[Impairments of different kinds of memory in patients with ischemic stroke].

27 patients aged 47-64 years with left hemispheric ischemic stroke with similar cognitive and educational levels were examined. Pharmacotherapy of the stroke directed to improvement of both memory and thinking was taken into consideration. To evaluate both different kinds of memory and a degree of the cognitive functions' impairments in the patients with a local ischemic stroke in anterior, posterior and middle parts of the left hemisphere the following tests were used: evaluation of both the immediate memory of the digits and the short-term audio-verbal memory; short-term trials of the pathological action of interference; Benton test, Meily test; evaluation of the declarative, procedure, semantic, strategic, mediated, long-term kinds of memory. It is shown that remembering of the new information concerning verbal names, digits, was worse in mediobasal, frontal and temporal location of the stroke in the left hemisphere and depended on clinical manifestations of the disorders. The working semantic and strategic kinds of the memory were also damaged, while long-term memory (autobiographic, professional) was impaired in less degree.

[Dynamics of the personality profile and cognitive functions in finlepsin treatment of partial epilepsy].

The aim of modern antiepileptic therapy is not only to decrease frequency of the seizures and to prevent their rise, but to decrease negative consequences of the treatment too. It is, first of all, to decrease the influence of the antiepileptic preparations on the cognitive functions and, therefore, on quality of the patients' life. During therapy of 40 patients (8-57 year-olds) with partial epilepsy the severity of the seizures as well as emotional and cognitive functions of the patients were evaluated. A significant decrease of the severity of the seizures was observed during therapy with finlepsin-retard (p < 0.002). These changes correlated both with a decrease of anxiety according to MMPI questionnaire (p < 0.0001) and with an increase of the volume of the short-term imaginary memory. Finlepsin-retard is effective therapy for epilepsy.

[Epilepsy as a clinical and neurophysiological problem].

On the basis of both own investigations and publications the next basic aspects of epilepsy were discussed: a) a clinical one--definition of epilepsy, seizures' classification, children's and women's epilepsy, status epilepticus; b) a neurophysiological aspect--epileptic and antiepileptic mechanisms, the significance of slow EEG activity, interhemispheric interactions, influence of the seizures on brain homeostasis.

[The influence of hypogeomagnetic field on bioelectric activity of the brain in epilepsy].

An influence of geomagnetic field (GMF) on bioelectric activity of the brain was studied in 20 epileptic patients by the use of "hat", which attenuated GMF. A short-term attenuated GMF increased a EEG synchronisation by alpha-rhythm, generalized slow-waves discharges. Some similarities and differences were found in attenuated GMF effects and in the results of hyperventilation trial. The main role in perception of attenuated GMF belongs to the parietal area of the right hemisphere.

[Depakin 300 and Depakin-chrono in the therapy of epilepsy].

The results of administration of Depakin 300 and Depakin-chrono during 4-year period were analysed in 137 patients aged 14-36 years with idiopathic generalized, cryptogenic, symptomatic partial and nonclassified epilepsy. Depakin was prescribed as basic medicine in idiopathic generalized epilepsy, while in partial and nonclassified epilepsy this drug was more frequently administered in combination with some other anticonvulsants. Early administration of Depakin was highly effective: a remission was observed in 91.2% of the patients during a year. During 3 years a remission was observed in 75.4%, frequency of the fits decreased more than by 50%--in 14.1% of the cases, no response was found in 10.5% of the patients (in cases of gross structural changes of the brain, inefficiency of the preceding therapy during 5 years). Frequency of side-effects was 1.7-12.3%. Indications to treatment are discussed: in addition to generally accepted recommendations (absence, myoclonic primary generalized, tonic-clonic forms of epilepsy, of reading, photosensitive epilepsy), application of Depakin is also recommended in nonclassified epilepsy with generalized epileptic EEG-patterns as well as a drug of choice in all cases of nonclassified fits. Depakin is also quite efficient as an additional drug in cases of partial epilepsy resistant to treatment.

[Renin-aldosterone system in secretion of cortisol in patients with arterial hypertension in European North].

Renin activity (RA), concentration of aldosterone and hydrocortisone in plasma were measured by radioimmunoassay in 78 males with arterial hypertension living in the Far North. RA and aldosterone concentrations were high in patients with borderline arterial hypertension irrespectively of hemodynamic type of the disease. Hydrocortisone levels in them were normal. In hypertension stage I and II RA and hydrocortisone concentrations were normal, while aldosterone levels have risen. Renin-aldosterone index showed high RA in all hemodynamic types of hypertension.

[The mechanisms of pain in root compression].

