Fine structure of active and healed duodenal ulcer.

In order to characterize the fine structure of active and healed duodenal ulcers, we examined tissue specimens of patients with active duodenal ulcer disease (n = 30) before and after treatment with either antacids (n = 16) or H2-receptor antagonists (n = 14), by light microscopy and various electron microscopic techniques, e.g., scanning and transmission electron microscopy. The characteristic histological feature of both the active and healed duodenal ulcer was the appearance of periodic acid-Schiff (PAS)-positive epithelial cells at the edge of the ulcers. Electron microscopy revealed that these cells were similar to a special type of mucus-secreting cell in the antrum (surface mucous cell). Their mucus granules contained mainly neutral glycoproteins. Helicobacter pylori were found attached to these cells in tissue specimens from 12 of 30 patients (40%). The mucous structure destroyed during the ulcerative phase regained its normal net-like structure after treatment. The ultrastructural healing process of duodenal ulcer was characterized by the presence of gastric metaplasia, by stunted microvilli of the duodenal epithelium (p less than 0.001 vs. control group), and an increased number of lysosome-like bodies (p less than 0.001 vs. control group) of the epithelial cells. These results were independent of the type of treatment, and showed that the repair mechanisms were incomplete after a 4-wk period of treatment.

[Percutaneous nucleotomy: an alternative to chemonucleolysis?].

In this study, we compare two groups of patients treated with chemonucleolysis (CNL) (101 patients) and percutaneous nucleotomy (PN) (100 patients) for sciatic pains. Indications for treatment were drawn along similar lines. The patients treated with CNL originated from the Department of Neurosurgery, University Hospital in Mainz and underwent treatment between 1983 and 1987. The second group with PN were from the teaching hospital in Wiesbaden and were treated during 1987 and 1988. Postoperatively, 80% of the CNL group showed good results. However, postoperative progress was slow with back spasms as a common uncomfortable side effect. In 72% of the patients in the PN group, successful treatment was demonstrated with results occurring almost immediately after treatment.

The striking histological feature of both, the active and healed duodenal ulcer, was the appearance of PAS-positive epithelial cells. Electron microscopy revealed similarity to a special type of mucus secreting cell of the antrum. The destroyed mucous structure during the ulcerative phase regains the normal net-like structure after treatment. The ultrastructural healing process shows constant features independent from the type of treatment and shows that the repair mechanisms are incomplete after a 4 week treatment.

[Histological and ultrastructural findings in the healing phase of duodenal ulcer].

To evaluate and further characterize duodenal ulcer healing macroscopical, histological, and ultrastructural criteria were examined in 24 patients with active duodenal ulcer disease under two different treatment schedules. After the endoscopic confirmation of an active duodenal ulcer patients were randomly assigned to treatment with either an antacid (group A) or with an H2 blocker (group B). Tissue specimen were taken during endoscopy from the margin and 1 cm from the ulcer before therapy and from the ulcer scar or the previous ulcerated region after a 4 week treatment. A macroscopic ulcer healing by endoscopic criteria was confirmed in 83% of the patients in group A and in 83% of group B. The prominent histological feature of duodenal ulcer healing was a significant increase of PAS positive epithelial cells. Ultrastructural changes of the duodenal mucosa were still present at the end of the treatment, even in the presence of complete macroscopic healing. The ultrastructural healing process shows constant patterns independent from the type of treatment. The characteristic morphological feature by electron microscopy of the healing duodenal ulcer is the appearance of a metaplastic mucus secreting cell of the antrum-type. The devastated mucous structure during the ulcerative phase regains the normal net-like structure after 4 weeks therapy.
