Human pharmacological investigation of a human recombinant tumor necrosis factor preparation (PAC-4D) a phase-I trial.

15 patients aged between 24 and 66 years with 10 different malignant tumor diseases were treated with a recombinant human tumor necrosis factor preparation PAC-4D in a phase-I trial. The starting dose was 10(5) U PAC-4D as an intravenous short infusion. The maximally tolerable dose is around 18 X 10(5) U/m2. As the main clinical side effects were observed: fever, chills, hypertension with subsequent hypotension, lethargy, transient somnolence, headache, neurological deficiency symptoms, nausea and vomiting. Important laboratory-chemical parameters were the increase in transaminases and, in higher dose levels, leukocytosis with the left shift and lymphopenia in the differential blood picture. As dose-limiting toxicity are estimated hypotension, and neurological side effects and hepatotoxicity. In one female patient who received 27 X 10(5) U PAC-4D there appeared pronounced, histologically verified necroses in the metastases of a malignant fibrous histiocytoma.

[Fulminant hepatitis in Listeria septicemia].

We report on a 44-year-old male patient admitted with acute severe icteric hepatitis. Listeria monocytogenes was isolated from blood cultures. The further course was complicated by meningitis and the patient finally died of multiorgan failure. Autopsy revealed nodular cirrhosis of the liver with cholestatic hepatitis and focal subacute liver dystrophy, as well as granulating meningitis. The case is discussed in the context of five previously published observations on hepatitis due to Listeria monocytogenes.

[Treatment of peritonitis during continuous ambulatory peritoneal dialysis (CAPD) with co-trimoxazole, cefazolin or vancomycin].

Three initial treatment schedules of peritonitis during continuous ambulatory peritoneal dialysis are analysed. In 20 patients 56 episodes of peritonitis were treated by co-trimoxazole, 29 episodes in 20 patients by cefazolin, and 29 infections in 22 patients by vancomycin. The efficiency of the treatment modes was comparable. Vancomycin was found to be appropriate in particular because of the resistance characteristics of bacterial isolates.

Amino-terminal deletions in the presequence of an imported mitochondrial protein block the targeting function and proteolytic cleavage of the presequence at the carboxy terminus.

Subunit IV of yeast cytochrome oxidase is made in the cytosol with a 25-residue presequence. This presequence targets subunit IV into mitochondria and is removed by a protease in the matrix space. Here we show that removal of as few as 4 amino-terminal residues from the subunit IV presequence (which had been attached to the cytosolic protein dihydrofolate reductase) blocks import of the protein into mitochondria and proteolytic removal of the presequence by the soluble matrix protease. Thus, this protease requires not only an appropriate cleavage site at the carboxy-terminal end of the presequence, but also information at the extreme amino terminus of the presequence.

[Allopurinol-induced kidney failure with hepatitis and squamous dermatitis in pre-existing kidney insufficiency].

We report on a female patient with analgetic nephropathy in whom allopurinol therapy was started because of asymptomatic hyperuricemia and who 2 months later developed a syndrome characterized by icteric hepatitis, exfoliative dermatitis and progressive renal failure. After discontinuation of allopurinol and temporary peritoneal dialysis the patient recovered from the initially threatening condition. Analysis of case reports from the literature indicates that this syndrome is due to the allopurinol metabolite oxypurinol and is most frequently observed in patients with renal failure on concomitant treatment with diuretics.

Y-specific DNA sequences in male patients with 46,XX and 47,XXX karyotypes.

Y chromosomal DNA sequences were detected in three of four 46,XX males and in one 47,XXX male. One reiterated Y chromosomal sequence, Y-190, was localized by in situ hybridization to the distal short arm of an X chromosome of the 47,XXX male. This result is compatible with the hypothesis that an aberrant X/Y interchange has occurred, most likely during paternal meiosis, and that this interchange accounts for Y chromosomal material and sex reversal in this 47,XXX individual.

Orientation and molecular map position of the complement genes in the mouse MHC.

Over the past few years six gene clusters have been isolated from the major histocompatibility complex (MHC) of the BALB/c mouse encompassing a total of 1600 kb of DNA and 48 genes. The molecular distances between these gene clusters and the orientation of four of the six clusters on chromosome 17 is not known. Here we use pulse-field gradient gels and Southern blot hybridization to establish large-scale genomic restriction maps covering several hundreds of kb surrounding the three gene clusters located in the K, I, S, and D regions of the MHC. Comparison of the maps orients the complement gene clusters in the S region with the 21-OHB gene pointing towards the K end and the C2 gene pointing towards the D end of the MHC. The distances between the E alpha and 21-OHB genes is 430 kb and between the C2 and TNF-alpha genes at least 420 kb.

Closed rat jejunal segment in situ: role of pre-epithelial diffusion resistance (unstirred layer) in the absorption process and model analysis.