It is shown that antidromic stimulation of peripheral nociceptors induced by radicular compression is the initial pathogenetic factor of pain in patients with so-called paralytic sciatica and trigeminal neuralgia. Subsequently, segmental and suprasegmental mechanisms of pain retention are involved. The prevalence of one or another pain mechanisms at different stages of trigeminal neuralgia may determine the choice of adequate therapy.

[A quantitative assessment of the blood supply to the head by indirect rheography].

Clinical-experimental basis of the possibility of application of the method of indirect rheography of head (IRH) in wide clinical practice was presented. Estimation of the results reproduction and accuracy of the method was performed too. 40 healthy individuals of both sexes at the age of 18-50 and 102 patients with spinal osteochondrosis, arterial hypertension and neurocirculatory dystonia were examined. It was determined that IRH provided 93% of results reproduction and its precision was 90% and higher in comparison with ultrasound method. It was established that head circulation was 750-1200 ml/min or 12-24% of cardiac output in healthy individuals. The results of the study of head blood circulation in some groups of patients were also discussed.

[Circulatory encephalopathy in patients with arterial hypertension].

More accurate definition of the leading factors of development of discirculatory encephalopathy (DE) in patients with arterial hypertension was the aim of this investigation. The analysis of correlations between clinical and computer tomographical symptoms of DE on the one hand and the indices of central hemodynamics, head blood circulation and presence of atherosclerosis of head arteria on the other hand was performed. It was found that congestion in head venous system was the main factor in DE development. The conclusion was made that both atherosclerosis of head arteria and arterial hypertension promoted venous congestion in head by means of decreasing the amplitude of arterial pulsation and in turn the development of DE. Meanwhile, the increase of minute blood flow prevents it.

[Prefrontal epilepsy].

Prefrontal epilepsy was considered as one variation of frontal-lobar epilepsy with prefrontal location of the focus. 14 verified cases were selected as the basis of the analysis of etiological factors, clinical peculiarities and possible pathogenetic mechanisms of epileptic fits in prefrontal epilepsy: generalized convulsive attacks, seizures of sudden fall, fits of stillness and pseudofits.

[Photosensitive epilepsy].

Photosensitive epilepsy (PE) is the most frequent form of reflex epilepsy. Authors picked out two PE forms based on the analysis of the 16 own observations. The first form is PE with environmental factors of the process' provocation, which is characterised by genuine character of disease, pre- and pubertal period of its beginning, considerable prevalence of males among the patients, rare generalized tonic and clonic fits and favorable prognosis. The second form of PE is significantly rarer PE form -with self-induction of the fits. It is characterized by the beginning of the disease in preschool period, even sex distribution, quite frequent evaluated organic damages of brain, nonconvulsive fits, non-efficiency of antiepileptic drugs. The mechanisms of pathogenesis and the ways of fits' prophylaxis were discussed.

[Clinical neurology and the structural-functional organization of the nervous system].

The evidences of the primary role of clinical thinking over the instrumental one as well as effectiveness of the ontophylogenetic analysis of clinical phenomena are presented. From this point of view the following syndromes were analysed: pyramidal, prefrontal cortical epilepsy, amnestic aphasia, deafferent pain. The role of denervation hypersensitivity in the epileptogenesis was also examined. The clinical data served the basis for validation of the role of antidromal irritation in pathogenesis of pain in radicular compression. The problem of hemispheres' interactions was analysed too.

[The action of a magnetic field on the bioelectrical activity of the brain in healthy subjects and epilepsy patients].

The influence of reduced geomagnetic field as well as alternative magnetic field action on the brain bioelectric activity was analysed in 18 healthy individuals and in 20 epileptic patients. The alteration of magnetic field elevated functional activity of the brain synchronizing structures and increased either epileptic activity or activation of epileptic focus. The data allow to conclude that the magnetic field effects are mediated by the deep sensory nerve-pathways. The role of the right hemisphere in magnetic field perception and participation of back temporal areas in these processes are considered.

[Lamiktal in the treatment of epilepsy patients].

Lamictal (Lamotrigin) action was studied in 61 patients with partial and generalized epileptic seizures (not fewer than twice a month). Most of the patients received Lamictal in doses of 200-300 mg daily as well as other antiepileptic medications (before lamictal all patients were treated without effect with various antiepileptic medications). Effectiveness of treatment with lamictal includes: the cessation of epileptic attacks (5 patients), its more rare appearance (31 patients), transformation into more light fits (15 patients). Lamictal has a broad spectrum of antiepileptic activity: it stops generalized tonic-clonic seizures, absences, simple and complex partial seizures as well as secondarily generalized tonic-clonic seizures. The medication is also effective in patients with psychopathologic symptoms.
