[Diseases of the shoulder joint from the orthopedic viewpoint].

The cardinal symptom in all shoulder disorders is shoulder pain. We have to differentiate between genuine shoulder pain originating from the glenohumeral joint and its periarticular structures and referred pain originating at a site distant from the shoulder joint, e.g., the cervical spine or the internal organs. The differential diagnosis of genuine shoulder pain is discussed. The options open to the radiologist for reaching a correct diagnosis in different sorts of shoulder pain are shown.

Arthritis↗

[CT diagnosis of soft tissue injuries in habitual shoulder dislocation].

Computed tomography can demonstrate most glenohumeral structures accurately. If the articular cavitz is filled with air, CT ist the method of choice in preoperative diagnosis to determine the capsular extension and to assess the destruction of the glenoid bone rim and glenoid labrum.

Arthrography↗

[CT diagnosis of recurrent subluxation of the shoulder (author's transl)].

CT provides a clear, unobstructed and reproducible view of the shoulder joint. This permits, unlike conventional methods, for the first time a complete view of the anatomy and of the factors which predispose to a recurrent subluxation of the shoulder. In addition it is possible to see bone lesions resulting from the subluxation.

Chronic Disease↗

The so-called "periarthritis humeroscapularis"--possibilities of an operative treatment.

The pathological alterations of the periarticular tissue in the differential diagnostics of so called periarthritis humeroscapularis are demonstrated. Their pathological mechanism is explained briefly. Clinical and radiological methods to differentiate the pathological symptoms are described. It has been tried to define the indication and the exact date for an operative intervention. Those operative treatments are demonstrated which allow the expectation of good results in clinical practice.

[Treatment of the combined musculus trapezius/serratus anterior paresis with scapulothoracal arthrodesis according to Spira (author's transl)].

The combined paresis of the musculus trapezius and musculus serratus anterior results in a positional change and major loss of active mobility of the shoulder girdle. This greatly disturbs the mechanics of movement of the upper arm. The individual signs include a lowering of the shoulder blade in ventro-caudal direction in accordance with the weight of the arm, as a result of the absence of muscular restraint. In this position, the scapula exercises so to say an adducting action on the upper arm, so that this will usually not exceed an abduction of about 40 degrees, in relation to the perpendicular line of C7, althoug it may be freely mobile in the glenohumeral joint. Increase of elevation of the upper arm and of the force exercised by the musculature acting on the glenohumeral joint can be achieved via a correction of the position of the shoulder girdle associated with simultaneous stabilization of the shoulder blade at the posterior thoracic wall. Using an actual case as basis, scapulothoracic arthrodesis in the modification according to Spira is described as a method to correct the position of the shoulder blade and to stabilize the scapula. Other possibilities of stabilization in the form of musculature transpositions and paresis operations are discussed. The article also comments on the indication of scapula fixation.

[Biomechanical problems in acromio plasty of the shoulder joint (author's transl)].

In the confines of the spatium subacromiale, the anatomic structures are subjected to increased wear by friction and convolution. In case of compression syndromes relief is obtained by a partial resection of the acromion, by an acromio plasty, or by a complete removal of the acromion, i.e., the acromionectomy. By analysing the anatomic preparation we have found that a resection of the ligamentum coraco-acromiale and the anterior third of the acromion can give a sufficient decompression without any substantial disturbance of the mechanism of the shoulder girdle, while the acromionectomy implies an unnecessarily extensive procedure which involves considerable disadvantages for the biomechanics of the shoulder girdle.

Acromioclavicular Joint↗

[Paresis of the brachial plexus: subcapital varus osteotomy, arthrodesis of shoulder joint and above-elbow amputation (author's transl)].

In brachial plexus paresis with partial sensory sparing in the upper arm and complete motor paralysis we amputate through the humerus at the distal limit of sensation. The remaining proximal humerus is fixed by an arthrodesis of the shoulder joint, combined with a varus-osteotomy below the head. This increases with axillary space, facilitates the fitting of a prosthesis and improves care of the skin in this critical area. It also improves the outline of the shoulder the muscles of which shrink.

Amputation, Surgical↗

[Congenital upward displacement of the shoulder blade].

Congenital upward displacement of the scapula is typified by changes in the shape, position, and attitude of the scapula; in a high percentage of cases it is associated with other malformations of the axial skeleton and the internal organs. As a result of the malposition of the scapula and its insufficient mobility in the scapulocostal joint the ability to raise the upper arm is limited. The need for treatment of this condition derives on the one hand from cosmetic considerations and on the other from the limitation of the mobility of the upper arm. However, in addition to this, the diagnosis of Sprengel's deformity must also lead one to look for accompanying malformations which may have much more serious consequences than the simple upward displacement of the scapula. Surgery represents the only effective form of treatment for Sprengel's deformity. The indication for surgery is based on a classification by degrees, taking both the functional loss as well as the conspicuousness into consideration. The choice of surgical procedure will depend on the severity of the malformation and the age of the patient.
