In vivo study of platelet kinetics by 75Se-methionine in different haematological disorders.

Labeling of platelets in vivo by 75Se-methionine was performed in premalignant and malignant haematological disorders for evaluation of the kinetics of platelet maturation. The "normal" platelet maturation time (time between the injection of the isotope and maximum radioactivity of separated blood platelets) in eight non-haematological patients showing normal platelet counts was 9.1 days. A shortening of platelet maturation time of 5-7 days was observed in three of four cases with panmyelopathy (high bone marrow cellularity), in three of four cases with malignant lymphatic disorders (multiple myeloma, chronic lymphocytic leukaemia, lymphosarcoma), and in two of four cases with myeloproliferative syndromes. No correlation to the peripheral platelet counts was observed. For explanation of the premature platelet release from the bone marrow a disturbance of the megakaryocyte maturation is suggested.

Serological investigations in hymenoptera sting allergy: IgE and haemagglutinating antibodies against bee venom in patients with bee sting allergy, bee keepers and non-allergic blood donors.

Specific IgE-antibodies and haemagglutinins to bee venom were determined in fifty-five bee sting allergic patients, fifty-seven bee keepers and fifty-two blood donors without evident allergy to hymenoptera stings. IgE-antibodies were detected by RAST in 70% of allergic patients, 40% of bee keepers and 12% of blood donors. Most bee keepers with detectable IgE-antibodies to bee venom recorded severe local or even general reactions to bee stings. Most blood donors with detectable specific IgE had been stung by hymenoptera in the past. High titres of haemagglutinating antibodies against phospholipase A were found in most bee keepers, occasionally in bee sting allergic patients but only rarely in blood donors. The determination of specific IgE-antibodies to insect venoms by RAST seems to be a valuable method for the diagnosis of hymenoptera sting allergy.

[Statement of the bioptic diagnostic in intrathoracic sarcoidosis--team work of 6 lung hospitals of the GDR (author's transl)].

A nearly nation-wide team-work of six chest hospitals was made with 4284 patients to analyse all performed diagnostic procedures up to the present. Its frequency and diagnostic importance in relation to the radiographic stages of the disease were examined. Especially the procedure of bronchologic examination, having been partly underrated with the morphological ascertainment of sarcoidosis, are now purposefully analysed. This showed that also with perbronchic punction-biopsy and bronchoscopic excision morphologically confirming of the diagnosis was possible at a high percentage. On the other side the right diagnosis was made clinically before bioptic procedures in the majority of the patients. The overall conclusion is that, according to the experiences made in our hospitals, the relatively harmless bronchologic procedures are in general sufficient to obtain bioptic verfication of the diagnosis "sarcoidosis". Other bioptic operations like mediastinoscopy or lung biopsy can be restricted to cases with reasonable doube of the diagnosis.

[Postoperative treatment after lung resection in old age (author's transl)].

177 lung cancer patients over 70 years old treated during the period from 1961 to 1975 were analyzed and evaluated for surgical resection. 109 cases were lobectomies, 39 nontypical resections and 29 were pneumonectomies. The postoperative mortality was 6.2%. Our concept of surgical treatment for patients with lung cancer over 70 years of age is as follows: Avoid pneumonectomy, prefer lobectomy or non-typical resection. The five years survival rate among 77 elderly patients amounted to 35%.

CO2 reduction to formate by NADH catalysed by formate dehydrogenase from Pseudomonas oxalaticus.

The direct reduction of CO2 to formate is catalysed by formate: NAD oxidoreductase in the presence of substrate amounts of NADH. Proof for this reaction is supplied by the detection of a CO2-dependent NADH oxidation, and by the identification of [14c] formate as the product of a NADH-dependent reduction of [14c]carbonate. The enzyme-catalysed CO2 reduction by NADH attains the equilibrium predicted by thermodynamic considerations, a state which is also reached from the formate side. The Michaelis constant for CO2 is about 40 mM indicating the low affinity of the enzyme for this substrate. The corresponding value for formate is 0.1 mM. Under the special conditions employed the enzyme catalyses the formate oxidation about 30 times faster than the CO2 reduction. That CO2 and not HCO3- is the active species in the reduction was shown by comparing the ph dependency of the velocities of the forward and back reactions and by observing the kinetics of CO2 reduction during the simultaneous attainment of the CO2-HCO3- equilibrium.

Originalarbeiten.

In 5 cases of polycythaemia vera and 2 cases with other myeloproliferative disorders accompanied by thrombocythaemia (megakaryocytic myelosis), the megakaryocytes were differentiated and studied by use of the combined application of cytophotometric determination of the DNA content and autoradiography with tritiated thymidine (3H-TdR) in vitro. A shift to the right of the megakaryocyte series, occurence of high polyploidy cells at 64c and a decrease of the 3H-TdR-labeling indices were observed. The data suggest a disturbance of the rhythmical polyploidization of the megakaryocytes, consisting of an elevated proportion of rest cells at the different ploidy stages. The maturation capacity of megakaryocytes may be related more to the resting than to the DNA synthesizing cells.

Fluorogenic substrate turnover in single living cells.

Intracellular diffusion properties and enzyme activities in single living cells can be analysed by means of fluorogenic substrates that diffuse into the cells where they are converted into a fluorescent product by an enzymic reaction. The reaction-kinetic analysis of this process as a system of consecutive reactions provides information on the diffusion of the substrate into the cells, on intracellular enzyme activities and on the efflux of the fluorescent product. Separation of diffusion and enzyme-mediated processes is obtained by inducing specific changes of the cellular membrane using gramicidin D. A model for the functional interpretation of the experimental findings is proposed. Application of this method as a viability test for freshly prepared and frozen platelets is discussed.

Proliferation characteristics of lymphoid cells of human bone marrow in health and in chronic renal failure.

Proliferation of bone marrow lymphoid cells of 8 healthy subjects and 7 patients with chronic renal insufficiency was compared using the cytophotometric measurement of the DNA content combined with 3H-thymidine labelling in vitro. In renal insufficiency the cells showed a marked decrease of the proliferation activity. This is apparent from the impaired labelling indices and accumulation of diploid and unlabelled cells. The data suggest that the lymphoid cell proliferation is affected by the same pathophysiological mechanism as that leading to impairment of red cell production. By subclassification according to cytoplasmic basophilia, this proliferation disturbance could be related to the basophilic and not to the pale type of lymphoid cells.

Serological investigations in 15 cases of bird fanciers disease.

Serological studies in 15 patients with bird fanciers disease are reported. Quantitative analysis of serum proteins showed elevation of IgG in almost every case. High IgE was observed in three patients with isolated asthmatic reactions. In four patients alpha1-antitrypsin was transiently diminished. Two of these had heterozygous deficiency of this protein (genotype MS and MZ). Precipitins against avian sera and avian droppings were found in all patients, but also in some exposed and nonexposed controls. Cross-reactions to avian antigens from different species were observed quite regularly in patients but not in controls. One case is presented suggesting that these cross-reactions may be of clinical importance. Precipitins against other inhaled antigens such as Aspergillus fumigatus, cereals or Micropolyspora faeni were observed significantly more frequently in patients than in controls. These precipitins showed no cross-reactions with those against avian antigens.
