Glutamic acid decarboxylase in sea lamprey (Petromyzon marinus): characterization, localization, and developmental changes.

We have carried out assays for glutamic acid decarboxylase (GAD) in homogenates of brain and spinal cord from larval and adult sea lamprey (Petromyzon marinus). The enzyme had similar characteristics in both stages. Optimal pH was 6.8; optimal temperature was 27-30 degrees C; Km at 27 degrees C was 5 mM. GAD activity was distributed uniformly along the length of the spinal cord. Specific activities for the larval cord and brain were 26 and 63 nm CO2/mg protein/h, respectively. The specific activities for the adult cord and brain were 29 and 236 nm CO2/mg protein/h, respectively. Thus, the activity of cord homogenates did not change significantly between larval and adult stages, but that of the brain increased about fourfold.

Partial nephrectomy in surgical treatment of calculous disease.

Of a total of 158 partial nephrectomies, a detailed follow-up for up to sixteen years was obtained in 131. The rate of true ipsilateral recurrences was 26 per cent. Computer analysis of various parameters indicated that a previous history of a stone in the same or the contralateral kidney, or the presence of multiple or scattered stones in the affected side, were associated with a particularly high recurrence rate. Urinary infection at the time of or prior to operation was not of prognostic significance. The importance of complete excision of the calyceal neck was confirmed. It was concluded that the operation is logical for removal of a diseased segment of kidney, but generally cannot be recommended in the hope of removing a stone-bearing area of kidney and hence reducing the recurrence rate.

Cervical instability in an achondroplastic infant.

Cervical spine instability is a rare finding in achondroplasia. We present the previously unreported case of C2-C3 subluxation producing progressive quadriparesis in an achondroplastic infant. Operative treatment with appropriate fusion was performed, and an excellent result was obtained.

Degenerative lumbar spinal stenosis.

Advanced degenerative multilevel disk disease of the lumbar spine with resultant spinal stenosis may lead to neurogenic intermittent claudication, which closely mimics claudication due to aortofemoral disease. Narrowing of the lumbar spinal canal, the lateral recesses and intervertebral foramina secondary to degenerative disk disease is brought about by hypertrophy of laminae, facet joints and ligamentum flavum, descent of the pedicles, posterior bony ridging and occasional disk herniation. The symptoms are elicited by lumbar lordosis and are relieved by lumbar flexion. Five patients with degenerative lumbar stenosis are presented. Decompressive laminectomy yields satisfying results, regardless of the advanced age of some of the patients, and allows normal ambulation.
