[Interstitial (lymphogenous) disseminated pulmonary tuberculosis: diagnostic capacities of computed tomography].

Computed tomography (CT) as a priority radiation diagnostic technique for interstitial lung diseases was used to examine 35 patients with disseminated interstitial lymphogenous tuberculosis (ILT). Individuals having ILT amounted to 29% of the total number of the patients with disseminated pulmonary tuberculosis. The major CT markers of ILT are interstitial diffuse bilateral lesion with the reticular or reticulonodular macrostructure. ILT is characterized by its lymphogenous origin with involvement of the superficial and peripheral lymphatic network, by a productive inflammatory reaction showing no tendency to consolidation and cavernization, by none or meager bacterial isolation, by a negative or insignificant tuberculin sensitivity, as well as by difficulties in the identification of a granulomatous component. In terms of the level of damage to the interstitial structures, ILT presented with predominant intralobular interstitial lesion as small reticularity in 16% of the patients, with predominant intralobular interstitial lesion as large reticularity in 59%, and with predominant peribronchovascular interstitial lesion in 25%. The identified CT types of ILT are noteworthy for a chemotherapeutic approach and its results and for the degree of development of interstitial fibrosis.

[Computed tomography in diagnosis of post-tuberculous intrathoracic lymphadenopathies].

Among 454 adults with various lung diseases, patients with calcified intrathoracic lymph nodes (CITLN) formed after latent and spontaneously cured primary tuberculosis infection were identified by computed tomographic findings. Among the patients with tuberculosis, there were 6% of cases with CITLN, 68% with central cancer, 46% with peripheral cancer, 37% with pneumonia. Among the patients with tuberculosis, CITLN as a possible source of reactivation of tuberculosis infection and occurrence of secondary tuberculosis had no substantial impact on the occurrence of secondary tuberculosis in adults. The pathogenetic significance of CITLN can be followed in malignant tumors, bacterial pneumonias, and in central cancer of the lung in particular.

[Lomefloxacin in complex treatment of acute progressive form of pulmonary tuberculosis].

The effect of lomefloxacin in combination with isoniazid, pyrazinamide, streptomycin or ethambutol on the process of tuberculosis and concomitant nonspecific pathological processes in the lungs was investigated in the treatment of 69 patients with newly diagnosticated acute progressive destructive tuberculosis of the lungs and isolation of Mycobacterium tuberculosis. It was shown that lomefloxacin provided rapid disappearance of the disease clinical signs and bacillus isolation in the patients with tuberculosis due to the tubercle bacilli susceptible or resistant to the main antituberculosis drugs, acceleration of the resolution of the infiltrates and caseous lesions in the lungs and elimination of the pathological processes in the bronchi. The tolerance of lomefloxacin was satisfactory and it did not affect the tolerance of the other antituberculosis drugs.

[Combined chemotherapy of patients with tuberculosis - new regimens and dosage forms].

310 patients with pulmonary tuberculosis disseminating bacteria received isoniazid, rifampicin, streptomycin (ethambutol) and pirazinamid in different regimens and dosage forms. The drugs were administered in sequence or simultaneously, in single or divided doses. The best time and number characteristics as regards the discharge negativation were obtained in pirazinamid administration at a single dose daily or each other day irrespective of other drugs intake. 100 patients were given isoniazid, rifampicin and pirazinamid in multicomponent form tricox (Jemis, India). Tricox proved effective under additional administration of isoniazid in the same dose as was fixed in tricox.

Antitubercular Agents↗

[Tarivid in the combination therapy of patients with pulmonary tuberculosis].

The effectiveness of tarivid, a new drug having a broad spectrum of antimicrobial action and antituberculous effect, was studied. The drug was given to 29 patients with newly detected destructive pulmonary tuberculosis and bacillary excretion. Eight patients had microbacterial resistance to streptomycin, rifampicin and isoniazid, 12 had tolerance to them, in 9 patients tuberculosis was complicated by a nonspecific inflammatory process. The course of treatment ranged from 3 weeks to 3 months. The drug was well tolerated. Its effect was manifested by reduction of intoxication, resolution of the inflammatory and pericavitary pulmonary lesions and bacilli absence. Tarivid can be considered an efficacious drug in the multimodality therapy of tuberculosis patients.

[A thermo-nasal test for detecting allergy to antitubercular drugs].

A diagnostic test has been suggested for the detection of the allergic-type adverse reaction to antituberculosis drugs. The drug-allergen is applied to the nasal mucosa and the temperature reaction of the alla nasi tissues is measured with the help of a mesomorphic thermoindicative film.

[Tisamide in the complex treatment of tuberculosis].

The outcomes of tisamid treatment of newly diagnosed patients with bacillary pulmonary tuberculosis were analysed. To estimate the efficacy of tisamid and its administration indications, chemotherapy of the patients was performed with the use of two therapeutic regimens. Isoniazid, rifampicin and streptomycin (or ethambutol) were given to 73 patients, while other 72 ones were treated with the same drugs plus tisamid. The patients in each group were subdivided into slow, rapid and homozygotic (the most rapid) acetylators. Tisamid, when prescribed to newly-discovered patients with destructive tuberculosis, accelerates recovery, by excluding the risk of a hepatotoxic action. Tisamid in a combined treatment with isoniazid, rifampicin and streptomycin is mostly recommended for patients with a rapid acetylation phenotype, i.e. the cases for whom a short-term chemotherapy is possible.

[Pharmacokinetic bases for the drug-free prevention of drug-induced hepatitis in pulmonary tuberculosis patients].

The causes and frequency of occurrence of toxic hepatitides determined by isoniazid and rifampicin, were analysed in 551 patients with primary tuberculosis detected by clinical pharmacological methods. The intravenous mode of drug administration was shown to be indicated for prevention of isoniazid hepatitides in patients with a fast type of acetylation. Doses and methods of therapy with isoniazid and streptomycin leading to their high concentrations in the patients' body should be avoided to prevent liver affection with rifampicin. Since rifampicin hepatitides develop in persons with a slow type of acetylation, rifampicin and isoniazid with streptomycin should be injected with a 6-hour interval to prevent them.
