[Complication-free pregnancy following bone marrow transplantation for severe aplastic anemia].

A bone marrow transplant had to be performed in a woman of 25 years of age because of severe aplastic anaemia. Immunosuppressive conditioning treatment consisted of 50 mg/kg cyclophosphamide given for four consecutive days. Regular menses reappeared already after four months. Pregnancy was confirmed after a year that resulted in giving birth to a healthy child in the 38th week of pregnancy via Caesarean section.

[Eye changes following bone marrow transplantation].

Blood diseases such as acute leukemia can now be cured by bone marrow transplants. Complications such as a chronic host-versus-graft reaction lead to typical conjunctival changes, as described for Sjögren's syndrome and ocular pemphigoid. A patient with typical eye complications is described and the probable immunological course discussed.

Treatment of hairy-cell leukemia with recombinant leukocyte A interferon.

A 29-year-old male patient with hairy-cell leukemia was referred for bone marrow transplantation. For tumor mass reduction a treatment with recombinant leucocyte A interferon was started. A complete remission could be achieved in the third month of treatment and no adverse side effects were observed. The patient refused bone marrow transplantation and a relapse was diagnosed 51 days after discontinuation of interferon therapy. Treatment of hairy-cell leukemia with recombinant leukocyte A interferon seems to be a promising approach.

[Cytomegalovirus hyperimmune globulin and cytomegalovirus-tested blood products--a pilot study in bone marrow graft recipients].

Nineteen bone marrow transplant recipients had cytomegalovirus prophylaxis using a commercial hyperimmune globulin. Seronegative donors were selected for blood component substitution. The incidence of seroconversion was 25%. One patient acquired fatal cytomegalovirus associated interstitial pneumonitis. The value of this combined cytomegalovirus prophylaxis in bone marrow transplant recipients in comparison to sole passive immunization is recommended to be the subject of a controlled trial.

[Bone marrow transplantation in panmyelopathies and leukemias with special regard to gnotobiotic measures].

Bone-marrow transplantations were performed in 71 patients, 11 with panmyelopathy, 17 with recurrence of acute leukaemia, 25 with acute leukaemia and remission, 18 with chronic myeloid leukaemia. The transplantation was allogenic in 67, autologous in 2, isologous in 2. Eight patients each survived in the panmyelopathy and chronic myeloid leukaemia groups. In the group of patients with acute leukaemia only one patient of those in a recurrence survived the transplantation for several years, but after 6 years there was another recurrence. Of the 20 patients with acute myeloid leukaemia who received the transplantation during their first remission, 11 are still alive. Retransplantation because of the recurrence was employed in one case each of acute and chronic myeloid leukaemia. Main cause of death was interstitial pneumonia with an overall risk of 23%. Only 5% of patients developed severe acute graft-versus-host reaction, grades III-IV. The low incidence of this reaction is possibly due to the strict gnotobiotic measures which in most of the patients led to decontamination of the intestinal tract.

Clonal evolution with isodicentric Ph1 chromosome in Ph1-positive CML: karyotypic conversion after bone marrow transplantation.

Clonal chromosomal evolution was observed in a 16-year-old boy suffering from Ph1-positive CML. An isodicentric Ph1 chromosome appeared 20 weeks after the initial diagnosis. At that time an allogeneic bone marrow transplantation was performed. Thereafter, during an observation period of more than 13 months, chromosome analyses showed neither the Ph1 chromosome nor the abnormal isodicentric variant. Close cytogenetic monitoring is suggested to reveal early unfavorable prognostic signs of the onset of blast crisis before it becomes evident in the bone marrow morphology.

Laminar air flow versus barrier nursing in marrow transplant recipients.

Forty-eight patients with acute leukaemia in relapse (n = 14), acute leukaemia in complete remission (n = 19), chronic myeloid leukaemia (n = 8) or severe aplastic anaemia (n = 7) received a marrow transplant. The first 26 patients were nursed in laminar-air-flow plastic isolators while the next 22 patients were treated in barrier nursing rooms. Gnotobiotic parameters and morbidity in the 2 groups are compared. Good decontamination of the gastro-intestinal tract was obtained using either of the 2 isolation techniques. The incidence of bacterial and mycotic infections, as well as the supportive care required by the patients was almost equal in both groups. Our results also suggest that the incidence of graft versus host disease may decrease with efficient decontamination of the patients.

Anemia, Aplastic↗

[Substitution of blood components].

