[Prevention of postoperative complications in diffuse purulent peritonitis in children].

A complex pathogenetic approach to the prevention of postoperative complications in generalized fibrinopyogenic peritonitis is suggested. It includes a wide operative access, one-stage cleansing with antiseptics and detoxification solutions, ultrasonic cavitation of the abdominal cavity, creation of laparostomy, local infiltration of the wound edges with 0.01% thymalin solution, correction of hemostasis disorders, and increase of the organism's general reactivity. With the use of this complex the results of treatment were significantly improved in 21 children and the number of complications was much less than in the control group of patients subjected to operation.

[Predicting the course of wound healing after appendectomy using the hydroxyproline test].

A test for free hydroxyproline in blood plasma, urine, and wound exudate was conducted in a group of children who were operated on for acute catarrhal and destructive forms of appendicitis in an uncomplicated or complicated post-operative wound process. The changes in the concentration of the biochemical index were most regular in the wound exudate. Free hydroxyproline increased after the operation, reached highest values in 48 hours, and then reduced coming close to the control values 5-6 days after. In wound complications it increased three- to four-fold as compared to the control values, which could serve as an informative test in predicting the course of the postoperative wound process in children.

[Economic substantiation of intensifying the activities of pediatric surgical departments].

The article calculates the economical efficacy and averted economical loss in intensive work of a pediatric surgical department. The introduction of a complex of therapeutic and preventive measures producing a purposeful effect on the wound process made it possible to reduce postoperative complications to 0.3% and the general hospital stay to 3.0 days in a group of patients with "minor" surgical diseases and to hospitalize additionally at least 900 patients annually. More than 203,000 roubles are saved annually due to a shorter stay in the hospital and increased bed turnover.

[Experience with the use of methods of improving the healing of surgical wounds in children].

To improve healing of the operative wounds in children, a complex of measures is suggested, which stipulates the reduction of patient's stay in the hospital before the operation, the use of transverse skin incision, atraumaticity of the operation, meticulous layer-by-layer suturing of the skin wound, free of dressings management of the operative wounds, early removal of skin sutures. As a result, the duration of hospitalization reduced, bed turnover increased, a high cost effectiveness was achieved.

[An instrument for intraosseous punctures].

The paper outlines an intraosseous paracentesic tool, a needle-screw device that combines the properties of a drill and a hollow tube with external thread. The techniques of bone paracentesis and appropriate procedures are described. The results of examining osseous injuries while using various tools to form perforating tools have shown that the needle-screw device has advantages over others.
