Physiological profiles and therapeutic goals--graphical aids support quick orientation in intensive care.

Although present-day technical facilities and modern patient monitoring produce an overwhelming number of measured parameters in intensive care medicine, there is a great need to develop decision aids to enable ICU personnel to become quickly orientated in the pathophysiological state of the patients under their care. One possible way for the representation of a multidimensional pattern of physiological values is the graphical display of a physiological profile of a patient's state. A combination of this display and the concept of 'therapeutic goals' is well suited for reducing the complexity of the computer output and for aiding in the necessary decisions in a twofold manner: substantial deviations of a single parameter are instantly visible and may be checked for their causes, the physician obtains an impression of the most serious deviations from the intended therapeutic goal for his patient. So she/he can act to support the patient in achieving the most likely course for recovery.

[Growth chart for ages 0 to 3 years].

The body mass and body length of 84 boys and 91 girls were investigated from birth up to the age of three years. From these data growth charts were drawn up. The development of body mass and body length related to chronological age, on the one hand, and the development of body mass related to body length, on the other hand. were set out. The growth charts are recommended as standards for reference purposes.

[Errors in the determination of oxygen consumption using the analysis of respiratory gases].

Analytical investigation of errors showed that the noninvasive determination of oxygen consumption (VO2) from the analysis of respiratory gases fails in giving as reliable results as expected in view of the small measuring uncertainties of internationally available analyzers. Absolute errors of delta FO2 = +/- 0.002 for oxygen fractions and delta FCO2 = +/- 0.001 for carbon dioxide fractions already cause the clinically just tolerated error limits of +/- 20% deviation from the true VO2 value to be exceeded under normal conditions. The approximation formula VO2 = VE (FIO2 - FEO2) gives the most reliable results for low inspiratory oxygen fractions (FIO2). It leads, however, systematically rising with FIO2, to an underestimation of VO2. Assuming 0.7 less than or equal to R less than or equal to 0.9 (R = respiratory quotient), the maximum relative error of VO2 can be limited to 21% for air (FIO2 = 0.21) and 30% for FIO2 = 1 by an appropriate correction factor K using the equation VO2 = VE (K.FIO2 - FEO2). For VO2 greater than or equal to 250 ml/min with VE = 10 l/min, K = 1.006 is suggested.

Experimental investigations in adult respiratory distress syndrome (ARDS). Repeated pulmonary lavage in LEWE-mini-pigs. I. Pulmonary mechanics, gas exchange, and pulmonary hemodynamics.

Disturbed water balance at the alveolo-capillary membrane resulting in decreased pulmonary compliance is one of the functional characteristics in the Adult Respiratory Distress Syndrome (ARDS). In artificially ventilated LEWE Mini Pigs repeated pulmonary lavage was performed. The aim was to produce a state like ARDS by affecting the lung alveolar surfactant system which is included into the sum of factors which are responsible for the normal water balance of the alveolo-capillary membrane. This condition was supposed to be maximized if the lung compliance reached its lowest value after pulmonary lavage. Regarding this the pathophysiologic changes in this ARDS model were characterized by increased airway resistance and dead space ventilation as well as decreased arterial oxygenation. Severe ventilation perfusion abnormality was represented hemodynamically by the increased pulmonary vascular resistance and the decreased pulmonary non-shunt blood flow. The morphological (macroscopic, light-and electronmicroscopic) changes indicate strongly that in this type of ARDS model all functional compartments of the alveolocapillary membrane are severely affected. Decreased colloid osmotic pressure of blood plasma samples underlines the severity of changes in the water balance at the alveolo-capillary membrane in this type of lung injury.

Application of bromhexine metabolite VIII (NA 872) and bromhexine-glucose in treatment of adult respiratory distress syndrome; experimental and clinical results .

The authors study the changes in cardiopulmonary function after application of bromhexine metabolite VIII (NA 872) in an animal model of adult respiratory distress syndrome (ARDS) and bromhexine glucose solution (BGS) in ARDS patients. After damage of the lung alveolar surfactant by intratracheal atomization of 0.2 M HCl in LEWE Minipigs significant changes in lung compliance, dead space ventilation, venous admixture, alveolo-arterial oxygen tension difference, pulmonary vascular resistance, and oxygen uptake are present under application of NA 872 in comparison to untreated animals. In ARDS patients treated with BGS, pulmonary function is significantly ameliorated indicated by changes in static effective compliance, oxygen exchange ratio, and alveolo-arterial oxygen tension difference.

Gas exchange, pulmonary mechanics and haemodynamics in adult respiratory distress syndrome: experimental results in Lewe miniature pigs.

Adult respiratory distress syndrome (ARDS) is a common medical emergency in respiratory care complicating a great variety of traumas and diseases. An animal model from Lewe miniature pigs has been developed to study the ARDS under standardized conditions; it is based on aspiration pneumonitis, a disorder often observed in ARDS, injuring the lung alveolar surfactant system. The experimental study was conducted under neuroleptanalgesia. ARDS was produced by intratracheal application of hydrochloric acid (0.2 mol/l) in an amount of 1.0 ml/kg body wt. The animals were ventilated automatically by a standardized ventilatory pattern in IPP mode. In all animals the time course of oxygenation ratio (Pa,O2/F1O2), arterial CO2 tension (Pa,CO2), ratio of alveolo--arterial oxygen tension difference to inspired oxygen fraction (Aa,DO2/F1O2), oxygen exchange ratio ((AaDO2/Pa,O2), lung compliance (CL), inspiratory airway resistance (RrsI), dead space ratio VD/VT), pulmonary artery pressure (PAP) and systemic blood pressure were studied. Changes in quasi-static volume--pressure curves, percentage change in lung water content and gross pathological finding were used to integrate the findings into a system of pathophysiological changes in ARDS. The animal group to which hydrochloric acid was administered shows severe pulmonary distress leading to death within 3.5--7.5 h. No significant changes in the measured parameters could be observed in the control group over a 14 h period. The results suggest that aspiration pneumonitis in Lewe miniature pigs is very suitable to investigate various problems in pathogenesis of ARDS. The model provides reproducible results which correlate very well with findings in different ARDS states. The models serves both to compare clinical states and to search for newer therapeutic manoeuvres.

[Relationship between social characteristics and behavior in pregnancy].

From 1973 to 1975 five hundred Berlin women have been questioned in puerperum. The authors seeked for possible differences in pregnancy behaviour during planned and unplanned gravidity. 40% of the women had an unplanned pregnancy. A higher fraction of unplanned pregnancies was found in young, unmarried women being under education, women without profession, plurigravids, and those who had never taken the anti baby pill.--In 5 of 8 parameters there were no differences in behaviour between planned and unplanned pregnancies (nourishment, use of drugs without doctor's order, coffee, alcohol, cohabitation near term).--In the group of planned pregnancy more mothers drank sufficient quantities of milk, read books about pregnancy and delivery, and smoked less. But the differences are small. Therefore the authors don't believe that fact of unplanned pregnancy is a considerable factor of risk for fetus and newborn.