Cell substitution in the form of "tailored haemotherapy " should be an essential part of medical oncology at the present time. In the conventional therapy of solid tumors patients do not enter into prolonged phases of severe haemopoietic insufficiency. Accordingly platelet and granulocyte transfusions will be exceptional. It is mainly the red cell transfusion which plays the most important role. The possible complication of cell substitution should always be kept in mind. Here the risk of alloimmunisation which makes a continuation of transfusion therapy or a subsequent bone marrow transplantation problematic, is just one of the examples. Modern cell separation techniques allow the production of highly concentrated cell preparations aiming at the reduction of the frequency of transfusions and minimizing the risk of sensitisation. If rich concentrated preparations have to be given to immunodeficient patients an irradiation should proceed their transfusion, otherwise the immunocompetent lymphocytes contained in the preparations could induce graft versus host reactions. Allogenic bone marrow transplantation is more becoming the treatment of choice for severe combined immunodeficiency, severe aplastic anemia, acute myeloid leukemia, and chronic myeloid leukemia. At the present time the place of allogeneic or autologous bone marrow transplantation as a treatment of lymphomas and solid tumors is still unsettled.

[Results of treatment of acute myeloid leukemia in adults].

Sixty-five previously untreated patients (34 women, 31 men) with acute myelocytic leukemia, ranging in age from 15 to 71 (median 43) years, were treated for remission induction with 7-day courses of cytarabine (100 mg/m2/day continuous i.v. infusion) together with daunorubicin (45 mg/m2/day rapid i.v. injection) on days 1, 2 and 3. Supportive care consisted of broad spectrum antibiotics for fever in the presence of granulocytopenia and substitution of erythrocytes and platelets. The complete remission (CR) rate was 55.3%. The mean numbers of chemotherapy courses and days to achieve CR were 1.5 and 41, respectively. Sex and age appeared to have no effect on the remission rate. For remission maintenance, cyclic courses of cytarabine (i.v. 100 mg/m2 q 12 hr X 10) were given with each of four drugs, 6-thioguanine (p.o. 100 mg/m2 q 12 hr X 10), cyclophosphamide (i.v. 800 mg/m2 on day 1), CCNU (p.o. 100 mg on day 1) or daunorubicin (i.v. 45 mg/m2 on days 1 and 2) in rotational sequence. The median remission duration was 44 weeks, the median survival time for all patients 31 weeks, for those responding to therapy 74, and those not responding 8 weeks.

[Bone marrow transplantation for aplastic anaemia and acute leukaemia (author's transl)].

Twenty-six bone marrow transplantations were performed at the West Germany Tumour Centre at Essen between December 1975 and December 1981. Three patients had aplastic anaemia, 23 acute leukaemia. Of the leukaemia patients 13 had a recurrence and ten were in full remission at the time of the transplantation. Two of the patients with a recurrence received deep-frozen autologous marrow from the remission phase. One patient with aplastic anaemia had an identical twin as donour, while all other patients received allogeneic marrow from histocompatible family members. Eleven patients are still alive, all three of those with aplastic anaemia, one of those with leukaemia in a recurrence, and seven of those with acute leukaemia in full remission. Two of the survivors have a chronic graft-host reaction, but the others are symptom-free, after an observation period of up to four years. Main causes of death were leukemic recurrence (6), infection (5), and graft-host reaction (2). It would seem that at present bone marrow transplantation offers the best chance of a cure for severe aplastic anaemic and acute leukaemia in adults, but only if transplantation is undertaken early and not in the end-stage of the disease.

[Effect of long-term cytostatic therapy on the hematopoietic stem cells].

CFU-C and diffusion chamber studies were performed in patients with teratocarcinoma, who underwent long term chemotherapy. No significant decline of bone marrow CFU-C or diffusion chamber cell recovery was found during twelve months of cytotoxic treatment. In contrast to the results in these patients the CFU-C-content of the remission marrow in leukemic patients showed a significant decrease in relation to the duration of remission and chemotherapy.

Human peripheral blood "null-lymphocytes", a highly enriched pool for granulocytic stem cells (CFU-C).

A "Null"-lymphocyte preparation containing a high concentration of granulocytic stem cells (CFU-C) was isolated from human peripheral blood. 3 lymphocyte purification procedures were shown to produce an enrichment of CFU-C: 1. Removal of mononuclear phagocytes by iron phagocytosis and plastic adherence procedures. 2. Passage through Ig anti-Ig columns. 3. Removal of E-rosette-forming T cells. A combination of all 3 purification steps resulted in a 30- to 40-fold enrichment of CFU-C in the final "Null"-lymphocyte population as compared to the initial lymphocyte fraction isolated by Ficoll-Urografin density gradient centrifugation.
