Endoprosthetic arthroplasty of the ankle joint. A clinical and radiological follow-up.

Eighteen ICLH ankle arthroplasties in 16 patients were followed up 15 to 52 months postoperatively (mean 36 months) by a review of the records, and clinical and radiological examinations. Five arthroplasties were performed for osteoarthrosis, 13 for rheumatoid arthritis. The overall clinical result was rated excellent in 2, good in 8, fair in 6, and poor in 2 joints. In osteoarthritic joints the results were somewhat poorer, no patient obtaining a rating of excellent but 2 of good, 2 of fair, and one of poor. Radiolucent zones greater than 2 millimeters were seen around the tibial component in 7 cases. Loosening defined as radiographic signs of movement between the prosthetic components and bone was present in 4 cases. The high occurrence of obvious loosening and large radiolucent zones indicates that mechanical problems will be encountered frequently in the future. From the results of this study it is concluded that ankle arthroplasty has a definite place in the treatment of severe arthritis in rheumatoid patients.

Auto-traction for treatment of lumbago-sciatica. A multicentre controlled investigation.

In this controlled prospective study of the Auto-traction method for the treatment of lumbago-sciatica, 82 patients were randomly allocated to either treatment with Auto-traction for up to three 1-hour sessions in 1 week, or they were given a corset and advised to rest. The orthopaedic surgeons participating in the study worked at six different hospitals and all had limited experience of the Auto-traction method obtained during a 1-week course. All patients were clinically evaluated by an independent observer who also performed the follow-up examinations 1 and 3 weeks after the treatment sessions. In addition a 3-month follow-up was performed by letter. The Auto-traction method gave prompt relief of pain and a normalizing of the SLR test more often than treatment with only a corset and rest. The difference between the two treatment groups was statistically significant. The immediate difference noted between the treatment groups had decreased slightly at 3 weeks but was still statistically significant at this time.

Frequency of dividing cells, a new approach to the determination of bacterial growth rates in aquatic environments.

Frequency of dividing cells is suggested to be an indirect measure of the mean growth rate of an aquatic bacterial community. Seasonal changes in frequency of dividing cells were found which covariated with the bacterial uptake of C-labeled phytoplankton exudates. Batch and continuous culture growth experiments, using brackish water bacteria in pure and mixed enrichment cultures, were performed to establish a relationship between frequency of dividing cells and growth rate. An improved technique for bacterial direct counts, using fluorescent staining and epifluorescence microscopy, is presented. Based on a 6-month survey in a coastal area of the Baltic Sea, the bacterial production in the photic zone is estimated. Compared to the total primary production in the area, the bacterial population during this period utilized approximately 25% of the amount of carbon originally fixed by the primary producers.

Subclinical protein-energy malnutrition in under-privileged Ethiopian mothers and their newborn infants.

An increased glycine/valine ratio in the maternal and cord vein plasma was observed in under-privileged women and their newborn babies in Ethiopia. There was a delay in the decrease of branch-chained amino acids during the immediate neonatal period. These findings indicate malnutrition in utero and are identical with those of a previous study in an under-privileged group in Pakistan. A reduced cord/maternal ratio of tyrosine and methionine was found in the Ethiopian material, indicating placental dysfunction of unknown origin.

Partial amputation of the foot for diabetic or arteriosclerotic gangrene. Results and factors of prognostic value.

One hundred and eighty-three conservative amputations of some part of the foot in 161 patients with gangrene from diabetes or arteriosclerosis have been studied retrospectively. They constituted 48 per cent of all amputations in one orthopaedic service over a period of twelve years, during which the minimal feasible procedure was always chosen. Sixty per cent healed soundly, but in over a third of these cases at least one revision to a higher level on the foot had been required. Factors that significantly influenced the outcome of the initial operation were the level of amputation, the age of the patient, the interval between the onset of gangrene and operation, anaemia and pyrexia.

Morphology of dentin surfaces in prepared cavities.

Rotating instruments produced cleaner cavity surfaces at 400,000 rpm than at 200,000 rpm. And tungsten carbide burs left cleaner surfaces than did the diamond instruments.
