Serum immunoreactive elastase: is it useful for the diagnosis of pancreatic cancer?

We have measured serum immunoreactive pancreatic elastase 1 concentrations in 90 patients with pancreatic cancer in order to determine its usefulness in the diagnosis of this tumor. Abnormal elastase 1 concentrations were found in only 58 (64.4%) of the 90 patients. Fifty (55.5%) had abnormally high values, and eight (8.9%) had abnormally low values. No significant differences in elastase 1 levels were observed between patients with resectable cancer (n = 15) and those with unresectable cancer (n = 75). Moreover, no significant differences were found between elastase 1 concentrations of patients with pancreatic cancer and those of 71 patients with chronic pancreatitis. We conclude that serum elastase 1 measurement does not represent a significant advance in the diagnosis, whether early or late, of pancreatic cancer.

Structural and chemical characterization of gallstones resistant to dissolution therapy.

X-ray diffraction, i.r. spectroscopic, and chemical analyses have been carried out on radiolucent gallstones resistant to dissolution therapy. Cholesterol represents the main component of all the examined stones, while the ratio between the amounts of pigmented material and calcium carbonate is about 1 in the inner and outer layers of the stones and 3 in the medial layer. Calcium carbonate is present in two distinct crystalline forms: vaterite, which is the main inorganic crystalline phase, and calcite. The cell parameters of vaterite and calcite are shorter in the inner and outer layers of the stones than in the medial layer. The observed variation of the cell parameters has been related to the substitution of copper to calcium in the carbonate structures, on the basis of the data obtained on vaterite and calcite synthesized in presence of different copper concentrations in solution. The results indicate that the failure of the dissolution therapy can be related to the inhomogeneous distribution in the stones of calcium carbonate and calcium bilirubinate.

[Medical therapy of gastroesophageal reflux. Evaluation of the activity of clebopride by continuous intraluminal pH measurement].

Seven female and three male outpatients (mean age 45, range 37-54), suffering from gastroesophageal reflux underwent therapy with clebopride, a new selective antidopaminergic agent. Before and after treatment (1 mg b.i.d. for ten days) 24 h-continuous monitoring of esophageal pH was done. Clebopride significantly lowered the number and the extension of gastroesophageal acid refluxes.

[Diagnostic use of the mathematical model of nonlinear regression in the interpretation of the crystalline bilirubin loading test in Gilbert's syndrome].

The results of a study conducted both on subjects affected by constitutional hyperbilirubinaemia (Gilbert's syndrome) and healthy subjects using the charge test with crystalline bilirubin are presented. The parameters which are involved in bilirubin kinetics were established by means of non linear regression methods (the Marquardt method). 19 Gilbert syndrome patients and 12 normal control subjects were studied. Important parameters readily applicable to clinical and diagnostic evaluation of subjects suffering from familial cholemia were identified.

[Study of intestinal transit using a radioisotope method].

Intestinal transit patterns were studied in 20 normal volunteers. It was found that small changes in transit time occurred in the passage from the caecum to the sigmoid and rectum, and that most subjects evacuated the capsule 38 hr after its ingestion.

[Changes in intestinal transit time induced by bromopride in functional diseases of the colon].

Intestinal transit time was studied in two groups of 8 patients with irritable colon. In the first group, the time was longer than normal, whereas in the second it was accelerated. Three subjects in each group received a placebo, while the other five received 60 mg/day bromopride for 15 days. No change in transit time was noted in the controls. Two subjects in Group I displayed a significant reduction in transit time after bromopride, while deceleration and normalisation were observed in 4/5 patients in Group II.

Cardiomechanical effect of k-strophanthoside administered to healthy volunteers by the rectal route.

Eight healthy volunteers were given k-strophanthoside (Strofopan) 1 mg rectally in order to evaluate changes in the cardiac function. Cardiomechanical measurements were performed by the polygraphic method before administering the drug and 1, 2, 3, 4, 5 and 6 h later. Three of the eight subjects continued to receive k-strophanthoside 1 mg daily for a further 6 days. Similar evaluations were made at the same times on day 3 and on day 7 of treatment. All indices considered varied to a statistically significant extent and showed the maximum effect at about the 4th h. The most evident changes were observed in the ICT and the PEP/LVET ratio. Here, the maximum variations were 46.3% and 29.5%, respectively, as compared with baseline values (P less than 0.01). When treatment was given for 7 days, the baseline values measured on day 3 and day 7 before administration showed a moderate and constant decrease in all parameters as compared with the first baseline value. This proves that k-strophanthoside administered rectally possesses steady-state activity with time. From a comparison of the changes in the same parameters as observed in a previous investigation after administering digoxin 0.50 mg orally there is reason to conclude that the effects obtained after administering k-strophanthoside 1 mg rectally were of the same magnitude.
