Two entities of precursor T-cell lymphoblastic leukemia/lymphoma based on radiologic and immunophenotypic findings.

Precursor T-cell lymphoblastic leukemia/lymphoma (T-ALL/LBL) presents a mediastinal mass in one half of cases. Although the immunophenotypic features of T-ALL/LBL have been analyzed in several studies, few studies have been focused on the relationship between the anatomic distribution of lesions and immunophenotypic findings. We analyzed the clinicopathologic findings for 17 patients with T-ALL/LBL diagnosed since 1993 and whose radiologic findings were available. Data on 14 men and 3 women with a median age of 26 years (range, 10-61 years) were analyzed. On the basis of radiologic findings, the cases were divided into thymic type (n = 8) and nonthymic type (n = 9). Patients with the thymic type of T-ALL/LBL had a large mediastinal mass and minimal systemic lymphadenopathy only in the supradiaphragmatic region. Those with the non-thymic type had predominantly systemic lymphadenopathy that included infradiaphragmatic lesions. Expression of CD8 (6/7 versus 0/9) was more frequently found in the thymic type (P < .001), whereas expression of CD56 (0/7 versus 5/9) was more frequent in the nonthymic type (P = .034). In conclusion, T-ALL/LBL was divided into 2 entities, thymic type and nonthymic type, on the basis of radiologic findings and immunophenotypic features. Analysis of the expression of CD8 and CD56 would be useful for biologically classifying T-ALL/LBL into the 2 types. This study was performed in a single institution, was retrospective, and had a limited number of patients; multicenter confirmatory studies are warranted.

Prognostic significance of MRI findings in patients with myxoid-round cell liposarcoma.

OBJECTIVE: The aims of this study were to determine the prognostic significance of MRI findings in patients with myxoid-round cell liposarcomas and to clarify which MRI features best indicate tumors with adverse clinical behavior. MATERIALS AND METHODS: The initial MRI studies of 36 pathologically confirmed myxoid-round cell liposarcomas were retrospectively reviewed, and observations from this review were correlated with the histopathologic features. MR images were evaluated by two radiologists with agreement by consensus, and both univariate and multivariate analyses were conducted to evaluate survival with a median clinical follow-up of 33 months (range, 9-276 months). RESULTS: Statistically significant MRI findings that favored a diagnosis of intermediate- or high-grade tumor were large tumor size (> 10 cm), deeply situated tumor, tumor possessing irregular contours, absence of lobulation, absence of thin septa, presence of thick septa, absence of tumor capsule, high-intensity signal pattern, pronounced enhancement, and globular or nodular enhancement. Of these MRI findings, thin septa (p < 0.05), a tumor capsule (p < 0.01), and pronounced enhancement (p < 0.01) were associated significantly, according to univariate analysis, with overall survival. Multivariate analysis indicated that pronounced enhancement was associated significantly with overall survival (p < 0.05). CONCLUSION: Contrast-enhanced MRI findings can indicate a good or adverse prognosis in patients with myxoid-round cell liposarcomas.

[A case of pulmonary Langerhans' cell histiocytosis with liver involvement].

We report a case of pulmonary Langerhans' cell histiocytosis with liver involvement in an adult. An asymptomatic 51-year-old man who had, until his referral to our hospital, been a smoker was suspected on examination to have multiple metastases to the lungs and liver. Chest CT showed multiple micronodules, located predominantly in the upper and middle lung fields. Many of these nodules were cavitary. Abdominal US showed multiple, well-defined, round and oval nodules of decreased echogenicity within the liver. Hepatic nodules were enhanced rapidly by contrast-enhanced CT. Comparatively large nodules were demonstrated as well-circumscribed hypoattenuating lesions with ring enhancement. Since further work-up did not demonstrate any evidence of malignancy, open lung biopsy was performed. The results of open lung biopsy were consistent with Langerhans' cell histiocytosis. The infiltrates consisted of variable numbers of Langerhans' cells with folded nuclei and eosinophils. Immunohistochemically, the histiocytes were positive for S-100 protein and CD1a, and negative for CD68. The nodules regressed within 5 months after smoking cessation, and the patient became free of disease 10 months after the initial presentation.

