Tuberculous abscess of the liver in acquired immunodeficiency syndrome.

We present two human immunodeficiency virus (HIV) positive patients; each was diagnosed with tuberculous abscess of the liver and treated with good clinical response. A review of the literature indicates there is no typical clinical presentation. A high index of suspicion plus special stains of tissue material are required to make the diagnosis in appropriate patients, especially because this is a treatable condition even in patients with AIDS.

Leiomyosarcoma in jejunal diverticulum.

Malignancy arising in a jejunal diverticulum is extremely rare. We report a leiomyosarcoma arising from a jejunal diverticulum. This is the second reported case of smooth muscle tumor arising from this site. Despite general belief that small bowel diverticula are devoid of muscle coat, there is presence of a thin muscle coat which can give rise to a neoplasm. We report such a case.

Angiographic visualization for percutaneous renal biopsy.

Fifteen patients underwent visualization of the kidney for percutaneous biopsy by selective renal angiography. Biopsies were performed in 11 patients, and adequate renal tissue was obtained in all instances. Postbiopsy angiogram revealed pinpoint bleeding in ten patients and significant extravasation in one case, which was controlled with a selective epinephrine infusion. In four cases, biopsy was not performed when the cortex was found to be less than 5 mm thick.

Popliteal artery branches: percutaneous transluminal angioplasty.

Percutaneous transluminal angioplasty of the branches of the popliteal artery was performed in six patients, two of whom also had femoropopliteal angioplasty. Dilatation was performed with catheters ranging from 5.5 to 7 French. None of the popliteal branch dilatations was performed with an inflated balloon. Excellent dilatation, as documented by angiography and Doppler ultrasound pressure recordings, was obtained in four cases and moderate and slight improvement in one case each. Two of the four patients with excellent results had prompt healing of toe amputations and are doing well at 19 and 22 months after angioplasty. In the other two patients with excellent results, below-knee amputation was necessary despite patency of the dilated segment in one. In the other patient with initially excellent results, reocclusion occurred 4 months after angioplasty. In the patients who had moderate and slight improvement on angiography and in distal pressures, no significant clinical course change occurred. No patient had clinical deterioration after angioplasty and no significant complications occurred.

Painless dissections of the aorta presenting as acute neurologic syndromes.

We describe three patients who had painless dissections of the aorta which resulted in neurologic syndromes at the time of presentation. Two patients had acute hemimotor and sensory findings. In one of these cases progression to paraplegia occurred. In a third patient, acute weakness and ischemia of a leg occurred at presentation. We review previously described painless aortic dissections. Such aortic dissections may be suspected in the setting of an acute neurologic event by abnormalities in the examination of the peripheral pulses and the heart and by attention to characteristic chest x-ray changes.

Varied sonographic patterns in Echinococcus liver disease.

Echinococcus granulosus has been an uncommon disease in the United States. Cases may appear unexpectedly because of increased immigration from endemic areas. Over the past 3 years, 10 cases of hydatid liver disease were diagnosed by sonography in our institution. Sonography is a simple noninvasive method of early diagnosis even when clinical signs are nonspecific. The sonographic patterns of this condition are described.
