Simultaneous heparin extracorporeal LDL precipitation and hemodialysis. First clinical experience.

Simultaneous heparin extracorporeal LDL precipitation (HELP)-dialysis was carried out at weekly intervals in six patients with end-stage renal failure, associated hyperlipidemia, and a high risk of premature atherosclerosis. Evaluating 135 single treatments, a mean acute total cholesterol/LDL reduction of 31% and 39%, respectively, was found, while clearance of urinary substances was comparable to that in regular hemodialysis treatment. Treatment tolerance was excellent and no derangements in albumin, hemodialysis parameters, or blood coagulation were detected.

Mitoxantrone: mechanism of action, antitumor activity, pharmacokinetics, efficacy in the treatment of solid tumors and lymphomas, and toxicity.

Mitoxantrone is similar to Adriblastin in its mechanism of action and antitumor activity. Objective remissions were obtained in 20-30% pretreated patients and in 23-44% of untreated patients by single-drug treatment of patients suffering from metastatic breast cancer. The objective response rates to Mitoxantrone in combination with CTX, 5-FU, MTX, VCR, MMC. Prednimustine or Vindesine were 16-46% in treatment and 38-89% in primary treatment. Randomized studies comparing Mitoxantrone with Adriblastin in single-drug and combination treatment did not show any significant differences in efficacy. However, Mitoxantrone was significantly less toxic. Remission rates of between 24 and 54% were achieved by single-drug treatment in pretreated patients suffering from non-Hodgkin lymphoma. Mitoxantrone appears to be active in ovarian cancer, lung cancer and hepatocellular carcinoma.

[Treatment of epicondylitis with locally injected orgotein (double blind study)].

The efficacy of locally injected orgotein in epicondylitis was investigated in a double-blind clinical trial. The results of the previously used method of treatment--injection with procaine hydrochloride--were applied as a comparison. In nearly all cases of acute epicondylitis, improvement was seen after orgotein injections. More than 70% of these patients were symptom-free 6 weeks after beginning therapy, a result significantly superior to that of the compared method. Both regimens produced distinctly less favorable results in chronic epicondylitis. In acute epicondylitis orgotein may be regarded as an alternative to the more common and potentially risky corticoid therapy.

[Blood screening for Down's syndrome in women under 35 years of age with an age-independent index].

The need for alternatives to amniocentesis, beset by 1% abortion rate, has been increasingly voiced over the past years and also includes the under 35 age group. However, the known age-dependent screening programs such as Alpha program according to Wald (1988) and the Dermalog Program according to Norgaard-Pederson (1990) only yield a detection rate of 50 or, at best, 70% (Müller, 1992) for this age group. We tried to improve this result by index calculation. Compared to the tested age-dependent screening programs the Ulm Index achieves a 25 to 40% higher detection rate (85%). For pregnant women beyond 35 years the detection rate of Down's syndrome can even be raised to 95-100% by a combination of the Alpha or Dermalog program with the Ulm Index, without entailling an increase in the rate of false-positive results.

[The apolipoprotein E gene and Alzheimer disease phenotype].

The relevance of the apolipoprotein E epsilon 4 allele as risk factor for Alzheimer's disease is independent of age. This was demonstrated in 126 patients and 72 healthy controls. The age in both groups varied between 44 and 95 years. An earlier onset of symptoms among carriers of the epsilon 4 allele, however, was observed only in the oldest patients. This may reflect a selection bias which is due to the association of the epsilon 4 allele with two competitive age-dependent risks: for Alzheimer's disease and for coronary heart disease. In a subsample of 64 patients who had participated in a longitudinal study no relationship was found between the apolipoprotein E genotype and clinical symptoms or symptom progression.