Chronic cystic lung disease: diagnostic accuracy of high-resolution CT in 92 patients.

OBJECTIVE: The objective of this study was to determine whether the various chronic cystic lung diseases can be differentiated on the basis of the pattern and distribution of abnormalities on high-resolution CT. MATERIALS AND METHODS: High-resolution CT scans in 92 patients with chronic cystic lung diseases (18 with pulmonary Langerhans cell histiocytosis, 18 with pulmonary lymphangioleiomyomatosis, 17 with usual interstitial pneumonia, 16 with lymphocytic interstitial pneumonia, 15 with emphysema, and eight with desquamative interstitial pneumonia or respiratory bronchiolitis interstitial lung disease) were retrospectively assessed by two independent observers without knowledge of the clinical or pathologic data. The observers recorded the abnormalities, the most likely diagnosis, and the degree of confidence in that diagnosis. RESULTS: The two observers made a correct first-choice diagnosis in 148 (80%) of 184 interpretations. The correct diagnosis was made in 100% of interpretations of usual interstitial pneumonia, 81% of desquamative interstitial pneumonia or respiratory bronchiolitis interstitial lung disease, 81% of lymphocytic interstitial pneumonia, 77% of emphysema, 72% of lymphangioleiomyomatosis, and 72% of Langerhans cell histiocytosis. The two observers made a diagnosis with a high degree of confidence in 105 (57%) of 184 interpretations. The confident diagnosis was correct in 98 (93%) of 105 interpretations. CONCLUSION: Although various chronic cystic lung diseases often have a characteristic appearance that allows their distinction on high-resolution CT, considerable overlap exists among the CT findings. Therefore, lung biopsy is often required for a definitive diagnosis.

CT and MRI features of recurrent tumors and second primary neoplasms in pediatric patients with retinoblastoma.

OBJECTIVE: The aim of our study was to describe the CT and MRI findings of recurrent tumors and second primary (malignant and benign) neoplasms in patients with retinoblastoma and to evaluate imaging features to assist in distinguishing them. MATERIALS AND METHODS: Records of 445 pathologically confirmed retinoblastomas were retrospectively reviewed. Thirty-four patients with recurrent retinoblastomas and 15 patients with second primary neoplasms who underwent CT and MRI were evaluated by two radiologists with agreement by consensus. RESULTS: Invasive patterns of recurrent tumors included type A, intraocular tumor (n = 13); type B, intraorbital tumor with spread into the optic nerve shown as enlargement and marked enhancement of the optic nerve on contrast-enhanced CT or MRI (n = 6); and type C, tumor extending to the lateral aspect of the orbit and invading the brain via the sphenoidal bone (n = 2). Thirty-eight percent of patients with recurrent tumors had distant metastases (n = 7) or leptomeningeal metastases (n = 6). Leptomeningeal metastases were found only in recurrent tumors. Second primary neoplasms included osteosarcoma (n = 5), rhabdomyosarcoma (n = 5), meningioma (n = 4), and other tumors (n = 3). A significant difference was seen between the patients' ages at the time of diagnosis of recurrent tumors and second primary neoplasms (p < 0.0001). Extraorbital tumors were found more frequently among second primary neoplasms than among recurrent tumors (p < 0.001). CONCLUSION: Both recurrent tumors and second primary neoplasms in patients with retinoblastoma often show characteristic imaging features. The tumor distribution on CT and MRI may help in differentiating recurrent tumors and second primary neoplasms.

Contrast-enhanced dynamic computed tomography for the evaluation of tumor angiogenesis in patients with lung carcinoma.

