Amphotericin B and liver function.

Amphotericin B is well established as a highly efficacious agent against systemic fungal infections in humans. The therapeutic potential of amphotericin B is limited due to its side effects. Although in clinical use for more than 35 years, impairment of liver function is not considered to be a typical adverse effect of amphotericin B. Experimental data suggest that the drug may interfere with the hepatic cytochrome P450 and may thus influence the metabolic capacity of the liver. A confirmation of such a finding in patients treated with amphotericin B would be of value. The incidence of severe acute or subacute hepatotoxicity in response to amphotericin B is very low. Frequently, in critically ill patients, it is not always clear whether liver abnormalities are caused by an antifungal agent or whether they are due to the critical condition of these patients. Nevertheless, there are experimental data suggesting that amphotericin B may influence the metabolic capacity of the liver. Accordingly, drug interactions during prolonged amphotericin B treatment seem possible. Careful monitoring of liver function in those patients receiving amphotericin B in combination with other drugs, which undergo hepatic metabolism or are potentially hepatotoxic, is recommended. In this review, the current understanding and knowledge of the clinical significance, detection, and possible pathogenesis of amphotericin-B-induced liver damage are presented. With respect to the current experimental data, the influence of the drug on the hepatic microsomal cytochrome P450 are also presented and discussed, as is the impact of several clinical studies using different amphotericin B formulas in humans.

[Education in psychosomatic medicine and psychotherapy specialty: evaluation by students].

In the context of evaluation, experiences, attitudes, expectations and assessments concerning teaching psychosomatic medicine and psychotherapy were examined. For that purpose students of the winter term 1993/94 were given a questionnaire before and after the practical course. The students had little experience with the subject. They described low expectations regarding the training. However, they showed a very positive attitude towards the subject. After the course, they judged the training positively. In particular, the affective-subjective contents, considered as being typical for the subject, correlated with positive judgements of the teaching. Thus, in may be concluded that these aspects may be conveyed to the students by means of a Balint-like method of teaching and that these aspects are accepted by the students.

[The clientel of a psychotherapeutic polyclinic in the light of a documentation base].

A documentation system for use during the diagnostical process with psychotherapy outpatients is outlined, and sociodemographic, medical and psychosocial data of newly admitted patients of a university psychotherapy outpatient department are presented. The population structure is described on the basis of data collected during the first 18 months of the implementation of the assessment system. Data are compared with those of other psychotherapeutical institutions that differ with regard to setting (outpatient vs. inpatient), health care sector (acute care vs rehabilitation) and geographic region. Sociodemographic and medical data are given. Patients' mean age is 33.5 years. Two-thirds of the patients are female. The majority is single and has higher formal education. Slightly more than half of the patients are working. Modes of admittance, somatic and psychological complaints and aspects of illness behaviour are described. Pertaining to diagnostical classification, affective disorders (34%), somatoform disorders (15%) and anxiety disorders (11%) are most frequent. Mean duration of symptoms is 6 years. However, only few patients had prior psychological treatment. Treatments most frequently selected at the time of assessment were psychodynamic psychotherapy (41%), behavioural therapy (24%), and group therapy (10%). When comparing the study population with those of other institutions, differences showed up especially in contrast to rehabilitation clinics, where older, male, and working persons as well as those with somatoform disorders and longer durations of illness are more frequent.

[Treatment of psychotic patients with music therapy].

The present contribution describes the experience taken from music therapy of psychotic patients. The emotional and cognitive music perception and its possible influence on self perception and strengthening of ego are discussed. Since exercise instructions were limited the observed improvement of communication seems rather due to intra- and interpersonal effects of active improvisation than to a training process. With regard to schizophrenic patients possible effects of music therapy are discussed in the light of self-object-differentiation.

[Emotional experience, expression and communication in musical improvization. A qualitative-quantitative single case study].

In this study the theory-conducted empirical procedure for analyzing active improvisatory music therapy of a single case will be described. The operationalization and tests for validation will be described. According to the aim of the study, the results show that the used method is appropriate to describe different and repetitive (emotional) patterns of improvisatory playing, which reflect the personal (emotional) and interpersonal pathology and healthy parts of the examined patient. With the aid of several tests the results of validating the macro-process will be discussed.
