[Traumatic carotid sinus reflex death].

The review of 7 autopsy cases shows, that the diagnosis of a sudden death due to mechanical irritation of the carotid sinus is possible, when an injury of this reflectogen zone is represented autoptically, when there are signs of acute cardio vascular failure and other causes of death are out of the question. Taking account of the ascerteinments at the scene as well as observations by witnesses of the death process is important for diagnostic reason. The proof of the carotid lesion depends on the preparation of neck organs in artificial lack of blood. The histological analysis of stepwise taken slides of the carotid arteries above the bifurcation is informative. The insurances consider a death due to carotid sinus reflex as an accident; in criminal law this kind of death is regularly linked to strangulation; diagnostic decision criteria are presented.

[Simulation of strangulation findings by postmortem rescue and transport measures].

Improper rescue or transportation of a corpse by gripping the neck or using a sling can cause traumatization that might erroneously suggest strangulation in subsequent autopsy. On the basis of 3 observations, postmortal damages which, at first sight, had seemed suspicious are presented and discussed. In the first case a drowned person's neck showed a dried skin line simulating a strangulation by ligature. The 2nd case dealt with a male person who had died ventricumbent in consequence of an apoplectic cerebral haemorrhage, and who had been lifted by embracing the neck shortly after death; at the post-mortem examination, massive blood extravasations were found in the cervical musculature. In the 3rd case, a strap was slung around the neck of a male epileptic--subsequently found to have served for carrying a saxophone. Both great hornes of the thyroid cartilage were fractured. Inquiry revealed that the undertakers had used the sling to hoist the corpse in which changes due to decomposition had already developed.

[Chromotrop-aniline blue stain for improved demonstration of fresh heart muscle fiber damage].

The Chromotrop-Anilinblue (CAB) trichrome staining method, developed for liver specimens, enables rapid detection of fresh damage to myocardial fibers by staining contraction-bands bright red. Dehiscence of the transverse striation, although yet unexplained in its pathogenetic significance, becomes visible with CAB staining. Overall, the method seems less sensitive to artefacts than former so-called infarction-staining-methods. Contraction-bands are not conclusive evidence for ischemic damage, as they also appear in traumatic heart damage (e.g. knife- and gunshot-wounds).

[Non-Hodgkin lymphoma with infiltration of the optic nerve (author's transl)].

Infiltration of the optic nerve is very rarely found in any form of malignant lymphoma. We report on a 24-year-old male patient suffering from non-Hodgkin lymphoma of the stomach. While recovering after treatment of the primary tumor by chemotherapy he developed papilledema in both eyes and almost total loss of vision. Radiotherapy resulted in restitution of vision within a few days. Later there was a transient acute detachment of pigment epithelium. Malignant cells in the CSF could not be detected until three months after the onset of ophthalmological symptoms. Histopathological sections revealed meningeosis sarcomatosa with infiltrations into the optic chiasm, tracts and nerves.

[Development of rust stains on the skin due to contact with a gun].

In order to study the conditions for the formation of so-called rustmarks on the skin after contact with weapon steel tests to provoke rustmarks were performed on corpses and living bodies. These tests were only successful under the condition of a firm contact with the weapon steel and a certain minimum contact time between the weapon and the skin. The experiments showed that the critical parameters for the appearance of rustmarks are, first, the humidity of the skin and the environment, respectively, as well as the contact time, and second, the state of the weapon surface (greased or ungreased). Both the ambient temperature and the pH value of the skin (alkaline or acid) are irrelevant to the formation of rust. The longest time period until rustmarks appeared was observed on dry skin in contact with a greased weapon, namely 22 hours on a corpse and 170 min on a living subject. When using a greased gun humidity accelerated the formation of rust. An ungreased surface of the weapon also resulted in faster rust formation. The minimum time necessary for the formation of rust determined under the most favourable circumstances was 135 min for a corpse and 27 min for a living person. During the early postmortal interval the rustmark may therefore be another piece in the jigsaw towards determining the time of death on suicides committed with firearms.

[Suicide by severing blood vessels using local anesthesia: reduced risk in extraction of HIV infected tissue using a stomacher].

Authors report an unusual suicide undertaken by a medical person who after applying the local anesthetic lidocaine, opened a leg vein with a scalpel. The cause of the suicide was presumed advanced AIDS symptoms with a progressive Kaposi sarcoma and Non-Hodgkin lymphoma. For the toxicological investigation of the tissue we used the stomacher extraction system, a method not widely known in Forensic science, however, recommended for the extraction of all infectious tissues.

[Recoil-induced rifle butt mark on the ceiling in suicidal gunshot injury].

In a suicide case the butt of the weapon (double-barrelled shotgun) produced a characteristically shaped mark at the ceiling, which could be physically explained by the recoil effect and was a clue that the victim had fired the shot himself. The ammunition used was a cartridge with a rifled shotgun slug (Brenneke type). The contact shot into the temple resulted in the rupture of the skull with exenteration of the brain. As the weapon remained lying on the body of the suicide after the shot, brownish traces of rust could form at the places of contact with steel parts.

[Suicidal carbon monoxide poisoning in an electric car. An unusual case report].

The authors report a case of a man who committed suicide by poisoning with carbon monoxide in his electric vehicle. He applied a small motor generator with no exhaust normally used for charging the vehicle's batteries at home, that was found on the loading space behind the seat. This demonstrates the value of a thorough scene investigation.

[Lodged bullet or bullet penetration? Re-entry of a projectile and pseudo-shot].

In an extended suicide an 11-year-old boy was killed by two shots into the back. In spite of corresponding entry and exit points one of the projectiles was found inside the body. By laying open the respective bullet track, examination of the deformations of the bullet, on which a total of three ricochet points were identified, and a second visit to the scene both the order in which the shots were fired and the reason why one of the projectiles was lodged inside the body, although there was an exit point, could be clarified: On firing the second shot the victim had been lying on the wooden threshold of a door, from which the projectile ricochetted. The terms "reentry" and "pseudo exit point" are discussed. The retention of the bullet in the depth of an alleged exit wound emphasizes the importance of post-mortal X-ray examinations.

[The "golden sniff": suicide by sniffing heroin].

The best known but apparently rare method to commit suicide by the intake of drugs is the 'golden shot'. As a result of a criminalistic scrutiny in combination with an exploration of the medical history, the findings of the autopsy and the toxicological analysis it is often not possible to discriminate between an accidental and a suicidal intoxication. The manner of death is therefore in most cases undeterminable. The presented unusual suicide by a letal opiate intoxication do to sniffing the drug would not have been clarified, neither with a complete examination of the history, an entirely performed autopsy, the determination of the way of ingestion of the drug nor with a complete toxicological analysis. The diagnosis is based primarily on the availability of a farewell letter. This case demonstrates that it is very difficult to determine the manner of death. Examining drug addicts it is exceedingly important to perform an overall examination by visiting the scene, by inspecting there the corps and the environment and by performing a complete autopsy with toxilogical examinations.