BACKGROUND: The objective of this study was to investigate the role of contrast-enhanced dynamic computed tomography (CT) in the evaluation of tumor angiogenesis in patients with lung carcinoma and to assess its importance in predicting tumor size and lymph node involvement. METHODS: Dynamic CT scans were evaluated retrospectively in 130 patients with primary lung carcinoma who did not have distant metastasis and who underwent surgical resection with mediastinal lymph node dissection. Histopathologic findings of vascular endothelial growth factor (VEGF) expression and microvessel density (MVD) were correlated with the maximum attenuation values of time-attenuation curves (MAV) on dynamic CT scans. RESULTS: The MAV in VEGF positive tumors was greater compared with the MAV in VEGF negative tumors (66.1 +/- 4.6 vs. 30.9 +/- 2.7, respectively; P < 0.0001). An association was found between the MAV in VEGF positive tumors and MVD (correlation coefficient = 0.650; P < 0.0001). No difference was found in tumor size with pathologic confirmation and the MAV, VEGF expression, or MVD. The MAV, VEGF expression, and MVD in lung tumors with lymph node involvement were greater compared with the same values in lung tumors without lymph node involvement. CONCLUSIONS: The MAV of dynamic CT may be a predictor of tumor angiogenesis in patients with lung carcinoma and lymph node involvement.

Thoracic manifestation of Wegener's granulomatosis: CT findings in 30 patients.

Our objective was to describe the CT findings of thoracic Wegener's granulomatosis. At presentation, both conventional and thin-section CT scans were available in 30 patients with Wegener's granulomatosis. Serial CT scans (range of intervals: 1-25 months, mean 4.5 months) were available in 20 patients. The initial and follow-up CT scans were analyzed retrospectively by two observers in terms of pattern and extent of parenchymal and airway lesions. Positive CT findings were seen in 29 of 30 (97%) patients at initial presentation. The most common pattern was nodules or masses seen in 27 of 30 (90%) patients. They were multiple in 23 of 27 (85%) patients, bilateral in 18 (67%), subpleural in 24 (89%), and peribronchovascular in 11 (41%) in distribution. Bronchial wall thickening in the segmental or subsegmental bronchi was seen in 22 (73%) patients. Large airways were also abnormal in 9 (30%) patients. Patchy areas of consolidation and ground-glass opacity were seen in 7 (23%) patients, respectively. In 17 of 20 (85%) patients in whom follow-up CT scans were available, the parenchymal or airway lesion showed complete or partial improvement with treatment. The CT findings of Wegener's granulomatosis, although multiple and variable, consist mainly of bilateral subpleural or peribronchovascular nodules or masses and bronchial wall thickening in the segmental or subsegmental bronchi. Parenchymal and airway lesions improve with treatment in most patients.

Role of contrast-enhanced dynamic CT in the diagnosis of active tuberculoma.

STUDY OBJECTIVES: To assess the role of contrast-enhanced dynamic CT in the diagnosis of active tuberculoma. SETTING: Hospitals with an isolated ward for tuberculosis. METHODS: Fifty-five subjects with newly diagnosed active tuberculoma and 24 subjects with inactive tuberculoma were examined and evaluated retrospectively. Six subjects with active tuberculomas and seven subjects with inactive tuberculomas were confirmed by histologic and microbiologic evaluation of resected specimens, whereas the remainder of the subjects with tuberculoma were confirmed clinically. The subjects were receiving iopamidol, 370 mg/mL IV, at a rate of 3.0 mL/s on contrast-enhanced dynamic CT. The time-attenuation curve was obtained and adapted to a gamma function. The peak height (PH), maximum attenuation subtracted by the background attenuation, relative flow (RF), and mean regional flow were used for comparison. MEASUREMENTS AND RESULTS: In the surgically confirmed group, the PH and RF values of six subjects with active tuberculomas were significantly higher than those of the seven subjects with inactive tuberculoma (p < 0.05). Similarly, in the subjects with noninvasive diagnoses, the PH and RF values of 49 subjects with active tuberculoma were significantly higher than those of the subjects with inactive tuberculoma (mean +/- SD PH, 43.4 +/- 4.1 Hounsfield units [HU] vs 11.6 +/- 2.7 HU, p < 0.0001; RF, 0.012 +/- 0.001/s vs 0.006 +/- 0.001/s, p < 0.05). When the cutoff value was defined as mean +/- 2 SD, the sensitivity and specificity of the diagnosis for active tuberculoma were 77.1% and 96.4% in PH, and 68.5% and 88.8% in RF, respectively. CONCLUSION: Contrast-enhanced dynamic CT is a potentially valuable tool for the diagnosis of active tuberculoma.

