[Conditions of work and hearing in traffic policemen in Moscow].

Conditions of work, sound pressure levels on the city roads, direct and indirect hearing loss risk factors and hearing were studied in 200 traffic policemen working on the roads and 50 policemen working in offices. It was found that working conditions correlate with hearing loss in traffic policemen, so hearing loss in the policemen working on the roads is occupational and requires adequate prophylactic and therapeutic management.

[Otological effects of the impulse noise].

Literature data are reviewed on otoacoustic emission which is not widely known in Russia as a method of assessing acoustic function. Normal parameters of otoacoustic emission obtained in subjects with normal hearing are provided.

[The decrease of risk of occupational diseases among rail road workers: future strategies].

Diagnosis, registration and prophylaxis of occupational diseases among railway workers remain unsatisfactory so far. Thus, creation of the Railway Register of Occupational Diseases is essential. The Register includes several blocks related to each other and covering enterprises, medical and social institutions. Given relatively low primary occupational morbidity in railway transport, the Register's software considers incidence of occupational diseases in Russian Federation among analogous occupations, matched in working environment classes, length of service and occupational hazard. The authors obtained the matched incidences of occupational diseases, and can extrapolate the relationships revealed to railway workers, forecast high risk of occupational diseases among these workers.

[The development of occupational hearing loss under the influence of pulsed noise].

The deafness was studied in workers engaged into the weaving and the blacksmithing. Both groups of workers were exposed to the noise dose of 109 dB estimated according to Robinson's formula. Processing of received data in diminished hearing revealed the reliable difference between the studies groups both in quantity of the cochlear neuritis and the grade of the hearing failure. Slight hearing failure predominated in the weavers' group whereas the moderate and severe forms appeared in the blacksmiths. Analysis revealed the positive correlation between the overall noise dose and diminished hearing in all studied groups concerning high and voice frequencies. The impulsive noise appeared to be more aggressive than the constant one.

[Diseases of the upper respiratory tract in workers in "dusty" jobs].

Diseases of the upper respiratory tract are wide spread in the workers exposed to dust and closely correlate with the length of service. Pathologic process grows severe and disseminates the upper respiratory tract according to prolongation of the exposure to dust. The main pathologic mechanisms of process in the upper respiratory tract mucosa are: disorder of mucociliary clearance, disturbance of the regional immunity and the acid-base balance of tissues. Criteria of occupational connection of the nose, throat and larynx dystrophic diseases in workers exposed to dust are given.

[Actual problems of allergy in occupational pathology].

Data from different sources concerning the modern status of the allergy problem in the occupational pathology are represented. Features of immunologic pathogenesis in occupational respiratory tract diseases formation due to the simple and composite substances, protein denaturants, immune modulators and histamine-liberators organic and non-organic by origin are described. The most promising directions for the further investigation of the problem are stated and based.

[Impulse acoustic and mechanical processes and their medico-biological evaluation].

Impulse mechanical and acoustic processes whose action on the body of man may result in unfavourable consequences are ever widely practiced today in industry and in the environment. Impulse processes are characterized by certain physical peculiarities which determine the specificity of their biological effect. In turn, this gives rise to the problems of their hygienic estimation and standardization. At the same time out of the diversity of physical parameters which characterize impulse noise and vibration, one should choose, for hygienic estimation and standardization, only those of them that are responsible for the biological effect to the greatest degree. Different authors suggest different approaches to the hygienic standardization of those processes, which depends on what parameters are recognized by them as more adequate to their biological effect.

[Screening-diagnosis of occupational hearing disorders].

Hypoacusis is of leading prevalence in the structure of occupational morbidity in the USSR. Preventive medical examinations are considered a major prophylaxis measure which can be further improved with automatized screening diagnostic systems. Supplementary devices to the "ASKORS" automatized system are proposed to diagnose the pronosological forms of noise-induced health disorders.

[Occupational disease centers--an important element in the improvement of health services for the workers].

Occupational morbidity is tending to increase, but the official statistics still underestimates occupational pathology cases, which can be explained by inadequacy of preventive medical examinations, poor training of physicians in occupational medicine, as well as the underdeveloped technical basis of health services and the unavailability of coordinating and consulting centres for providing guidance in occupational diseases' prevention. The establishment of such centres would improve the scope of preventive activities most significantly.

[Actual problems of the treatment of the main forms of occupational diseases].

The article presents data on the treatment of dust induced diseases of the lungs, occupational bronchial pathology, lead poisoning, allergic skin diseases, occupational hypoacusis and occupational NS disorders. The treatment techniques included relatively new therapies of which non-medicinal techniques were preferred, such as needle therapy, entherosorption, phytotherapy, manual therapy, vacuum massage, etc.

[Clinical picture, diagnosis, and prevention of occupational allergic diseases].

The paper is concerned with a clinical picture of various types of occupational allergic diseases caused by exposure to occupational allergens. In view of a clinical course and pathogenesis of these diseases a set of diagnostic methods was developed, including allergy-oriented examination of patients using skin, endonasal and inhalation tests. Early diagnosis of allergic diseases and interruption of the workers' contact with occupational substances of allergic and irritant action are recommended for the prevention of severe allergic lesions of the respiratory organs and skin. The use of special protective ointments and pastes is indicated in skin diseases.
