Generation of anaphylatoxin C3a in plasma and bronchoalveolar lavage fluid in trauma patients at risk for the adult respiratory distress syndrome.

OBJECTIVE: To determine the generation of anaphylatoxin C3a in plasma and bronchoalveolar lavage fluid in trauma patients at risk for the adult respiratory distress syndrome (ARDS). DESIGN: Prospective study. SETTING: ICU in a university hospital. PATIENTS: Severely traumatized patients at risk for the ARDS (n = 25). INTERVENTION: EDTA plasma samples and bronchoalveolar lavage fluid were obtained. MEASUREMENTS AND MAIN RESULTS: Complement proteins C3, C4, C5, and the inhibitors C1-inhibitor, Factor H, and Factor I were quantitated in EDTA-plasma samples obtained every 6 hrs during the first 48 hrs after ICU admission and every morning from days 4 to 14 after injury. In bronchoalveolar lavage fluid, the complement activation production of C3a-desArg was quantitated and the volume of epithelial lining fluid was calculated. All patients showed a decrease of the complement proteins C3, C4, C5 and of the inhibitors C1-inhibitor, Factor H, and Factor I during the first 24 hrs, indicating complement consumption. Patients developing ARDS (n = 11) showed significantly higher C3 concentrations and a higher C3a/C3 ratio in the first few hours after multitrauma. Follow-up bronchoalveolar lavages demonstrated highly increased amounts of C3a in epithelial lining fluid during the first 24 hrs, mainly in ARDS patients and, to a lesser degree, in non-ARDS patients. To determine the origin of C3a in bronchoalveolar lavages, the ratio of C3a in epithelial lining fluid and plasma was calculated. CONCLUSION: The C3a of epithelial lining fluid to plasma ratio was extremely high in patients developing ARDS, but even the non-ARDS group had a ratio greater than 1, indicating that a substantial local complement activation occurs in the lung.

[Somatic radiation load by radiography of the temporomandibular joint].

660 exposures were performed on an Alderson phantom to determine the somatic radiation exposure values for standard examinations of the temporomandibular joint. Comparative examinations with the classic contact technique according to Parma served to make clear the results obtained. The lowest amount of surface exposure was observed in contact examinations of the temporomandibular joint. The use of a flat tube leads to a 13-fold reduction in surface exposure compared to contact examinations according to Parma. Furthermore, a heavy gradient of radiation exposure from the area of exposure to the environment was stated in using a tube (flat tube and ear tube).

Isolation of late complement components by affinity chromatography. II. Purification of the human complement component C6.

We developed a new procedure for the rapid and gentle isolation of the human complement component C6 comparable to that described previously for C9. The procedure is based on affinity chromatography. As a first step, C6 is immunoabsorbed on insolubilized anti-C6 antibodies. These antibodies were derived from C6-defective rabbits (Freiburg strain). C6 was eluted with 3 M thiocyanate, pH 7.2, with a recovery of 15--23% of its hemolytic activity and a more than 270--fold purification. Impurities were removed in a second step by an "anti-impurity" column. The final product yielded a 12% recovery of the hemolytic activity and the purification factor was higher than 1300. The final product was homogeneous in SDS polyacrylamide and immunoelectrophoresis.

Activation of complement by radiographic contrast media: generation of chemotactic and anaphylatoxin activities.

Iodinated radiographic contrast media such as methylglucamine diatrizoate and sodium ioglycamate activate serum complement in vitro. This was shown by a dose-, time-, and temperature-dependent decrease of total hemolytic complement activity in normal human serum, consumption of C4 and C6 activity, conversion of C3 to C3b, and generation of C5-derived chemotactic and smooth muscle contracting activity. Complement activation was achieved even in sera depleted of immunoglobulin and properdin, which may indicate that contrast media induce complement activation by mechanisms different from the classical or alternative pathway.

[Clinical and radiological control examinations after cyst surgery in the permanent juvenile dentition].

The present study involves 23 patients with 25 cysts whose conditions were assessed postoperatively by means of clinical and radiological criteria. The radicular and follicular cysts were most frequently located in the anterior region of the upper jaw. With 17.4% of the total number of bone cysts, the percentage of cysts on juvenile permanent teeth is relatively low. Teeth that did not respond to irritants were the most frequent complication in the operating area. Nevertheless, the clinical and radiological findings tell against the non-vitality of these teeth.