Radiologic manifestations of proximal-type epithelioid sarcoma of the soft tissues.

OBJECTIVE: Our study was conducted to summarize the clinical and radiologic presentations and imaging features of proximal-type epithelioid sarcoma, a rare aggressive soft-tissue sarcoma, in 16 patients. CONCLUSION: The chest wall, inguinal region, thigh, and perineum were the most common sites of the disease. Tumors exhibited multilobulated contours in 87.5% of the patients, which may be suggestive of this tumor type. Nodal involvement was identified on CT and MR imaging and confirmed pathologically in 62.5% of the patients. Although CT or MR findings are nonspecific, proximal-type epithelioid sarcoma shows a multinodular configuration and admixes with regional lymph node metastasis on imaging studies.

Solitary fibrous tumor of the pleura: MR appearance and enhancement pattern.

PURPOSE: The aim of the current study was to characterize the MR appearance of solitary fibrous tumor of the pleura (SFTP). METHOD: Twenty-two consecutive patients with histologically confirmed SFTP were retrospectively evaluated with MRI. RESULTS: Tumors demonstrated low signal intensity (n = 18, 82%), iso signal intensity (n = 2, 9%), and high signal intensity (n = 2, 9%) on T1-weighted images. On T2-weighted images, tumors were observed as low signal intensity (n = 10, 45%), high signal intensity (n = 2, 9%), and mixed signal intensity (n = 10, 45%). Solid component in the proliferation of tumor cells corresponded to low signal intensity in 12 tumors (55%) on T2-weighted images. On gadolinium-enhanced T1-weighted images, inhomogeneous enhancement (n = 18, 82%), nodular enhancement (n = 6, 27%), and homogeneous enhancement (n = 4, 18%) were observed. The enhanced lesion within the tumors had dense tumor tissue and dilated microvessels in the pathologic specimen. CONCLUSION: SFTP shows variable appearance and enhancement pattern on MRI according to morphologic tumor heterogeneity.

Lung tumors evaluated with FDG-PET and dynamic CT: the relationship between vascular density and glucose metabolism.

PURPOSE: The aim of the current study was to evaluate the relationship between FDG-PET and dynamic CT in lung tumors. METHOD: Forty consecutive patients with pulmonary tumors underwent whole-body FDG-PET and contrast-enhanced dynamic CT. The size of tumors was 2.6 +/- 0.2 cm (mean +/- SD) at the largest diameter. The standardized uptake value (SUV) of FDG-PET, peak attenuation (APA), and relative flow (RF) of dynamic CT were evaluated. All patients underwent surgery, and tissue samples were available to be studied. The intratumoral microvessel densities (MVDs) were counted and compared with the radiologic parameters. The duration between radiologic examinations and surgery was within 2 weeks in all patients. RESULTS: The mean SUV, APA, and RF of lung cancers were significantly higher than those of benign lesions (p < 0.05). The mean APA and RF of lung cancers correlated with mean SUV (APA : r = 0.665, p < 0.0001; RF: r = 0.848, p < 0.05) and mean MVD (APA: r = 0.801, p < 0.0001; RF: r = 0.723, p < 0.05). The mean SUV of lung cancers correlated with the mean MVD (r = 0.612, p < 0.001). No correlation was found between the mean APA, RF, SUV, and MVD in benign tumors. CONCLUSION: The APA and RF of dynamic CT correlated with the SUV of FDG-PET imaging in lung cancer. The APA and RF of dynamic CT as an index of blood pooling may be related to increased glucose metabolism in lung cancer.

Primary malignant fibrous histiocytoma of the chest wall: CT and MR appearance.

