Intraneural hemangioma of digital nerve diagnosed with MR imaging.

Hemangioma may occur at any location. Although the imaging findings of hemangioma are well known, it may be difficult to differentiate this entity from other soft-tissue tumors. Intraneural hemangiomas are extremely rare. We present a girl with a painful swelling in the fourth web space of her right hand. Since it was painful, the referring clinician considered the possibility of a glomus tumor. However, demonstration of dilated feeding or draining vessels suggested the diagnosis of hemangioma. At surgery a hemangioma of the ulnar digital nerve of the ring finger was resected marginally.

Efficacy of transvaginal contrast-enhanced MRI in the early staging of cervical carcinoma.

The objective of this study was to evaluate the efficacy of transvaginal contrast for local staging of cervical carcinoma. Fifty patients diagnosed with cervical carcinoma prospectively underwent magnetic resonance (MR) imaging before and after vaginal opacification (VO) with a mixture of 25 ml saline and 25 ml barium. T2-weighted (T2W) TSE images in axial and sagittal planes were compared before and after vaginal opacification. Dynamic T1W images in sagittal and fat-suppressed T1W images in transverse planes were also evaluated after intravenous contrast administration. Involvement of vaginal wall, lumen, and fornices; parametrium; rectum; and bladder were noted. Changes in local tumor staging and in treatment planning were also assessed after vaginal opacification. MR results were later compared with surgical pathological findings. Twenty-eight patients who went through surgical staging were included in the study. VO did not change any of the MR interpretations in 14 patients (50%). Correct staging was achieved with T2W TSE images with and without VO (in sagittal and transverse planes) in 78.5% and 50% of the patients, respectively. VO correctly lowered staging in seven and increased it in three patients compared with sagittal standard T2W images. Treatment planning was also changed in four (14%) of these patients. When overall accuracy of MR staging to indicate the appropriate treatment was evaluated, patients would have received the proper treatment in 90% and 79% of the cases when only T2W sagittal images with and without VO were evaluated, respectively. Dynamic gradient-echo images in sagittal planes and postcontrast T1W images in transverse planes evaluated with T2W series after VO, accomplished correct staging in 23 (82%) of the patients. MRI staging in early cervical cancer may be difficult and overestimated, especially if the tumor is slightly extended into the proximal vagina. Use of vaginal contrast medium is an easy, well-tolerated, and effective method to better delineate the borders of the tumor. It increases the specificity and accuracy of MR staging by showing the exact relation of the tumor with the vaginal wall and beyond.

Effects of prophylactic thyroid hormone replacement in euthyroid Hashimoto's thyroiditis.

Hashimoto's thyroiditis is the most frequent autoimmune thyroid disease. L-thyroxine therapy can reduce the incidence and alleviate the symptoms of this disease. The aim of this study was to evaluate the effects of prophylactic L-thyroxine treatment on clinical and laboratory findings of patients who were euthyroid at the time of diagnosis. Thirty-three patients who had diagnosis of euthyroid Hashimoto's thyroiditis were randomized to two groups, one group received prophylactic L-thyroxine treatment and the other was followed-up without treatment. Initial thyroid function tests, autoantibodies, ultrasonography, fine needle aspiration biopsy and peripheral blood lymphocyte subsets were similar in the two study groups. After 15 months of L-thyroxine treatment, there was a significant increase in free T4 and a significant decrease in TSH and anti-thyroglobulin antibody anti-thyroid peroxidase antibody levels. CD8+ cell counts increased in both groups, CD4/CD8 levels decreased significantly because of the increase in CD8+ cell count levels. Though there was no change in cytological findings, ultrasonography showed a decrease in thyroid volume in L-thyroxine receiving patients whereas an increase was detected in patients who were followed without treatment. In conclusion, prophylactic thyroid hormone therapy can be used in patients with Hashimoto's thyroiditis even if they are euthyroid.

Abdominal MDCT: liver, pancreas, and biliary tract.

Multidetector row computed tomography (MDCT) enables rapid thin-section acquisition of regional body anatomy. MDCT is adapted to hepatic and pancreatic imaging to produce, in appropriate clinical circumstances, a multipass multiplanar study obtained during defined circulatory phases to best outline vasculature and detect and characterize focal parenchymal lesions. In this chapter, a rational approach to hepatic and pancreatic MDCT acquisition is outlined with emphasis on circulatory phases and contrast material pharmacokinetics as well as biological characteristics of focal parenchymal lesions.

Solitary cysticercosis in the intermuscular area of the thigh: a rare and unusual pseudotumor with characteristic imaging findings.

A 33-year-old woman with cysticercosis presenting as a solitary tumor between the adductor muscle groups of the thigh is reported. Although such solitary soft tissue cysticercosis is rare and there are no sporadic reports in the literature with an intermuscular presentation, the typical appearance of the larvae within the mass made the diagnosis in this case easier compared with other cases of solitary soft tissue cysticercosis reported in the literature.

"Black" contrast effect during magnetic resonance arthrography attributable to inadvertent administration of excessive gadolinium chelates.

Excessive intra-articular administration of gadolinium chelates during magnetic resonance (MR) arthrography results in decreased signal throughout the joint on MR images. A case with this condition during MR arthrography of the hip is described, and a possible solution when this problem occurs (ie, performing the MR imaging examination after a waiting period of several hours) is suggested.