Deviated lysis: lysis of unsensitized cells by complement. V. Generation of the activity of low pH or low ionic strength.

In serum exposed to acid pH (6.4), a serum activity was generated which lyzed unsensitized erythrocytes in the presence of EDTA. It was similar to the d.l. activity found following serum activation by inulin (2). In contrast to the d.l. generation by the classical or by the alternative pathway of C activation, the generation of d.l. by acid pH did not require C4 plus C2 or C3 plus factor B resp. It was, thus, not dependent on any hitherto known pathway of C activation. A similar activity appeared when NHS was centrifuged in a sucrose gradient at low ionic strength. Physicochemical alterations of the component proteins which influence their affinity for each other are seen as the basis for the activation of the attack phase of C.

[Hereditary angioneurotic edema. Clinical aspects, pathogenesis and therapy].

A family with hereditary angioedema is presented. In 4 members of the family the diagnosis was confirmed by decreased C1 esterase inhibitor levels. Clinical course and pathogenesis are described briefly. New ways of treatment are discussed, especially injection of purified C1 esterase inhibitor and stimulation of C1 esterase inhibitor synthesis by oral therapy with attenuated androgens.

[Proposal for uniform equipment for dental radiodiagnostic offices].

Corresponding to the different objectives, the Working Group on Radiology of the Stomatological Society has designed 6 standardized work places for routine radiodiagnosis in stomatology. Apart from the objectives, the problems related to the division of rooms, equipment, radioprotection and film processing are dealt with separately and specifically. The work places for routine stomatological radiodiagnosis are intended for achieving an improvement in a sector of stomatological care.

Deviated lysis (d.l.): III. Kinetics of interaction of d.l. activity with chicken erythrocytes: evidence for E formation.

The interaction of d.l. activity with chicken red cells (CE) generates a cell intermediate with the properties of classical E*. Generation of CE* by d.l. activity at 37 degrees C is rapid, while there is a considerable lag in the conversion of CE* to ghost and hemoglobin. Conversion of CE* to ghosts can be blocked by high concentration of EDTA and/or 0 degrees C. CE* contain at least C6 and C9 on their surface.

Deviated lysis: transfer of complement lytic activity to unsensitized cells. IV. Parital isolation of the activity.

Deviated lysis (d.l.) was previously characterized as the lysis of non-sensitized erythrocytes by activated complement (C) in the presence of EDTA (1, 2, 3). The lytic activity was present in serum fractions of a m.w. in the proximity of 220,000. All the C factors C5 through C9 were found in these fractions and they were all needed for lysis. It is proposed that in d.l. small aggregates of the C components C5 through C9 coexist in the reaction mixture without further interaction. Only when appropriate receptors such as present on target cells surfaces are available, the factors react in a sequential order eventually to result in lysis of the target cell.

On the transport of various endogenous plasma proteins from blood to peripheral lymph in man.

A method to prepare peripheral lymph out of the foot of clinically healthy patients with respect to their blood capillaries and lymph vessels is presented. Following electrophoresis in molecular sieving polyacrylamide gel the lymph/plasma quotients of precentages of identical protein fractions in both body fluids as well as of their concentrations were graphically displayed dependent on their migration velocity within the gel. As the high or low lymph-plasma-relations are necessarily to be interpreted in the sense of a relatively high or low blood-lymph permeation the findings are speaking against an exclusive effect of the molecular sieving principle at the blood-tissue-barrier. An explanation for special lymph-plasma-relations would be provided by the assumption of a gel-filtration effect in the extravascular circuit of plasma-proteins. The results affirm the findings of former experiments on animals in an analogue model. The method as well as simplified variants might be useful for the investigation of peripheral nutritive disorders as well as of clinicopharmacological questionings.

[Experiences with linear and multi-dimensional tomography and zonography in the radiodiagnosis of the temporomandibular joint].

A Multigraph 2 served to compare linear and elliptic tomography with linear and circular zonography as to their diagnostic values in the examination of the temporomandibular joint. The contrasts and the blurring effects of the different forms of layers are studied with regard to the appreciation of the structures and contours of the joint-forming components. The linear zonography is considered the method of choice, mainly because it gives sharp contrasts.
