[Cardiac protection as priority strategy of cardiovascular prevention in high blood pressure].

Cardiac complications are the main cause of morbidity and mortality related with HBP in our setting and entail first magnitude human and social-health care consequences. Although the incidence of ACVA has decreased in recent decades, this has not occurred with the same intensity for cardiac complications, probably due to their multifactorial origin. Left ventricular hypertrophy, main etiologic responsible factor of hypertensive heart disease that includes heart failure, ischemic heart disease, arrhythmias and sudden death are found as nuclear element of the cardiac disease. Consequently, the magnitude, etiological diversity, vulnerability and social-health care implications grant the intervention on cardiac protection a priority role in the management of HBP and its complications.

[ARB II in chronic heart failure. Coincidences and divergences. Class effect?].

Although it is possible to suggest that the ACE inhibitor may have a certain class effect in regards to the benefits observed in the treatment of chronic heart failure, it does not seem reasonable that the same can be assumed in the case of ARB II. Morbidity-mortality studies in chronic heart failure have only been conducted with three of the seven ARB II presently commercialized in our country. These are losartan (ELITE II), valsartan (Val-HeFT) and candesartan (CHARM). The three studies have demonstrated the utility of these drugs, although with different nuances. In these clinical comments, we review the evidence available, stressing what effects could be common to all the ARB II, such as the prevention of diabetes or atrial fibrillation and what effects may only be attributable to some of these, at least up to now. However, new information of the already finished studies and some still on-going clinical trials may help us to know the effects of the ARB II in this disease more and better.

[Utility of perindopril in mild-moderate heart failure in daily clinical practice. METRICA trial].

INTRODUCTION AND OBJECTIVES: Angiotensin II converting enzyme inhibitors have an unquestionable utility in heart failure, although these drugs are underused and rarely used at the recommended doses in the clinical practice. The objective of this investigation was to study the simplicity of perindopril use in the treatment of heart failure and to confirm its clinical and radiological efficacy. MATERIAL AND METHODS: Perindopril was orally administered at 2-4 mg/day doses to 180 patients with heart failure and functional class I-III who were not being treated with angiotensin II converting enzyme inhibitors. The percentage of patients who reached the target dose (4 mg/day) was studied as well as clinical (functional class of the NYHA) and radiological efficacy of this treatment in these patients. RESULTS: The target dose (4 mg/day) in heart failure was reached in 87.5% patients, with a rate of adverse effects of 18.9%. Eighty-nine percent of patients in the functional class III and 38.3% of patients in class II had clinical improvement. The cardio-thoracic index decreased from 0.57 to 0.54 (p < 0.05). A minimal irrelevant change was observed in the analytical levels of creatinine and potassium. Only three patients (1.7%) abandoned the trial early in its course. CONCLUSIONS: An efficient dose for heart failure is simply and safely obtained with perindopril, with a significant improvement in the NYHA functional class and in the radiological cardio-thoracic index.

[Hypertension and heart failure in Primary Care and Cardiology consultations of in Spain].

OBJECTIVE: Analyze the results concerning the heart failure population of CARDIOTENS 99, a cross-sectional study of hypertension associated with cardiovascular disease carried out on 32,051 patients seen in Cardiology and Primary Care consultations. METHODS: In the analysis we included prospectively demographic, clinical, blood pressure, and therapeutic data of all patients seen on a single day by 1,159 physicians (21% cardiologists; 79% Primary Care physicians). RESULTS: 4% of the total sample of 32,051 patient had heart failure, representing 23% of the patients with heart disease. Median age was 70,8 6,7 years; 53% were women. Hypertension was the most frequent risk factor, appearing in 71% of the patients with heart failure. Hypertension was most frequent in Primary Care patients (76% versus 63%; p < 0,01), while ischemic heart disease was most frequent in cardiology patients (38% versus 27%; p < 0,01). Cardiology patients showed a trend for greater use of drugs in comparison with patients seen in primary care consultations but without significant differences (3,39 versus 3,35; p > 0,05). Diuretics were the drugs with greater prescription (63%). 44% patients with heart failure were on an angiotensin-conversing enzyme inhibitor and 14% on beta-adrenergic blockers. Systolic blood pressure was well controlled in only 19% of hypertensive patients with heart failure, according to the international recommendations (< 130 mmHg). The proportion of patients with blood pressure higher than 140 mmHg was superior among the patients seen by cardiologists than between those seen by Primary Care physicians (57% versus 47%; p < 0.01). CONCLUSIONS: Heart failure is often associated with hypertension. Control of blood pressure is inadequate in the majority of patients with heart failure. Ischemic heart disease is a more frequent cause of in the patients seen by cardiologists than in those seen by Primary Care physicians, and hypertension is the leading cause of heart failure in these patients. Prescription of drugs recommended for the treatment of heart failure it is generally very scant.

