[Surgical treatment of local-expanded forms of thyroid carcinoma].

An analysis of surgical treatment of 377 patients with advanced thyroid carcinoma is presented. The work included 82 combined, 90 expanded and 218 palliative operations (as well as 68 shave-resections). In 56 cases surgical interventions were two-or three-staged. This approach improved the immediate results and did not change the long-term results of treatment. It was shown that surgical treatment in advanced thyroid carcinoma gave good results in the majority of cases (recovery or long-term palliation). Combined and expanded operations were non effective in anaplastic thyroid carcinoma. In well-differentiated thyroid carcinoma the 10-year survival was noted in 70.2% of patients.

[Surgical strategies in the diagnosis and treatment of patients with differentiated thyroid cancer].

Results of examination of 215 patients with differentiated thyroid cancer (papillary, follicular, medullary) with various degrees of involvement of regional lymph nodes in the process are presented. The follow-up observation lasted from 2 till 10 years. It was established that metastases most frequently affect the jugular group of nodes so it was chosen for investigation in order to detect the micro-metastases at the subclinic stage. Division of the surgical treatment into two stages (biopsy of the regional lymph nodes and lymphodissection within 2-3 months in case of metastases) has allowed to improve early results and not worsen long-term results of treatment.

[Breast cancer in thyroid diseases].

The authors have analyzed 48 cases of breast cancer detected among more than 16,000 patients with thyroid pathology including thyroid carcinoma. It was established that breast cancer associated with thyroid pathology (thyroid cancer included) had specific features of the clinical course. Relative risk of breast carcinoma in thyroid cancer patients was not higher than the incidence in female population. Possible etiologic and pathogenic factors of the relationship between thyroid pathology and breast tumors are discussed. It is supposed that hypersecretion of prolactin and gonadotropins might play some role in such association. Radiotherapy and chemotherapy for breast carcinoma also can be predisposing factors to the development of subsequent thyroid pathology.

[Simultaneous operations in patients with thyroid diseases].

Twenty-four cases of simultaneous surgical treatment for coexisting diseases of the thyroid gland and other organs are discussed. Such operations were mainly performed for associated nodular goiter (including thyroid cancer) and benign or malignant tumours of other localisations. As a rule surgical interventions were planned beforehand. Only in 2 cases there were urgent operations for coexisting diffuse toxic goiter and complicated ulcer disease. In 4 cases thyroid resection and laparoscopic cholecystectomy were made simultaneously. Immediate and long-term outcomes were good. The indications and conditions of performing the operations are discussed.
