[Follow-up of patients with knee endoprostheses of the orthopedic clinic of the Dresden "Carl Gustav Carus" Medical Academy].

A report is given about an examination of 143 knee prostheses which were implanted between 1973 bis 1983. After biomechanic points of view the used prostheses were divided in four types. Very good to good results were found there in 70-80% of cases. Most positive results were reached by the so called sledge prostheses. In 44% of cases there were observed complications. The rate of complications is about 12% higher by patients whose basic disease is pcP.

[Shear fracture stress of the spongiosa and pressure fracture stress of cortical bone of cadaveric vertebrae].

By biomechanic examinations of the vertebrae of human dead bodies the ultimate stress due to shearing (Schubbruchspannung) of the spongy substance could be found out with 1.80 N/mm2 and the ultimate stress due to compression (Druckbruchspannung) of the cortical-belonging to the thick of cortical--with 280 N/mm2. Both measuring results are important for the ultimate strength of the connection (Verankerungsfestigkeit) of screws in vertebae.

[Correlation between roentgen morphometrically obtained data and cortical thickness indicative of anchorage strength of screws in human cadaver vertebral bodies].

Through the morphometric and histomorphometric examinations on the vertebral of the corpses, a functional connection between roentgenmorphometric plain facts (in transverse- and sagittal diameter of the vertebral body) and the cortical thickness can be established. The stress of the vertebral body through a screw can already preoperatively be assessed by roentgenmorphometric examinations that the cortical thickness plays an important dimension in the anchorage tightening of the screws on the vertebral body. By use of this knowledge the guarantee of operation on vertebral body is raised.

[Anchoring experiments on human cadaver vertebrae using various types of screws].

The firmness of the bone structure of vertebrae of human cadavers was investigated by turning screws of four different types into vertebrae T6 to L5 and subjecting them to axial tension up to the point of extraction. It was found that the cancellous bone screws (rather than the Zielke screw) had the best anchoring characteristics in bone tissue. The shearing fracture stress of the vertebral bone tissue was found to be approx. 1.80 N/mm2; the relation between the extraction forces and the individual vertebrae was also examined. In addition, the extraction forces were determined for screws inserted into vertebrae T6 to L4, as performed in Zielke's ventral spondylodesis procedure, as well as the lower tolerance limit for each vertebra in this segment of the vertebral column.