PURPOSE: The purpose of this work is to describe the CT and MR appearance of primary malignant fibrous histiocytoma (MFH) of the chest wall. METHOD: Eleven men and eight women (45-76 years old) with primary MFH of the chest wall who underwent both CT and MRI were enrolled, and the imaging interpretation was retrospectively compared to the pathologic specimen. RESULTS: All tumors were inhomogeneous in appearance on CT scans. All tumors showed high signal intensity on T2-weighted images. On T1-weighted MR images, tumors displayed inhomogeneous isosignal intensity in 15 cases (79%) and low signal intensity in 5 (21%) compared with the surrounding muscle. Tumors exhibited inhomogeneous enhancement in all except three localized tumors on enhanced CT and MRI. Invasion of intercostal muscle was noted on MR images in 18 patients (95%) and on CT in 11 patients (58%). CONCLUSION: There might be various radiologic appearances of MFH. However, CT and MRI are able to demonstrate the exact localization and disease extent of MFH arising in the chest wall.

Desmoplastic small round cell tumor: imaging findings associated with clinicopathologic features.

PURPOSE: To describe imaging findings of desmoplastic small round cell tumor (DSCRT) and to clarify the relation between radiologic appearances and clinicopathologic features. MATERIALS AND METHODS: CT scans and MRI examinations of four male patients with histologically confirmed DSCRT (mean age = 20 years) were retrospectively analyzed. RESULTS: The common imaging finding was multiple peritoneal masses with regular contour situated within mesentery. Tumors showed central low attenuation in 75% of patients on nonenhanced CT. All tumors showed inhomogeneous enhancement on CT. Small foci of punctate calcification were identified in a part of the tumor in all patients. Bone metastases were identified on enhanced CT in two patients 2 and 8 months after diagnosis, respectively. Pleural dissemination was identified in all patients, and one patient had double-sided dissemination. T2-weighted MRI showed inhomogeneous high signal intensity, and small cysts were identified in two patients. Fluid-fluid levels were identified in six tumors on T2-weighted images, which suggested the presence of hemorrhage. Tumors had inhomogeneous signal intensity low or isointense relative to skeletal muscle on T1-weighted images. Two lesions exhibited inhomogeneous enhancement with central low intensity consistent with necrosis. CONCLUSION: Imaging studies can depict disseminated characteristics and disease extent during the clinical course of DSRCT.

Adult neuroblastoma: radiologic and clinicopathologic features.

PURPOSE: The purpose of the current study was to define the imaging findings of adult neuroblastoma (ANB) and correlate them with clinicopathologic features. MATERIALS AND METHODS: The CT scans and MR images of six patients with histologically confirmed ANB (mean age, 49 years) were retrospectively analyzed. RESULTS: The arising sites of tumors included the retroperitoneum (n = 2), pelvis (n = 2), anterior mediastinum (n = 1), and adrenal gland (n = 1). The common CT finding was poorly marginated and heterogeneous mass without calcification. Cystic components were often found. On contrast-enhanced CT or MR images, tumors showed heterogeneous enhancement. On T2-weighted images, all masses demonstrated predominantly hyper signal intensity relative to skeletal muscle and the images showed heterogeneous appearance with focal areas of high intensity interspersed with septations of low signal intensity. Soft tissue masses with bone involvement were identified in one case on MR images. The CT findings in one tumor originating from mediastinum were similar to those of tumors arising from other sites. CONCLUSION: ANB is an uncommon malignancy that often pursues an aggressive clinical course, involves multiple sites, and has a poor prognosis. Tumors usually manifest on CT or MR images as a poorly demarcated mass mainly in the retroperitoneum and pelvis. Imaging studies can depict aggressive characteristics and disease extent of ANB.

Gastrointestinal stromal tumor. Correlation of computed tomography findings with tumor grade and mortality.

