[The use of the principles of military field surgery in rendering medical care to the victims during catastrophes].

Despite the existing differences in medical care of war and catastrophe casualties it is possible to apply a variety of organizational and management principles of military medicine for the disaster medicine. The authors express the opinion that in case of big catastrophes the medical service could operate by means of rapid deployment units formed on the basis of territorial principle.

[Gunshot wounds of the arteries].

The surgical tactics for the treatment of gunshot wounds of vessels should depend not only on the kind of the injuring shell but also on the concrete dimensions of the injury of vessels and surrounding tissues. The vital capacity of the vessels and adjacent tissues should be determined by the same criteria as that of well-known and sufficiently studied gunshot wounds.

[Neurologic manifestations of ischemia of the lower extremities following vascular injury].

Disorders of magistral vessels are accompanied by disturbances of motor and sensorial functions which may be erroneously qualified as traumas of the nervous stems. Such mistakes may lead to a late restitution of the magistral vessels and consequently to a necrosis of the extremities or severe disorders of functions. These disorders are not always climineted after a late renewal of the magistral blood. In this relation a clarification of the diagnosis may exert a certain indluence on the selection of treatment, inasmuct as traumas of the magistral arteries urgen surgical treatment while as injury of the nerves are not in need of such special measures.

[Remote results of treatment of injuries of large arteries of the limbs].

The results of treatment of 115 patients with vascular injuries were studied within the terms from 3 to 20 years after the operation. In 7 patients the vessels were ligated, in the remainder--repaired. The functional state of the extremity and blood vessels was studied using oscillo- and rheography, angiography, ergometry and other methods. In 81 patients (70.5%) the extremity function was normal, in 24 (20.8%)--moderately restricted, and in 10 (8.7%)--poor.

[Injuries of the large veins of the limbs].

161 observations of the extremity vein injuries, including 141 cases with associated arterial injuries and 11 cases with isolated traumas, as well as 9 cases of replantation of the extremity in complete tears are analysed. Venous reconstruction was performed in 37 patients, in the remainder veins were ligated. Indications to reconstructive procedures and ligation of injured veins are discussed.
