[Influence of reducing diets on the glucose-6-phosphate dehydrogenase activity of the fatty tissue in patients with metabolic-alimentary obesity].

The glucose-6-phosphate-dehydrogenase activity by the fatty tissue in patients with alimentary adiposity was studied. A well-marked fall of the activity by comparison with controls, especially in patients with hyperlipidemia, was revealed. Following treatment of the patients with reduction diets their falling body weight was paralleled by significantly rising activity of the fatty tissue G-6-P-D. Tests in vitro failed to show pyridozine and pyridoxal-6-phosphate to affect the activity of the enzyme.

[Effect of pyridoxine on patterns of lipid metabolism in patients with alimentary obesity].

Studies of patients with alimentary-metabolic obesity, which were treated with pyridoxine and were maintained on a reduced diet, revealed a normalizing effect of the vitamin on some patterns of lipid metabolism. In patients, treated with pyridoxine, body weight, content of total lipids, cholesterol, phospholipids, glycerids and beta-lipoproteins in blood serum were decreased more distinctly as compared with patients, which were only maintained on a reduced diet. In hyperlipidaemia the positive effect of pyridoxine was more pronounced than in the cases with normal content of lipids in blood.

[Effect of a reduced diet on hyperlipoproteinemia in metabolic-alimentary obesity].

The effect of calorifically reduced diet No 8e on the lipoproteinic composition of the blood serum in patients with metabolic-alimentary adiposity was studied. Most of the patients examined demonstrated hyperlipoproteinemia, basically of the IIb type, more seldom of the IIa and IV types. Following a course of the reduced diet the frequency of hyperlipoproteinemia of the II, III and IV types declines and the content of atherogenid lipoprotein fractions in the serum goes down. In young patients the effect of a reduced diet on the lipoprotein composition of the blood serum proved most effective.

[Effect of diet therapy on the course of chronic enteritis based on immediate and late results of dispensary observation].

The therapeutic effect of diets was studied in chronic enteritis patients during 5 years of dispensary follow-up. A scheme has been proposed for dietotherapy of patients under conditions of a long-term dispensary follow-up, after a course of inpatient treatment. The results obtained have proved the effectiveness of the diets used, their corrective action both on the clinical state of the patients, and on disorders in the digestive organs and metabolism.

[Effects of diet therapy on the course of chronic pancreatitis: ambulatory follow-up data].

The data of clinical, laboratory-biochemical and immunological investigations conducted in 75 patients with chronic pancreatitis have been analyzed. The patients who received diet therapy were investigated under clinical conditions and during dispensary follow-up (6-9 months after the discharge from the clinic). The ration, sparing mechanically and chemically the digestive organs, included on an average 120 g protein, 85 g fat and 350 g carbohydrates. The main clinical symptoms of the disease diminished under the influence of the diet therapy. Improvement of the pancreas function was recorded in the absolute majority of the patients (normalization of activity of proteolytic enzymes assayed in the blood serum: trypsin, trypsin-inhibitor, amylase, lipase). Favourable changes were also observed in immunological parameters. When the patients followed the dietetic recommendations at home, the positive effect of the diet therapy was enhanced. A conclusion has been made that patients with chronic pancreatitis should be given diet therapy not only under clinical but also under dispensary conditions, thus inhibiting the disease progressing.

[Dietetic aspects of preventing urolithiasis in patients with gout and uric acid diathesis].

Patients suffering from gout and uric acid diathesis received a specialized diet containing 70 g protein (85% of plant protein including 20 g of soybean protein isolate), 85 g fat and 350 g carbohydrates. As a result there was an improvement of the general well-being during dietetic management, a rise of pH of the urine to normal, a decrease in the concentration of uric acid in blood serum and urine with a simultaneous elevation of the latter's clearance, an increase in glomerular filtration, a reduction in blood cholesterol content. The data obtained allow the conclusion to be made about the necessity of the rigid observance of the diet by the patients. This will play an important role both in the treatment and prophylaxis of urolithiasis in the patients' population under consideration.

[Excretion of catecholamines and DOPA in lipid metabolism disorders].

Excretion of catecholamines and DOPA was impaired in patients with metabolic-alimentary obesity, with ischemic disease of heart, with atherosclerosis and excessive weight. Distinct decrease in content of adrenaline, noradrenaline and DOPA was observed in patients with obesity; the phenomenon was less pronounced in ischemic disease of heart, mainly in aged patients. Correlation was found between the rate of excretion of catecholamines and DOPA and the extent of hyperlipidemia. Dietetics did not normalize completely the impairments studied. Additional administration of pyridoxine caused a favorable effect.

[Epidemiological problems of obesity].

The results of epidemiological survey of the population living in the middle zone of the Soviet Union and in the GDR are considered. Comparison of the data obtained shows that overweight is a very prevalent abnormal condition among the test population groups in both countries and amounts to 26% (USSR) and 30% (GDR). Excess animal fat consumption in the presence of a decreased quota of vegetable oil has been established and a significant increase in monosaccharides intake noted. Attention is attracted by insufficient quota of fresh vegetables and fruit in the diets of the population of both countries. Qualitative and quantitative nutritional pitfalls were shown to be aggravated in obese people. Violation of dietetic regimens and decreased physical activity play a definite role in the development of overweight. A program on primary and secondary prevention of overweight has been established. It includes a number of measures both common and specific to the GDR and the USSR.

[Ways to detect and measures to prevent obesity].

The results of an epidemiological survey indicate high incidence of obesity and overweight among adult population in a district of Moscow and in the Moscow region. Obesity and overweight were recorded in 46.3% of persons examined at a large industrial enterprise, in 35% at a food plant; 16% of persons with overweight and 26% with obesity were recorded among the unorganized population of the district in Moscow; 35.4% of sovkhoz workers were obese. The survey has shown that the main reasons for high incidence of obesity are excessive nutrition and poor physical activity. Obesity is a risk factor for the development of various diseases and carries hazards for the health of the population.

[Metabolic characteristics of lipids and catecholamines in alimentary obesity complicated by diabetes mellitus in its latent form].

Alimentary obesity aggravated by latent diabetes mellitus is marked, apart from overweight and disturbed tolerance to carbohydrates, by high incidence of hyperlipoproteinemia running its course according to IIb and IV types, as well as by normal excretion rate of catecholamines. The reduced caloric value of the diet including sharp restriction of carbohydrates produces a favorable effect on the metabolic parameters under test, particularly on the level of low-density lipoproteins.

[Effect of diet therapy on several metabolic disorders in obese patients of different ages].

Data derived from a comparative study of some lipids and water metabolism rates in patients suffering mainly from metabolic-alimentary form of adiposity in 2 age groups (from 16 to 25 and from 35 years and upwards, respectively) are offered. In patients of both groups substantial changes in the lipid fractions of the blood were noted. The proportions of glycerides, beta-lipoproteins and free fatty acids in the blood were markedly increased. At the same time, all the patients, irrespective of their age, demonstrated a fall of the glucose-6-phosphate-dehydrogenase activity of the fatty tissue along with a reduced diurnal excretion of catecholamines with DOPA. There were in evidence characteristic disorders of the water metabolism that found their expression in an increased overall amount of water and also in a higher content of extra- and intracellular fluid. A low-calories diet used in conjunction with graded therapeutic exercises helped achieve a positive dynamics in the reduction of the patients' body weight along with a tendency toward normalization of the study metabolic rates in both of these age groups.
