[Fetal bigeminus following combined bronchodilator treatment of the mother].

After a combined bronchodilatory treatment of theophylline and terbutaline an intermittant fetal bigeminus is observed. The cardiotoxic changes disappear after days when treatment is stopped. After delivery the child is healthy. The slow disappearance of the arrhythmia is a result of the prolonged half-life of theophylline in the fetus. A combined therapy for asthma should be avoided. If this is not possible, regular cardiologic controls of the fetus should be carried out.

Arrhythmias, Cardiac

[Transitory increase of the thyroid stimulating hormone in the newborn after polyvinylpyrrolidon-iodine application during parturition (author's transl)].

A vaginal Polyvinylpyrrolidon-iodine application was carried out in 120 patients as a prophylaxis against infection. In nine infants there was an increase in the thyroid stimulating hormone, which disappeared after a few days. The possibility of a decrease in the conversion of 1-thyroxine to 1-triiodothyronine when there is an increase of r-triiodothyronine which cannot block the thyroid stimulating hormone sufficiently is discussed.

Bacterial Infections

[The diagnosis of fetal atrio-ventricular block (author's transl)].

The case is reported of a primigravida who was found to have a fetal bradycardia of 70 per minute. A caesarean section was done with the differential diagnosis intrauterine hypoxia or fetal arrhythmia. The infant was found to have a single ventricle, single atrium, a hypoplastic aortic arch and a coartation of the aorta. The infant died neonatally.--The case was reported to emphasize the necessity of fetal electrocardiography in continual antenatal monitoring to rule out an atrio-ventricular block prior to caesarean section.

Bradycardia

[Tubal fertility].

Animal experiments with 110 female guinea pigs proved an external ovum passage in 14 per cent of cases. These results ask for the conversative character of operations on the adnexes in sexually mature females. Literature reports on several cases of external ovum passage postoperatively. We presume, however, that the phenomenon of external ovum passage also results with an anatomically and physiologically normal sexual system more often than books in out discipline normally report. The basis of this study, which has the aim to prove in an experimental investigation with guinea pigs the phenomenon of external ovum passage, represented the delivery of a patient in the department of Obstetrics and Gynecology at the University of Göttingen, in whom precended operations of contralateral tube and ovary exstirpation.

[Transplacental cardioversion of fetal supraventricular tachycardia using sotalol].

Intrauterine fetal supraventricular tachycardia (FSVT) is a rare condition which is connected with organic heart disease in 4-10 per cent. Intrauterine pharmacologic cardioversion with digoxin and verapamil has been recommended previously. However, during pregnancy standard doses of digoxin result in suboptimal concentrations in the maternal serum and fetal therapeutic concentrations might demand doses inconvenient to the mother. Verapamil is not always effective. In this report we describe a case of FSVT. The arrhythmia was treated by administering sotalol to the mother after verapamil had failed to alter the fetal heart rate. This ist the first report of a fetal cardioversion with sotalol. We conclude that sotalol may be useful in the treatment of FSVT that is refractory to digoxin or verapamil.
