[Dynamics of prevalence of ischemic heart disease and risk of cardiovascular death in open population of Tiumen].

Prevalence of ischemic heart disease (IHD) has been repeatedly studied in 3 sex and age stratified representative samples of Tyumen citizens. Prevalence of IHD in Tyumen open population is 11.7 among men and 13.2% among women. Prevalence of definite IHD is 7.1 and 8.4%, respectively. Studies conducted with 5 years interval demonstrated rise of prevalence of IHD defined according to soft criteria among women at the account of its probable forms. Relative risk of cardiovascular death rose in middle aged and elderly men and in elderly women.

[Anti-ischemic efficacy of combination of trimetazidine with enalapril in patients with stable effort angina and metabolic syndrome].

Efficacy of combination of trimetazidine (60 mg/day) and enalapril in patients with stable effort angina and metabolic syndrome was assessed in a randomized placebo controlled study of 64 patients. Anti-ischemic activity of trimetazidine was found to be significant. It was most pronounced in patients with abnormal 24-hour blood pressure index, waist/hip circumference ratio >1.0 at the background of satisfactory clinical and laboratory compensation of type 2 diabetes.

[Effect of dipyridamole on the phenomenon of ischemic preconditioning in patients with ischemic heart disease during paired bicycle ergometer tests].

AIM: To assess effect of dipyridamole on ischemic preconditioning (IP) in patients with coronary artery disease by paired bicycle ergometer exercise tests (BETs). MATERIAL AND METHODS: Patients (men, n=12, age 51.7+/-2.5 years) with class II-III stable angina pectoris underwent paired BETs (tests 1 and 2 at 15 min intervals). Test 1 was regarded as baseline and test 2 as the preconditioning one. In 72 hours another pair of BETs was carried out (tests 3 and 4) with same time interval in between. Before second pair of tests the patients received 75 mg of dipyridamole orally. RESULTS: Significant improvements of several parameters of ECG were observed during repeat BETs (increased exercise duration, maximal heart rate and double product, decreased maximal ST-depression and number of ECG leads with >1 mm ST depression). These data suggested the presence of IP phenomenon in both paired BETs. However only test after dipyridamole was associated with significant increase of work load (from 82.4+/-10.5 to 96.4+/-10.1 W, p=0.033), shortening of ST-depression recovery time (from 367+/-45 to 254+30 s, p=0.032) and increase of increment of exercise duration (from 34+/-15 to 81+/-17 s, p=0.034). Number of ECG leads on with segment ST depression >0,1 mv was significantly less during test 4 compared with test 2 (2.6+/-0.4 and 3.4+/-0.5; p=0.045). This data reflected increase of ischemic threshold during test 4. CONCLUSION: Dipyridamole (75 mg orally) augmented IP in patients with coronary artery disease during repeated BETs.

[Effect of cardiac resynchronization therapy in the treatment of chronic heart failure].

As a result of cardiac resynchronization therapy in all 10 patients with chronic NYHA class III-IV heart failure at duration of observation from 2 to 20 months was noted positive dynamics: improvement of self feeling, increase of 6 min walking distance, decrease of heart failure class, and improvement of functional parameters of the heart according to echocardiography data. Implantation of biventricular cardiac pacemaker to patients with chronic heart failure is relatively safe and accomplish-able from technical point of view. Cardiac resynchronization in patients with pronounced chronic heart failure appears to be an effective supplementation of drug therapy.

[Five-year trends in main risk factors of coronary heart disease among open population of Tyumen].

AIM: To study five-year trends in main risk factors of coronary heart disease (CHD) among 35-54-year-old population of Tyumen. MATERIAL AND METHODS: Five-year monitoring of 1597 males and females aged 35-54 years was performed as two cardiological screening trials. The risk factors were assessed both by standard and extended WHO criteria. RESULTS: Risk factors studied had the following prevalence trends: a rise in the rate of obesity (7.2-14.5%), hypercholesterolemia by standard criteria (14.4-27.8%) and hypertriglyceridemia (5.8-16.7%) among men aged 35-44 years; a rise of the rate of isolated systolic arterial hypertension by extended criteria (3.5-8.8%), smoking (10.6-19.2%), hypo-alphacholesterolemia (3.9-11.6%) among women aged 35-44 years; a rise in the rate of isolated systolic arterial hypertension (3.9-14.7%), a decrease of diastolic arterial hypertension rate by extended criteria (49.0-31.3%) among women aged 45-54 years. Five-year monitoring revealed that among 45-54-year-old men the number of those who smoke irregularly significantly increased (7.3-21.5%), while the number of non-smokers fell (44.3-21.5%). CONCLUSION: Tyumen population is characterized by a high prevalence of the risk factors with a negative 5-year trend. The results of the screenings give grounds for conduction of a large-scale prophylactic program in an open Tyumen population with a focus on priorities of prophylactic interventions.

