[Dearterialization of the liver as a treatment method in hemangiomas].

The article is devoted to the experiences with treatment of hemangiomatous lesions of the liver. Results of palliative treatment of 65 patients by methods of dearterialization of the liver (ligation and embolization of the hepatic artery) are presented. The authors make a conclusion that dearterialization of the liver is a sufficiently effective method of treatment resulting in sclerosing the hemangiomas and substitution of them with the fibrous tissue. It is not followed by the development of pronounced complications and lethality. With clinical manifestations retained after embolization of the hepatic artery it is possible to perform resection of the liver with hemangioma, the operative intervention in this case being accompanied by less blood loss, and it is easier technically than when it is fulfilled without preliminary dearterialization of the liver.

[Simultaneous operations in patients with thyroid diseases].

Twenty-four cases of simultaneous surgical treatment for coexisting diseases of the thyroid gland and other organs are discussed. Such operations were mainly performed for associated nodular goiter (including thyroid cancer) and benign or malignant tumours of other localisations. As a rule surgical interventions were planned beforehand. Only in 2 cases there were urgent operations for coexisting diffuse toxic goiter and complicated ulcer disease. In 4 cases thyroid resection and laparoscopic cholecystectomy were made simultaneously. Immediate and long-term outcomes were good. The indications and conditions of performing the operations are discussed.

Characterization of prednisone, prednisolone and their metabolites by gas chromatography-mass spectrometry.

Human urinary metabolites of the synthetic corticosteroids prednisone and prednisolone were detected in the course of gas chromatographic steroid profiling as methoxime-trimethylsilyl derivatives. Metabolites were provisionaly identified by combined gas chromatography-mass spectrometry. The major metabolites were 11-keto/11-hydroxy conversion products, 20-hydroxy and 4,5-dihydro analogues of the parent drugs. Cortisone, 6-hydroxy and fully saturated A-ring compounds were minor metabolites. Retention indices and mass spectral data are presented.

Determination of methylprednisolone metabolites in human urine by gas chromatography-mass spectrometry.

Methylprednisolone and its metabolites were studied as their methoxyamine-trimethylsilyl derivatives by means of capillary gas chromatography-mass spectrometry. The expected unchanged drug and 11-keto and 20-hydroxy metabolites were found in human urine. The typical metabolites are 6,7-dehydro analogues of the above-mentioned compounds. Characteristic gas chromatographic profiles of urine steroids were obtained. Retention indices and m/z values are presented for methylprednisolone and its main metabolites.

Analysis of anabolic steroids in body fluids by capillary gas chromatography with a two-channel detection system and a computer.

A method is described for analysis of multi-component mixtures of steroid metabolites in biological fluids by means of capillary gas chromatography with glass and fused-silica columns and simultaneous detection of methoxylamine-trimethylsilyl derivatives with universal flame-ionization and selective nitrogen alkali flameionization detectors. A data system was applied to the on-line treatment of the results. Computer programs were designed for precise calculation of Kováts retention indices from the known values for selected natural urinary steroids. The programs allow the selection of nitrogen-containing components, normalized chromatogram plotting for both detection channels and qualitative and quantitative analysis. Results are presented on the detection of metabolites of methandrostenolone, 17 alpha-methyltestosterone, 19-nortestosterone and fluoxymesterone.
