.5 years. Treatment results in this small series were independent from size, location, age or sex of the patient. The initial, mostly unrelated symptoms persisted after treatment in most cases. The morbidity according to GOS in this small series (3 out of 19 patients deteriorated) is not representative of the complete series where morbidity and mortality was 5.4% and 1.2% respectively). In agreement with the literature reviewed herein, we recommend surgical treatment of previously unruptured aneurysms, especially when the patients are below 60 years of age. In patients with posterior circulation aneurysms, endovascular coil occlusion is a valid alternative, especially in older patients with concurrent medical problems.