After intraluminal injection of 0.5 ml buffer solution into closed jejunal segments (length, 3-5 cm) of anesthetized rats the appearance rates of a series of labeled substances in jejunal venous blood were measured for 30 min in situ (initial concentration, 0.02-10 mmol/l or 1 GBq/l tritiated water). The appearance rates quickly rose to a maximum and then declined almost exponentially. Model analysis of the descending of branch of the curves by two one-compartment models (perfect luminal mixing, radial diffusion without convection) revealed a relative pre-epithelial diffusion resistance of nearly 100% for benzoic acid, salicylic acid, L-lysine (0.02 and 1 mmol/l), alpha-methyl-D-glucoside, and L-phenylalanine; 80% to 95% for aniline and butanol; 50% to 80% for benzyl alcohol, theophylline, aminopyrine, antipyrine, dodecanol, and D-galactose; approximately 40% for tritiated water; approximately 30% for L-lysine (10 mmol/l); 10% to 20% for urea and benzylamine; and approximately 4% for erythritol. The shape of the curves was well described by a two-compartment model (intestinal lumen and "tissue", radial luminal diffusion without convection); the variability of the data, however, prevented closer analysis of the parameters of this model. Since pre-epithelial diffusion (unstirred layer) in the closed jejunal segment without peristalsis is the rate limiting step in the absorption process of highly permeant substances, information on intestinal epithelium can be obtained only with poorly permeant substances.

[The transition of morantel in cow's milk following administration in the retard form, "Paratect Bolus" (Pfizer)].

The transition of Morantel into bovine milk was studied with three lactating cows. The active compound was administered as Paratect Bolus which continuously releases the anthelmintic agent. No residues of Morantel could be detected throughout the 92-day experiment (limit of detection: 0.5 micrograms Morantel tartrate/kg milk), nor could the compound be found in the blood serum at day 35 of the experiment. The same day an amount of 825 micrograms/kg was observed in faeces.

Galactosyltransferase and sialyltransferase are located in different subcellular compartments in HeLa cells.

Galactosyl- and sialyltransferase have been localized by double immunofluorescence labeling in HeLa cells. Galactosyltransferase was found in a compact juxtanuclear structure previously shown to represent the Golgi apparatus (J. Roth and E.G. Berger (1982) J. Cell Biol. 93, 223-9), whereas sialyltransferase was localized to vesicles spread over the whole cytoplasm. These findings indicate different compartments for both transferases and support a model of subcompartmentation of glycosylation steps along the secretory pathway.

A double-blind study comparing monomethoxy polyethylene glycol-modified honeybee venom and unmodified honeybee venom for immunotherapy. I. Clinical results.

Thirty-five patients were allocated at random to immunotherapy (IT) in a double-blind way with either monomethoxy polyethylene glycol (mPEG)-modified honeybee venom (HBV) or HBV. The two groups were well matched regarding age, sex, skin sensitivity, HBV-specific serum IgE and IgG antibodies, and history of reactions after a field sting; mPEG-HBV-treated patients received doses that increased more steeply than doses of the HBV-treated patients. The maintenance dose of the former group (200 micrograms) was greater than that of the latter group (100 micrograms). During IT, both groups had the same frequency of local swellings after injections. Four patients receiving mPEG-HBV developed one mild systemic reaction (SR) during dose increase, whereas 10 patients receiving HBV demonstrated one or more of these reactions, compelling two patients to stop therapy. Following challenge with a honeybee sting after about 14 weeks of IT, six patients with SR were observed, four in the mPEG-HBV-treated group and two in the HBV-treated group. In the HBV-treated group, three patients were not challenged, one because of an insufficient IgG increase and two other patients because they dropped out of IT before reaching maintenance dose because of repeated SRs. Since the mPEG-HBV is extremely well tolerated during IT and the success rate is not significantly lower than with unmodified HBV, we suggest it as an attractive alternative to HBV for the treatment of HBV hypersensitivity. Increase of the maintenance dose may result in an even better clinical efficacy.

Influence of imipramine and chloroquine on lung phospholipid content and lung structure in newborn rats.

In continuation of previous experiments in adult rats the alteration in content and composition of lung phospholipids (PL) and lung structure of newborn rats of different gestational age was studied after three administrations of the amphiphilic drugs imipramine (75 mg/kg b.w.) and chloroquine (50 mg/kg b.w.). The fetuses were obtained by Cesarean section on day 20, 21, or 22 of gestation. Morphological examinations of the lungs of drug treated 21-day-old fetuses revealed a substantially better aeration than nontreated controls. 20 and 22 days old fetuses showed no differences in aeration between drug and saline treated rats. Analogous results were obtained with respect to the 1 h survival rate of newborns. Furthermore, imipramine and chloroquine cause a premature development of the pneumocytes type II as shown by electron microscopy. An increased PL content as well as disaturated phosphatidylcholine (DSPC) and phosphatidylglycerol (PG) proportion of fetal lungs, especially in animals born on day 21 of gestation, were found after drug treatment. The results suggest an acceleration of maturation of the pneumocytes type II in fetal lungs induced by amphiphilic drugs.

