[Water-soluble phosphorus-containing forms of ionol in experimental treatment of thermal burns].

The authors studied the membrane-stabilizing effect of two water-soluble preparations of antioxidants, analogues of ionol, in thermal burns. It was found that both preparations increase somewhat to a different degree the resistance of the red cell membranes to peroxide compounds and reduce hemolysis which is very characteristic of the first minutes and hours after the thermal trauma. The preparations alter the electrical properties of the red cells and affect the activity of phospholipases in blood plasma.

[Changes in several characteristics of the erythrocyte membrane and erythrocyte structure in severe burns in animal experiments].

It is determined in experiments on rats that the IIIB degree thermal injuries of 20% of the total body surface significantly reduce the erythrocytic membrane's resistance and cause the production of peroxides in 15 minutes after trauma. Simultaneously the electrical properties begin to change. The structure of erythrocytes is also altered. In 30 minutes post injury these alterations are in progress, but in 60 minutes they are less expressed. The structure of the erythrocytes is significantly altered. The authors consider that the influence of membranotoxic factors--the products of free radicals' reactions and phospholipases--is accounted for all noticed alterations.

[The organization of care for burn victims in catastrophes].

The experience in medical care and treatment of burned patients showed that adequate anti-shock infusion therapy can be established only 4-6 hours after the sustained trauma. However, even despite such a delay, many burned patients (75-80%) get well, due to the later adequate complex treatment controlled by experienced burn specialists [correction of combustiologists] in hospital units specially organized and equipped for this category of patients. Within the first 2-4 days after the trauma, it is recommended to distribute major accident victims between different specialized burn units, using specially equipped air and motor transportation vehicles.

[Blood phospholipases in burn shock].

The activity of blood plasma phospholipases A and C has been observed during burn shock periods in patients with severe burns affecting more than 20% of the body surface. It was experimentally demonstrated that the enzyme plasma activity increased 30 minutes after burning. Simultaneously, intensification of lipid peroxidation took place. Phospholipases are suggested to liberate from the damaged tissues with their following activation by free-radical products.

[Treatment of frostbite].

The article shows that treatment of frostbites in a special medium with controlled temperature and humidity makes the time of treatment shorter and prevents complications characteristic of such a trauma. The main purpose of such exposures of the injured areas is to achieve mummification and to make the operation as early as possible. The mummification of the soft tissues began within 5-8 days after trauma. Chemical necrectomy was possible on the 7th-9th days, followed by autodermoplasty on the 2nd day.

Combined Modality Therapy

[Therapeutic plasmapheresis in acute burn toxemia].

Curative plasmapheresis was used in 10 critically burned patients at the stage of acute burn toxemia with the delirious syndrome with the unfavourable prognosis. Good immediate results were obtained in all the patients: psychic disorders were arrested by the end of the procedure. Clinico-laboratory data have shown the curative plasmapheresis to be an effective detoxicating method resulting in the elimination of various toxic substances from the organism.

Adult

[Treatment of burns with infrared rays in patient isolators].

Experience with the treatment of 82 burned patients in isolation wards with the infrared radiation has shown the level of energy losses in these patients to be lower. the incidence of infectious complications to be less, the course of the disease to be better and terms of the treatment shorter than those in patients with similar burn surfaces treated by a bandage method. An automatic regulation of the intensity of infrared radiation gave the individual treatment in accordance with the metabolism level of the patient.

Adolescent

[Determination of double bonds in the lipid fraction of blood plasma in burn patients using the ADS-4M apparatus].

An ADS-4M analyser was employed to study the diagnostic potentialities of the method for determination of double bonds (DB) in lipids of the burnt's blood as a rapid method for revealing alterations in unsaturated fatty acids (USFA). The method was tried in 25 patients with varying gravity of burn injury and in 15 healthy subjects with a purpose of establishing the norm. It was discovered that in patients with burn, the number of DB differed from normal depending on the disease gravity. The time course of changes in the number of DB attests to the necessity of the use of the method under consideration for evaluating abnormalities in the content of USFA.

Adolescent

[Acute cholecystitis in severely burned patients].

Acute acalculous cholecystitis in burned patients develops due to general disturbances of microcirculation of polyetiological origin. Early laparotomy can facilitate diagnosis of acute acalculous cholecystitis in burned patients. Rapidly developing destruction of the gallbladder walls requires an active surgical tactics.
