A case of recurrent herpetic whitlow is discussed. The patient, a dental assistant at the time of her initial infection, developed herpetic whitlow caused by herpes simplex virus type 2 (HSV-2). She has subsequently experienced nine recurrent infections with lesion spread, scarring, and persistent paresthesia. The severity of the most recent recurrences appears to have diminished, and prodromal sensations, that is, increased local sensitivity, have occurred without the subsequent development of lesions.
Epithelial outgrowths from hamster cheek pouch explants were cultured for varying periods of time up to 22 days. Growth of the epithelial sheets was monitored, employing colcemid for demonstrating mitotic activity and tritiated thymidine for DNA synthesis. Mitoses and thymidine uptake were observed among epithelial outgrowths at a considerable distance form the original explant. The epithelial nature of the growing cell sheets was confirmed, employing electron microscopic techniques. The cells exhibited the presence of tonofilaments, desmosomes, ribosomes, Golgi, mitochondria, and rough endoplasmic reticulum. The cultured explants were treated with cyclic nucleotides in order to investigate their modulatory effects on epithelial cell differentiation. Dibutyryl cAMP induced marked mitotic inhibition (46.3%) in our assay, which was increased to 57% with the addition of theophylline. Dibutyryl cGMP showed only a mild (5%) stimulatory effect on mitotic activity. Dibutyryl cAMP enhanced keratinization in the epithelial cell outgrowths with the biogenesis of keratohyalin granules, whereas dibutyryl cGMP did not produce any observable alterations.
A comparison was made between the rate of iodide permeation of dentin by diffusion and by filtration. Before acid etching, filtration only doubled the rate of iodide permeation relative to that obtained by diffusion. After acid etching, filtration produced a 32-fold increase in permeation. Acid etching, by removing occluding debris, facilitates filtration more than diffusion.
This study examined the adequacy of sterilization of air-water syringe (AWS) tips following contamination during routine dental care treatment procedures. Although the internal chambers of most AWS tips were determined to be contaminated following use, no residual microbes were detected following autoclave sterilization of the AWS tips used during patient treatment procedures. Therefore, AWS tips apparently are readily heat sterilizable by conventional methods following clinical usage.
