[The effect of tocopherol and nicotinic acid on the microcirculation and blood coagulability in patients with ischemic heart disease].

Patients with ischemic heart disease were treated by routine techniques (53), using also tocopherol acetate (54) and combination of tocopherol with nicotinic acid (53). Changes of the microcirculatory bed of the bulbar conjunctiva was studied biomicroscopically and in the process of blood coagulation hemoelectrocoagulographically. Complex treatment using tocopherol acetate produced a positive effect on the coagulation properties of the blood and did essentially influence the fibrinolytic activity and microcirculation. Tocopherol plus nicotinic acid resulted in normalization of the blood coagulation process, favoured activation of fibrinolysis and improvement of the microcirculatory bed.

[Nicotinic acid in the treatment of chronic circulatory failure in patients with ischemic heart disease].

Examined were 98 patients with chronic forms of ischemic heart disease, mainly atherosclerotic cardiosclerosis with different degrees of cardiac insufficiency. Two groups were distinguished. Patients of the first group received traditional treatment while patients of the second group received also nicotinic acid agents. It was found that inclusion of nicotinic acid in the complex treatment of patients with ischemic heart disease increased the therapeutic efficacy.

[The importance of microbiological and immunologic studies for selecting the method of treating patients with acute pneumonia].

Quantitative microbiological examinations of the sputum allowed to establish the role of pneumococci and different bacterial associations in the development of acute pneumonia. The most typical were associations of Str. pneumoniae, staphylococci and neisseriae. The sensitivity of isolated pathogens is shown and recommendations on the use of most effective antibiotics are given. The indications to immunocorrective agents are outlined.

[Importance of associated microorganisms in the development of acute pneumonia].

Sputum samples obtained from 106 patients with acute pneumonia have been studied by the quantitative microbiological method. Different microbial associations have been shown to play an important role in the development of acute pneumonia (67.9%). Microbiological studies have revealed the prevalence of pneumococci in the etiological structure of the disease. Staphylococci, hemophilic bacteria and Neisseria have been found to take part in the development of acute pneumonia in a lesser number of cases and more often occur in combination with pneumococci or form different associations themselves. The so-called "etiological" type of microbial associations has been determined, two or three microbial species being isolated at high concentrations (10(6) and over) and the percentage of patients with such combinations being 59.7 +/- 5.8%. The analysis of the clinical course of the disease has revealed that the presence of different microbial species in patients linked, to a great extent, with the clinical picture of the disease. These data may be used for prescribing adequate etiotropic therapy and for prognostication. Quantitative bacteriological studies carried out in the dynamics of the disease permit timely detection of changes in the microflora of the respiratory tracts, the evaluation of the effectiveness of antibacterial therapy in the process of its implementation and the rational correction of therapeutic measures.

[Sputum microflora in acute pneumonia based on the data from a quantitative study].

In this work the data obtained in the quantitative investigations of sputum samples from 106 miners having acute pneumonia are presented. These investigations were carried out twice at the peak of the disease to determine the possible infective agent. The virological study of nasal impression smears by immunofluorescence and the serological study of paired sera made it possible to establish the viral and bacterial nature of the disease in 12% of cases. The expediency of the quantitative investigations of sputum, carried out twice, in combination with the study of the biological properties of opportunistic microorganisms was shown. Streptococcus pneumoniae proved to play the most important etiological role in the appearance of acute pneumonia in miners. This infective agent was detected in 82% of patients by the inoculation of sputum samples in "diagnostic" dilutions (10(-5) and higher). The associations of pneumococci with staphylococci, hemolytic bacteria and Neisseria were found to be capable of playing a significant role in the development of acute inflammation in pulmonary tissue, especially in those cases when these associations were isolated from highly diluted sputum (10(-5)).

[A case of Gierke's disease].

A rare clinical case is reported of a patient presenting with glucose-6-phosphatase deficiency (Gierke's disease). Shown in the advanced stage of the disease is the polyorgan involvement. The report contains a brief review of relevant medical literature concerning etiology, pathogenesis, clinical picture, diagnosis of von Gierke's disease.

[Recurrent thromboembolism of the pulmonary artery in a female patient with a gallbladder tumor].

A clinical case is submitted of development in a female patient of recurring thromboembolism of small branches of the pulmonary arteries preceding the clinical manifestations of a disease. Subsequently the above female patient was diagnosed as having developed carcinoma of gallbladder. An item of information is given concerning the syndrome of hypercoagulation and development of thromboembolism in malignant tumors.

[Lipid peroxidation abnormalities in miners with Reiter's disease and gout].

The paper presents results of investigations designed to study changes in lipid peroxidation and in antioxidant system of miners in different vocational groups with Reiter's disease and gout. It is shown that the condition of the above systems is affected by both diseases as such and occupational conditions of miners. Based on the secured findings it is recommended that treatment of the above patients be supplemented with an antioxidant agent, copherol in particular.

[Ten-year experience of the postgraduate education for general practitioners].

The article summarizes a 10-year experience of teaching general practitioners in internship. Different kind of teaching in the 1-year term was described as well as given questionnaire design results of internists to be. Intership organization development was considered in the article, it will help the work of general practitioners to be more effective.