OBJECTIVES: To assess computed tomography (CT) findings to assist in the distinction of low- and high-grade gastrointestinal stromal tumors (GISTs) and to estimate their relative risk (RR) for mortality. METHODS: Sixty-nine patients with clinicopathologically and immunohistochemically proven GISTs, including 44 patients with low-grade tumors and 25 with high-grade tumors, who underwent dual-phase CT for initial examination were included. Images were assessed retrospectively for tumor size, location, epicenter, types of surface, boundary, presence of invasion, enhancement pattern, hepatic metastasis, and peritoneal dissemination. Their RR for mortality was estimated by using a multiple logistic regression model. RESULTS: Statistically significant CT findings favoring a diagnosis of high-grade GIST and affecting the 5-year survival rate included a lesion larger than 11.1 cm (median + 1 SD), irregular surface, unclear boundary, presence of invasion, heterogeneous enhancement, hepatic metastasis, and peritoneal dissemination. Multivariate analysis showed RRs for mortality in lesions larger than 11.1 cm (RR=3.9), with the presence of wall invasion (RR=5.1), and with hepatic metastasis (RR=11.3), respectively. CONCLUSIONS: The CT features that suggest a high-grade GIST and predict poor outcome include hepatic metastasis, presence of wall invasion, and lesions larger than 11.1 cm.

Primary dedifferentiated liposarcoma of the retroperitoneum. Prognostic significance of computed tomography and magnetic resonance imaging features.

OBJECTIVES: To describe computed tomography (CT) and magnetic resonance (MR) imaging findings and to determine the prognostic significance of radiologic appearances in primary dedifferentiated liposarcoma of the retroperitoneum. METHODS: Initial CT and MR imaging studies of 20 pathologically confirmed cases of primary dedifferentiated liposarcoma of the retroperitoneum were retrospectively reviewed and assessed for correlations with the histopathologic features. CT and MR images were evaluated by 2 radiologists with agreement by consensus, and univariate analyses were conducted to evaluate survival with a mean clinical follow-up duration of 47 months (range, 5-114 months). RESULTS: Tumor invasion was more frequent in the anterior or posterior pararenal originating tumors than in pararenal tumors (P<0.05). Well-defined nonlipomatous masses juxtaposed with fatty tumors were identified in all cases. Calcification or ossification was seen in 6 patients (30%) on unenhanced CT. Imaging findings including attenuation, signal characteristics, and enhancement patterns of nonlipomatous masses were nonspecific regardless of histologic variances. Recurrent tumors (n=6) tended to invade surrounding organs. Univariate analysis revealed that calcification or ossification (P<0.05) and first recurrence with duration of a mean 13 months (P<0.05) identified by imaging studies had significant impacts on overall survival. CONCLUSIONS: Calcification or ossification and first recurrence identified by CT and MR imaging studies are significant adverse prognostic factors in primary dedifferentiated liposarcoma of the retroperitoneum.

Synovial sarcoma of the soft tissues: prognostic significance of imaging features.

OBJECTIVE: This study assessed the prognostic value of computed tomography (CT) and magnetic resonance (MR) imaging features in synovial sarcoma of the soft tissues. METHODS: CT and MR imaging studies were performed in 30 patients with pathologically confirmed synovial sarcoma of the soft tissues. CT and MR imaging findings obtained by 2 radiologists with agreement by consensus were compared for histopathologic features including tumor grade. Univariate analyses were conducted to clarify the impact of imaging findings on overall survival with a medium duration of 32 months. Multivariate analysis was estimated using a Cox proportional hazards model with the relative risk of each variable. RESULTS: Statistically significant imaging findings favoring a diagnosis of high-grade tumor included proximal distribution (P < 0.01), large tumor size (>10 cm, P < 0.05), the absence of calcification (P < 0.05), tumor possessing cyst (P < 0.01), the presence of hemorrhage (P < 0.05), and the presence of triple signal pattern (P < 0.05). Univariate analysis showed that proximal distribution (P < 0.05), tumor size larger than 5 cm (P < 0.01), the absence of calcification (P < 0.01), the presence of hemorrhage (P < 0.05), and the presence of triple signal pattern (P < 0.05) had a significant association with the disease-free survival (DFS). Multiple logistic regression models revealed that tumor size larger than 10 cm had a significant impact on the DFS with relative risk of 18.8 (P < 0.05). CONCLUSION: CT and MR imaging studies allow prognosis prediction in patients with synovial sarcoma of the soft tissues.