Posttranslational modifications as a cause of gonadal polypeptide diversity in the rat.

The testicular and ovarian protein patterns of prepubertal rats were studied by two-dimensional gel electrophoresis after both in vivo and in vitro translation. Analysis of the approximately 750 polypeptide spots detectable in gonadal extracts (in vivo translated polypeptides) revealed 14 testis-specific, and 2 ovary-specific spots. Conversely, only 4 testis- and 2 ovary-specific polypeptide spots (of a total of about 600) were detected after in vitro translation of the respective polyadenylated RNAs using the reticulocyte lysate system. Simultaneous in vitro translation plus microsome-mediated cotranslational modification increased the number (of testicular spots to 8, and of ovarian spots to 3 (of a total of 600). The polypeptides synthesized in vitro were well comparable with respect to molecular weight and isoelectric point to those synthesized in vivo. Therefore, testicular polypeptide diversity is mainly the result of co/posttranslational modifications. Many of these proteins appeared to be glycosylated as indicated by their specific binding to Concanavalin A. The system used demonstrates that co/posttranslational modifications play a role in the establishment of polypeptide and thus organ diversity.

Fixation of the unmethylated or the 5'-CCGG-3' methylated adenovirus late E2A promoter-cat gene construct in the genome of hamster cells: gene expression and stability of methylation patterns.

The late E2A promoter of adenovirus type 2 (Ad2) DNA can be inactivated by in vitro methylation of three 5'-CCGG-3' sequences at positions +23, +5, and -215 relative to the cap site in this promoter. This inactivation has been documented in transient expression experiments both in Xenopus laevis oocytes and in mammalian cells (K.-D. Langner, L. Vardimon, D. Renz, and W. Doerfler, Proc. Natl. Acad. Sci. USA 81:2950-2954, 1984; K.-D. Langner, U. Weyer, and W. Doerfler, Proc. Natl. Acad. Sci. USA 83:1598-1602, 1986). In the present study, in vitro-methylated or unmethylated promoter-gene assemblies were permanently fixed by integration in the hamster genome. In individually established cell lines, the degree of promoter methylation was correlated to gene activity. The pAd2E2AL-CAT construct, in which the late E2A promoter controls expression of the procaryotic chloramphenicol acetyltransferase (cat) gene, was fixed in BHK21 hamster cells by cotransfection with and selection for the pSV2-neo construct (P. J. Southern and P. Berg, J. Mol. Appl. Genet. 1:327-341, 1982) in which the early simian virus 40 promoter controls the gene for neomycin phosphotransferase. The pAd2E2AL-CAT construct was transfected in the unmethylated or in the 5'-CCGG-3' methylated form. The pSV2-neo plasmid was cotransfected in the unmethylated form. The stability of in vitro-imposed methylation patterns and cat gene expression were followed and correlated in a number of established cell lines which contained the constructs integrated in a non-rearranged configuration. The foreign DNA did not persist in the episomal state but was integrated, frequently in multiple tandems of the plasmid DNA. Among 19 cell lines established after transfecting the unmethylated pAd2E2AL-CAT construct, the late E2A promoter remained unmethylated (examined in 10 cell lines), and the cat gene was expressed in 18 cell lines. On the other hand, among 14 cell lines which were generated by transfection with the methylated construct, 7 cell lines did not express the cat gene, and the three 5'-CCGG-3' sequences in the late E2A promoter remained almost completely methylated. In five cell lines, the E2A promoter sequences were partly demethylated and the cat gene was expressed at low levels. Last, in two cell lines, demethylations were found to be extensive and strong cat expression was observed. It remained a question of considerable interest what factors determined the stability of methylation patterns that had been preimposed by in vitro methylation on specific sequences in a promoter, after this promoter was fixed by integration in the mammalian genome.(ABSTRACT TRUNCATED AT 400 WORDS)

Highly polymorphic DNA sequences in the distal region of the long arm of human chromosome 18.

Highly polymorphic repeated DNA sequences were detected at the end of the long arm of human chromosome 18 by a recombinant DNA probe containing 56 bp of human DNA (pERT25). This was shown by hybridization of pERT25 to DNA from a panel of human X rodent somatic cell hybrids, by dot blot hybridization to flow-sorted human chromosomes, and by in situ hybridization to metaphase chromosomes. The high degree of polymorphism detected by this 18q DNA fragment makes it potentially useful for various applications, including investigations into the genetics of trisomy 18 (Edwards syndrome), linkage studies, and paternity testing.