Gene expression profiles of epithelial cells microscopically isolated from a breast-invasive ductal carcinoma and a nodal metastasis.

Expression profiles of breast carcinomas are difficult to interpret when they are obtained from tissue in toto, which may contain a large proportion of non-cancer cells. To avoid this problem, we microscopically isolated cells from a primary invasive ductal carcinoma of the breast and from an axillary node harboring a metastatic breast carcinoma, to obtain pure populations of carcinoma cells ( approximately 500) and used them for serial analysis of gene expression. The expression profiles generated from both populations of cells were compared with the profile of a disease-free mammary epithelium. We showed that the expression profiles obtained are exclusive of carcinoma cells with no contribution of non-epithelial cells. From a total of 16,939 unique tags analyzed, we detected 559 statistically significant changes in gene expression; some of these genes have not been previously associated with breast cancer. We observed that many of the down-regulated genes are the same in both cancers, whereas the up-regulated genes are completely different, suggesting that the down-regulation of a set of genes may be the basic mechanism of cancer formation, while the up-regulation may characterize and possibly control the state of evolution of individual cancers. The results obtained may help in characterizing the neoplastic process of breast cancer.

Assessment of the relative therapeutic effects of vaccines on virus load and immune responses in small groups at several time points: efficacy of mucosal and subcutaneous polypeptide vaccines in rhesus macaques exposed to SHIV.

BACKGROUND: Due to the high cost, subject availability and ethical constraints, it is often critically important in pre-clinical and clinical studies to carry out an adequate statistical analysis of longitudinal multivariate data over several time points in trials in several small groups. OBJECTIVES: We aim to accurately assess and develop an appropriate distribution-free longitudinal model for an estimate of the comparative treatment effects of several biological factors in several small groups even if data sets should contain outlier measurements and censored values. This approach is used to evaluate the relative efficacy of mucosal and subcutaneous polypeptide vaccines in rhesus macaques exposed to SHIV. STUDY DESIGN: The algorithms of the non-parametric repeated measures ANOVA models [Mack GA. A quick and easy distribution-free test for main effects in a two-factor ANOVA. Communic Stat Part B: Simp Comp 1981;10:571-91; Akritas MG, Brunner, E. A unified approach to rank tests for mixed models. J Stat Plan Inference 1997;61:249-77; Brunner E, Puri ML. Nonparametric methods in factorial designs. Stat Pap 2001;42:1-52.] are described, programmed and assessed. The viral loads, CD4(+) and CD8(+) cell counts were analyzed at several time points in peripheral blood of the 11 MamuA(*)01 positive macaques intrarectally challenged with pathogenic SHIV-Ku2 and immunized intrarectally with synthetic HIV/SIV peptide vaccine. RESULTS: Using nonparametric ANOVA tests, we demonstrated with statistical significance that after intrarectal challenge with pathogenic SHIV-Ku2, intrarectally immunized monkeys expressed viral titers that fell below the level of detection in blood and intestine (which is a major reservoir of viral replication), whereas the subcutaneously immunized or control macaques had residual viraemia. Moreover, the proliferative response of T cells and both CD4(+) and CD8(+) cells were better preserved in intrarectally immunized animals. Robustness of the comparisons was confirmed by gradual removal of up to 50% of data points. CONCLUSION: Despite limited data, our analysis shows better preservation of both CD4(+) and CD8(+) cells in intrarectally immunized animals. The result is consistent with our hypothesis that mucosal immunization is more effective than systemic immunization and that an induction of specific CTLs in the intestinal mucosa, a key site of virus replication, with a mucosal AIDS vaccine ameliorates SHIV infection in non-human primates. Our analytical methodology can be applicable in comparative estimates of the different treatment-associated effects and their synergy for a variety of longitudinal data sets in small treatment groups.

[Prevalence of dyslipidemia in open population of Tiumen'].

