Correlates of cognitive impairment and depressive mood disorder in multiple sclerosis.

The psychopathological status of 25 inpatients suffering from clinically definite multiple sclerosis (MS) according to Poser criteria was assessed by using standardized methods (Structured Clinical Interview for DSM-III-R, Inpatient Multidimensional Psychiatric Scale, Hamilton and Montgomery-Asberg Depression Rating Scales and the Structured Interview for the Diagnosis of Alzheimer Dementia and Dementias of other Aetiology (SIDAM). Magnetic resonance (MRT) (0.5 T; T2-weighted sequence) of the brain was analysed by measuring the ventricular brain ration (VBR), the area of the corpus callosum (CC) and the extension of hyperintense lesions of the brainstem, the temporal lobes and the brain at all. Six of 25 (24%) of these moderately disabled patients (mean Extended Disability Score (EDSS) 3.3) were diagnosed to suffer from depressive mood disorder (major depression or dysthymia); 2 were demented. In correlation analysis, depression was unrelated to age, gender, duration of illness, status of disability (EDSS) or the results of cognitive assessment. No relationship between the depression scores and the different MRT measures could be identified. The presence or absence of gadolinium enhancement was also uncorrelated to depressive symptoms. Fatigue as measured by the Fatigue Severity Scale was unrelated to depression or subcortical brain atrophy (increased VBR) but significantly correlated to the area of hyperintense MRT changes in brainstem and midbrain. Cognitive impairment (decreased SIDAM scores) was correlated to the total area of hyperintense MRT changes of the brain parenchyma. The type of clinical course (relapsing-remitting vs chronic progredient) was not found to influence the affective or cognitive state in our MS patient's sample.

[Computer tomography in diagnosis of ovarian cancer].

In 90% of all cases an ovarian tumor about 2 to 3 cm in size can be visualized computertomographically using a contrast medium. Determination of the dignity can be achieved in about 81% of all cases. Malignant tumors which are impressive as smooth-walled cysts are only recognized as such in 50% of all cases. Correct staging also occurs in 50% of all cases. An accuracy of about 90% can be expected in aftercare.

[Radiologic diagnosis of metastases in the lesser pelvis].

Reviewing the literature and summarizing our own experience, the role of various imaging modalities concerning diagnosis of metastases to lymph nodes and organs in the pelvic cavity is discussed. Especially ultrasound, CT and lymphography are compared, the importance of fine-needle-aspiration is emphasized. Possible advantages of MRI are mentioned.

[Ovarian tumors in the computer tomographic image--the correlation of histology and CT diagnosis].

The value of CT in respect of preoperative determination of tumour status and of tumour classification was studied using 127 histologically identified ovarial tumours as basis in a blind study via mutual image classification by the two participating institutes. The medium accuracy of diagnosis was 82%, but the results were found to be greatly dependent on tumour histology. Quotas of more than 90% were obtained in case of functional cysts, cystomas , cystadeno -carcinomas and dermoids, whereas classification was less successful with the other types of tumours such as germ cell tumours, tumours of the sexually differentiated stroma, metastases and tubo- ovarial abscesses.

[Contribution to the study of angio-TC: timing of exposure and subsequent venous lavage].

The text reports the results of angio-CT examinations performed with single intravenous injections of small amounts of contrast medium and with the technique of timing of exposure and immediate venous flush with the use of a suitable injector (168 patients have been examined, in 157 of them the timing was correct while in 11 the timing was not adequate). The exposure is made after a certain period of time from the moment of the injection, corresponding to the number of heart cycles the contrast medium needs to reach the considered arterial district, during its first passage. The immediate venous flush allows one to use almost all the contrast medium to obtain the information and to take advantage of an almost compact bolus. The principal advantages of this technique are that a smaller amount of contrast medium and of radiation dose to the patient can be used.

[Preoperative classification of ovarian tumors by computed tomography].

127 histologically demonstrated ovarial tumors were studied in a blindfold test in order to find out to what extent a preoperative determination of dignity or diagnosis of the tumor kind is possible by computed tomography. The overall rate of correct determinations of dignity is 82%. In case of functional cysts and cystomas with thin cyst walls, cystadenocarcinomas and dermoid cysts, this rate is about 95%, whereas the classification results are less exact in case of cystic tumors with broadened cyst walls, preponderantly solid tumors and tumor-like lesions.
