[Goals of medical monitoring].

The paper deals with the problems of medical monitoring, namely how to solve those in the detection and classification of human abnormalities. The medical monitor is a complex hierarchic system and the authors state problems and tasks for each level of hierarchy and the objectives of studies in medical diagnosis and prediction while designing the hierarchic network structure of a medical computer monitor. A parallel is drawn between the image recognition theory and medical diagnosis.

[Computerized medical monitors: their status and outlook].

Analysis of about 30 medical computer monitors has allowed the authors to make a conclusion on the present status of this field of medical engineering, operating properties and technical data. The parameters to be monitored, methods of their determination and processing are dealt with. The lines of further development of Russian medical computer monitors are outlined.

Blood Pressure Monitors↗

[Development and experience in clinical use of computer monitoring systems in the cardiology intensive care unit].

To develop national monitor computer systems (MCS) for intensive cardiological care units, which should be reliable, easy-to-use, and cheaper than their foreign analogues is a pressing scientific and practical problem for Russian medical instrument making. The paper presents the results of development of and clinical experience with MCS at the intensive cardiological care unit of the Republican Clinical Cardiology Dispensary in the Republic of Udmurtia (Izhevsk).

[Scientific problems and tasks of medical monitoring].

Medical monitoring is regarded as a task of detecting and classifying human abnormalities. A computer medical monitor (CMM) is a complex hierarchical system with artificial intellect elements at the top levels of hierarchy. Each level of hierarchy performs a definite task in collecting and processing current information. Based on this approach, the authors put forward tasks of medical diagnostic and prognostic studies by developing the hierarchical network architecture of CMM.

[Computer medical monitors: state-of-the art and perspectives].

The paper shows that the currently available Russian and foreign computer medical monitors (CMMs) may generally solve the simplest tasks of data visualization, recording, and archiving. It is justified that to enhance the "intellectuality" of CMMs, it is necessary to turn to the solution of the problems and tasks of medical monitoring, by using the whole range of methods for biosignal detection and processing, which has been accumulated in modern radio engineering, in the theory of communication, and in engineering and medical cybernetics.

[Non-invasive blood pressure measuring device].

Algorithms have been developed for the Russian non-invasive blood pressure measuring monitor MPK-01 that applies a frequency excess pressure transducer. The monitor uses oscillometry. A preliminary estimate of the accuracy of blood pressure measurement for its conformity to the ANSI/AAMI (USA) and BHS (UK) standards has indicated the legitimacy of the algorithm proposed for processing if there are hardware restrictions inherent in this monitor.

[Computerization of medical technology].

The object of computerization is practical medical technology which is regarded as a complex of methodological and instrumental support for maintaining a physician-patient dialogue. The methodological support remain to be a prerogative of medicine and, consequently, relatively conservative. The instrumental support (as an aid for the methodological one) permits a constant extension of its stores. That is why the present paper considers the computerization of medical technologies in the context of extension of instrumental support. The replacement of paper technology in medicine by computerized one should primarily increase the level and quality of obtaining information on the patient and create high-levelled computerized systems: diagnostic monitoring, reference, expert ones. The creation of automatic working places for specialists of therapeutical-and-prophylactic institutions (TPI), their incorporation into the local computer systems of functional departments form an instrumental basis for informatization of the whole TPI. A scheme of the computer system of a resuscitation and intensive care is given as an example.

[Problems of medical monitoring of patients and requirements for computer monitoring systems].

Medical monitoring of patients is intended for controlling their physiological parameters, providing information to the medical staff as of critical, precritical physiological values, as well as for accumulating information on the patients' condition in time (trends). The existing monitoring systems require further improvement by expanding the types of measuring channels connected to them, providing a definite service, adaptability to a concrete medical institution. The basic orders to develop and produce such systems were got from the leading medical institutions of the Republic of Udmurt. A list of physiological parameters requiring monitoring, specifications of transducer systems and measuring channels (as a table), as well as demands by for channels, as required, records written down into the trend, as results is given. Possible variants of the design and use of computer monitors are provided.

[A unit for automatic blood cell counting].

The paper describes and characterizes a Hemoanalysis unit for automatic blood counting in the Goryaev chamber, presents an algorithm for automatic counting and results of its accuracy and rapidity performance.

[Automated workplace of the physician-endoscopist as a base for computerization of endoscopic studies].

The paper deals with the complex presentation of data for endoscopic studies on personal computers for the benefit of a physician in drawing up medical documents, establishing an accurate diagnosis, preparing a treatment regimen, as well as in facilitating his work, storing and processing data, searching for reference information, drawing up the dynamics of morbidity and its prediction. A scheme for complex presentation of the information required for endoscopic studies is given.

Diagnosis↗

[A neonatal computer monitor].

A neonatal computer monitor is described and its specifications are given. The monitor is designed to nurse terminal neonates. It exerts a control over the cardiovascular and respiratory systems in newborns with birth traumas. The monitor is intended for its use alone or in combination with a neonatal table. The later is an ingredient of a KRN-01 neonatal resuscitation unit that is manufactured by the Izhevsk Motozavod Production Association.

Electrocardiography↗

[A computer arterial pressure measuring device].

The paper describes a non-invasive oscillometric blood pressure (BP) measuring device. It provides an algorithm for the automatic measurement of systolic and diastolic BP values. A variant of assessing the BP measurement stability is proposed and the results of this assessment are given for 3 devices made by different firms. The total errors made by the studied device that functions as part of a physician's computer monitor are defined. Health institutions, resuscitation units, operating rooms for children and adults are an application sphere of the device.
