[The topicality of return to smallpox vaccination: problems and prospects].

Nowadays there are certain facts which evidence the necessity of return to smallpox vaccination in Russia, especially concerning people who will be engaged in the liquidation of the nidi of this infection, should such appear. Smallpox virus is kept in laboratories of some countries; there is a real threat of biological terrorism; population immunity to smallpox infection is virtually absent. Considering the present situation in development and production of smallpox vaccines and remedies against vaccine-related complications, the tactics of population vaccination in Russia may vary. The main problems concerning return to smallpox vaccination in Russia are the following: there are no modern schemes (schedules) of smallpox immunization; there are no currently available and safe vaccines for primary immunization of adults; there are no preparations for prevention and treatment of vaccine-related complications. Thus, the priority areas are: development of effective and safe oral adult vaccines for primary immunization; production of human and heterologous immune globulins and their storage.

Structural organization of the genome of SARS-associated coronavirus (Strain SoD) isolated on the territory of the Russian Federation.

Complete nucleotide cDNA sequence (29715 nucleotides) of SARS-associated coronavirus (strain SoD) isolated for the first time in the territory of the Russian Federation was determined. Phylogenetic analysis revealed maximum similarity between strain SoD genome and Frankfurt 1 strain genome. Three nucleotide substitutions determining two amino acid substitutions were detected.

[Intercavitary electrical stimulation of the gastrointestinal tract and mucosa with a stand-alone electric stimulator in the treatment of patients in the physicochemical stage of gallbladder disease].

The use of the SAE for the GIT and MC has a stimulating effect on the processes of bile production and secretion for patients with the physical and chemical stage of the gallstone disease. Over a short period of time the SAE for the GIT and MC normalizes the biliary tract motor functions removing the dissynergism of the biliary tract sphincter organ and restoring the sphincter motor functions. The SAE for the GIT and MC has a positive effect on major factors of bile concentration and components ratio aimed at the reduction of lithogenicity and its colloid stability improvement. The intracavitary electrical stimulation with the SAE for the GIT and MC both as a monotherapy and one of the complex anti-relapse methods is an efficient and simple method and has no side effects or complications. Therefore, it may be recommended for the treatment of patients with the physical and chemical stage of the gallstone disease.

[Pathogenetic characteristics of the development of arrhythmia in patients with ischemic heart disease].

ACTH, hydrocortisone and cyclic nucleotides levels, beta-receptors sensitivity, lipid peroxidation (LPO), antioxidant system, hemodynamics were investigated in 93 coronary patients without myocardial infarction in the presence of cardiac arrhythmia and after its correction. The clinical and laboratory findings indicate that it is primarily LPO activation and LPO products excessive accumulation in the blood that are responsible for arrhythmia emergence. Experimental data support these results by showing possibility of cardiac arrhythmia onset due to LPO products which raise the density of Na-Ca channels and inhibit the activity of Ca-dependent ATPase.

[State of the pancreas in persons with alcohol-related risk factors].

Clinico-laboratory and instrumental techniques were used to investigate the pancreas states in alcohol abusers at preclinical stage. Examined were 74 persons, 24-31 years of age, with established alcohol-related risk-factors, who would consider themselves healthy. The character and forms of alcohol addiction were determined by a psychiatrist. Complaints were studied in the group which fell into astheno-vegetative syndrome (56.2%), dyspeptic disorders (32%) and pain (14.1%). Ultrasound scanning of all the cases was performed. Pancreas tissue echostructure changes were detected in 16% cases, including fibrosis and edema in 7 and 5 cases, respectively. Hydrochloride methionine intraduodenal testing was performed to assess the pancreas external secretion indices. 98.6% cases manifested changes in external secretory function, e.i. increased concentration and secretion of amylase, decreased concentration and secretion of bicarbonates in the stable volume and rate of pancreatic juice secretion.

[Pathogenetic characteristics of cardiogenic shock in closed chest injury with cardiac contusion].

An analysis of clinico-laboratory examinations of 60 patients with a suspected contusion of the heart in an isolated and associated trauma of the chest and experimental investigations on 46 dogs have shown that the closed trauma of the chest with a contusion of the heart is accompanied by disorders of the contractive function of the myocardium and hemodynamics which contributes to the development of cardiogenic shock.

[Patho- and morphogenesis of infectious myocarditis].

An experimental study in white mice infected with influenza virus A1, staphylococcus, Pseudomonas aeruginosa, Escherichia coli or Proteus, showed toxic vascular, stromal and cardiomyocyte damage in the myocardium within the first 2-3 days (circulation disorders, edema, dystrophic and necrotic changes in cardiomyocytes), which by day 3-5 were replaced by inflammation resulting in the formation of myocarditic cardiosclerosis foci by the 3d-4th week. Certain immune disorders were detected in patients with infectious myocarditis; cardiac antigen was found in the serum of 24%, anticardiac antibodies in 22%, positive blast transformation tests in 7.5%, as well as reduced quantities of T cells. Coons's method at late dates after staphylococcal angina revealed antibodies to myocardial structures in 45.7% of patients, and those to connective tissue in 36.1-31.4%. Myocarditis signs were identified in part of those. A scheme is proposed for the pathogenesis of infectious myocarditis.