AIM: To evaluate levels of total cholesterol (TC), HDLP cholesterol, triglycerides (TG), occurrence of hypercholesterolemia (HCE), hypertriglyceridemia (HTG) and low HDLP cholesterol in the Tyumen population. MATERIAL AND METHODS: 866 men and women aged 25-64 years have been examined in one-stage cardiological screening within a two-stage epidemiological study of serum lipoproteins by WHO criteria. RESULTS: HCE was registered in 15.6% males and 18.8% females, a low HDLP cholesterol--in 5.3 and 4.2%, HTG--in 5.8 and 4.3% males and females, respectively. A high TC level and HCE occurred more frequently in women over 54 years of age; a low HDLP cholesterol level in insignificant fall in HDLP cholesterol and no age-related changes of this index were found both in males and females; TG were elevated in older males in moderate prevalence of HTG in the population. CONCLUSION: It is shown that Tyumen population is at high risk of cardiovascular diseases. Further studies are needed to specify targeted preventive programs.

[Knowledge of cardiovascular disease risk factors in open population of Tyumen and social gradient].

AIM: To study knowledge of cardiovascular disease (CVD) risk factors (RF) in open population of Tyumen with reference to social gradient. MATERIAL AND METHODS: Knowledge of CVD RF was studied in 2248 citizens of Tyumen (men and women, age 25-64 years) by means of mail questionnaire survey. The knowledge was assessed with consideration of education, occupation and marriage status. RESULTS: The examinees with primary education and manual labour workers were found to have little knowledge of the CVD RF while those with university education, engineers and specialists were well informed. Married population, widows and widowers knew about CVD RF more than divorced. CONCLUSION: The results of the study can serve a basis for a large-scale prophylactic program in open population of Tyumen.

General statistics of stochastic process of gene expression in eukaryotic cells.

Thousands of genes are expressed at such very low levels (< or =1 copy per cell) that global gene expression analysis of rarer transcripts remains problematic. Ambiguity in identification of rarer transcripts creates considerable uncertainty in fundamental questions such as the total number of genes expressed in an organism and the biological significance of rarer transcripts. Knowing the distribution of the true number of genes expressed at each level and the corresponding gene expression level probability function (GELPF) could help resolve these uncertainties. We found that all observed large-scale gene expression data sets in yeast, mouse, and human cells follow a Pareto-like distribution model skewed by many low-abundance transcripts. A novel stochastic model of the gene expression process predicts the universality of the GELPF both across different cell types within a multicellular organism and across different organisms. This model allows us to predict the frequency distribution of all gene expression levels within a single cell and to estimate the number of expressed genes in a single cell and in a population of cells. A random "basal" transcription mechanism for protein-coding genes in all or almost all eukaryotic cell types is predicted. This fundamental mechanism might enhance the expression of rarely expressed genes and, thus, provide a basic level of phenotypic diversity, adaptability, and random monoallelic expression in cell populations.

Mucosal AIDS vaccine reduces disease and viral load in gut reservoir and blood after mucosal infection of macaques.

Given the mucosal transmission of HIV-1, we compared whether a mucosal vaccine could induce mucosal cytotoxic T lymphocytes (CTLs) and protect rhesus macaques against mucosal infection with simian/human immunodeficiency virus (SHIV) more effectively than the same vaccine given subcutaneously. Here we show that mucosal CTLs specific for simian immunodeficiency virus can be induced by intrarectal immunization of macaques with a synthetic-peptide vaccine incorporating the LT(R192G) adjuvant. This response correlated with the level of T-helper response. After intrarectal challenge with pathogenic SHIV-Ku2, viral titers were eliminated more completely (to undetectable levels) both in blood and intestine, a major reservoir for virus replication, in intrarectally immunized animals than in subcutaneously immunized or control macaques. Moreover, CD4+ T cells were better preserved. Thus, induction of CTLs in the intestinal mucosa, a key site of virus replication, with a mucosal AIDS vaccine ameliorates infection by SHIV in non-human primates.

[Results of the one-stage screening for ischemic heart disease in the Tumen population sample].

AIM: To study prevalence of ischemic heart disease (IHD) in free living population of Tumen. MATERIAL AND METHODS: A one-stage cardiological screening covered 1608 Tumen residents (70.3% of the respondents of the mail questionnaire survey). IHD was stated by the standard methods. RESULTS: IHD incidence among male and female population of Tumen was rather high. Angina of effort was found in 4.5% of males and 5.1% of females, painless IHD was detected in 3.2 and 3.7% of male and female 55-64-year-old population, respectively, by extended criteria. IHD incidence rate by the extended criteria reached 12.5%. CONCLUSION: Primary results of the survey indicate that Tumen population is at high risk of cardiovascular diseases. This fact requires a detailed analysis of the trends in IHD prevalence in a prospective study.
