[Rare indication for a simple hip prosthesis with an extra-large femoral head following multiple failed exchange operations].

In certain rare cases of failure following multiple exchange arthroplasty at the hip, it is possible to create a strong and reliable connection between the pelvis and the femur, without replacing the acetabulum, by fitting a custom-made extra-large femoral head into the existing and stable large cavity. Two cases in which this method was used are described here and recommended for consideration in similar situations.

[Fosfomycin concentrations in serum and bile (author's transl)].

40 patients were given a single, short intravenous infusion of 4 g fosfomycin over a period of five to ten minutes. In 23 patients, the contents of the gall bladder were removed intra-operatively 30-105 minutes after the fosfomycin infusion. In 17 patients who had undergone cholecystectomy eight to ten days earlier, bile was obtained via a T-drain 30, 60, 120, 240 and 360 minutes after the fosfomycin infusion. Fosfomycin concentrations of 1-196 mg/l were present in the bile which had been removed intra-operatively. No correlation was found between the concentration and the time of removal. Patients with the highest alkaline phosphatase in serum, however, had the lowest fosfomycin concentrations. In the patients with T-drains, the highest concentrations (93 mg/l) were found 30 minutes after the fosfomycin infusion. By the sixth hour the concentrations had fallen to 19 mg/l.

[Morphological evaluation of joint-crossing expanded PTFE prosthesis (author's transl)].

Ten expanded polytetrafluoroethylene (PTFE) vascular grafts were implanted in dogs crossing the joint, performed as iliofemoral bypass. After 1, 3, 6, and 9 months the grafts were studied by light microscopy, scanning, and transmission electron microscopy. There was a good patency of all straight prostheses. The pre- and postkink areas showed thrombosis leading to stenosis. After 9 months endothelium was spread over all other parts of the luminal surface, including the kink area. Intramurally there was a focal ingrowth of fibroblastic cells with the characteristics of myocytes.

[Results of reconstruction of the carotid artery (author's transl)].

In 90 patients a total of 100 reconstructions of the carotid artery had been performed between 1972 and 1977. No patient died from the operation, 3 suffered central nervous complications, and 33 had transitory defects of cranial nerves. These defects were reversible in all but two patients. After an average observation period of 20 months 11 patients had died (6 from cardiac infarction, 2 from stroke, and 3 from unknown reasons). Of the 31 patients with clinical stage II, 24 were without symptoms, 5 had only rare transient ischaemic attacks, and two patients had the same frequency of attacks as preoperatively. Among the 39 stage III patients 35 showed complete or partial regression of symptoms whereas 4 had had repeat strokes. 55 of the 59 corrected stenoses were patent as shown by Doppler ultrasound. In contrast attempts to correct complete internal carotid artery occlusions showed unsatisfactory results.

[Investigations on the excretion of cefaclor in human bile (author's transl)].

The concentration of cefaclor in serum and bile was determined for up to 360 minutes after a single oral dose of 1 g cefaclor in 18 patients with continuous T-drainage of the bile duct. The rate of absorption varied, there being marked differences in the time. The peak bile concentration of 12.1 mcg/ml was reached after 120 min; the peak serum concentration of 12.3 mcg/ml, on the other hand, was reached after only 90 min. There was no accumulation of cefaclor in the bile.

[Iatrogeni vascular injuries (author's transl)].

We report on 34 intraogenic vascular injuries. In nine cases vascular trauma occured secondary to diagnostic procedures. The other 25 cases were due to therapeutic measures. Vascular reconstruction was successfull in all cases, except 2, which required above-knee amputation because of ischaemic damage. The causes of death in 4 cases were malignant diseases or multiple traumatic injuries.

[Pericardial tamponade, an acute emergency during transvenous cardiac pacemaker implantation (author's transl)].

Three cases of right ventricular perforation and acute pericardial tamponade during transvenous pacemaker implantation and one case of chronic electrode penetration are reported. Successful repair was achieved via pericardiotomy in all cases with acute tamponade. Fragility of the myocardium in the elderly is considered to be the major reason for this complication which may occur despite the use of flexible electrodes and correct surgical technique. The relatively small intervention of inferior pericardiotomy appears to be the therapy of choice in case of tamponade. This approach should be feasible when transvenous pacemakers are implanted. Immediate availability of general anesthesia and postoperative intensive care appears to be manatory.

[Revascularisation in peripheral vascular occlusions (author's transl)].

The 240 reconstructions of the femoral artery, performed within the last 4 1/2 years, involved 26 patients (27 bypasses) with combined femoral and peripheral vascular occlusions. The bypasses were connected with a segment of either the popliteal or the tibial artery. There was no operative mortality. The mean period of follow-up was 13 months: 18 of the bypasses were patent (2 patients died with open bypass), while 7 bypasses occluded (3 patients died). The authors feel, that isolated artery segment should always be explored. Only an autogenous vein graft should be implanted.

[The value of measuring intraoperative pressure in carotid endarterectomy (author's transl)].

During the last 4 years 92 operations at the carotid bifurcation were performed. In almost all cases carotid stump pressure was measured. If the stump pressure was below one third of the systemic pressure endarterectomy was done with intraluminal shunt. Under these conditions neurologic symptome deteriorated 3 times, but improved after a few days. There was no hospital mortality.

[Effectiveness of local and parenteral antibiotics in infection of aortic dacron grafts (author's transl)].

The effectiveness of local and parenteral antibiotic prophylaxis in infections of aortic dacron grafts was tested in animal experiments. In 20 foxhounds Dacron-interpositions, 4 cms in length, were inoculated with 50 million Staph aureus bacteria and then treated in 4 different ways: Nr. 1-5 no antibiotic treatment. Nr. 6-10 local antibiotic (Nebacetin, R) treatment. Nr 11-15 parenteral treatment with Gramaxin (R) i.m. Nr. 16-20 combined Nebacetin (R) and Gramaxin (R) treatment. The untreated dogs developed infection of laparotomy wounds, peritoneum udn prosthesis. Nebacetin gave improved results, whereas Gramaxin led to complete eradication of the infection. It is concluded, that antibiotic prophylaxis should be applied in vascular surgery, whenever alloplastic, homologous or autologous implants are used.

[Doppler measurement of arterial pressure for assessing therapeutic measures after arterial occlusion (author's transl)].

Arterial pressure differences between arm and foot arteries can be easily measured by the Doppler method and in this way the gradient across an arterial occlusion of the lower limbs approximately quantified. An increase in gradient after induced ischemia is inversely proportional to its compensation. Pre- and postoperative measurements on 51 limbs demonstrated that functional results of arterial reconstruction can be thus assessed and their further course objectified.