[Spanish Society of Cardiology practice guidelines on arterial hypertension].

High blood pressure is a well-known cardiovascular risk factor that is responsible for an elevated morbidity and mortality. However, although efficacious drugs for treatment and numerous and updated scientific training programs are available, the reality is that only a low percentage of patients are followed up in accordance with the rates which are presently considered normal. The purpose of these guidelines is to provide medical guidance for the prevention, detection and evaluation of hypertension, and to provide the best diagnosis and treatment. The factors involved in cardiovascular complications in the hypertensive patient are multiple. That is why this report places more emphasis in the individual cardiovascular risk stratification as part of the treatment strategy. The information obtained in the most recent studies published confirms the interest in achieving the greatest decrease in rates of blood pressure. This treatment to lower levels is especially useful in the high-risk subgroup. It maintains the necessity of nonpharmacological measures or lifestyle modifications in all patients with high blood pressure who either need or do not need drug therapy. All pharmacological groups may be used, but it is appropriate to choose the specific antihypertensive agent adapted to the clinical and individual situation with the use of low doses of drugs to initiate therapy and the use of appropriate drug combinations.

[Current status of arterial hypertension as a cardiology topic: point of view of the cardiologist. Task force on Arterial Hypertension of the Spanish Cardiology Society].

Even if arterial hypertension (HT) is a process of multiorganic involvement, it is nevertheless in practice, one of the most powerful factors for the development of cardiovascular complications that increases very seriously, both cardiovascular morbidity and mortality. Paradoxically, in Spain, a significant proportion of cardiologists have neglected for many years this disease, at least, at its initial phase, acting mainly, only when the usual and severe cardiovascular complications of HT were clinically well established. Currently, this attitude is changing in such a way, that in the near future, the cardiologist's commitment to HT, will be definitely engaged in both; basic research and clinical aspects. The Spanish Hypertension Working Group, has worked out in an inquiry sent to all Spanish cardiologists, in order to know their point of view in relation to the main aspects involving HT and cardiological practice. This paper collects the results of the answers given anonymously by the doctors.

[Guide-wire rupture as a complication of coronary angioplasty. Apropos 2 cases and a review of the literature].

Percutaneous transluminal coronary angioplasty is a useful therapeutic tool in the treatment of ischemic heart disease. Nowadays this procedure presents acute complications of different severity in 10% of the cases. The rupture of the guide wire is a very uncommon complication that may produce important consequences as occlusion of the artery of systemic embolism. The management of this event may be interventional or conservative, depending on the clinical situation of the patient and the position of the guide wire inside the vessel. We report the 2 cases of rupture of the guide wire observed in our center in 1,000 consecutive procedures; in one case it was decided to leave the fragment in the distal portion of the artery, and in the other case the fragment was extracted surgically. We review the literature about this rare complication.

[Rupture of the intraventricular septum and free wall of the left ventricle secondary to non-penetrating thoracic injury. Doppler echocardiography diagnosis].

The heart is often injured in the setting of blunt chest trauma with a broad spectrum of cardiac lesions. We present a twenty-nine years old man with interventricular septal and rupture and left ventricular free wall rupture following chest trauma. Doppler echocardiography was essential in the diagnostic and therapeutic procedure. We concluded that Doppler-echocardiography must be performed in all patients with suspicion of cardiac affectation after blunt chest trauma.

[Pseudotumoral bronchial tuberculosis].

We analyse our own experience of a rare tuberculosis presentation that we call pseudotumoral bronchial tuberculosis. 9 cases were studied, which due to radiological, endoscopic and clinical features, a diagnosis of bronchogenic carcinoma was made. The median age was 63.7 years, 7 were male and 2 were female. The hilar or mediastinic radiological pattern was frequent. The bacterial diagnosis was made through the study of the samples obtained by bronchoscopy, except in 2 cases which were diagnosed through post-bronchoscopy sputum examination. The bronchial biopsy showed the typical necrotic granulomas of tuberculosis in only one case. Supported by the bad evolution of our patients treated without steroids, the possible efficacy of early use of steroid is suggested to avoid the most important sequelae, bronchial stenosis.

[Anomalous origin of the left coronary artery from the right sinus of Valsalva with retro-aortic path].

We present the case of a patient with a dilated cardiomyopathy in which an anomaly in the origin of the left coronary artery, that arose from the right sinus of Valsalva through an own ostium and followed a retro aortic course, was recognized as an angiographic finding. The possible pathways the anomalous artery may follow, and their different clinical significances are reviewed. The angiographic sign of the aortic root, described for the identification of the anomalous origin of circumflex artery, is reviewed and its utility in the case of anomalous left coronary artery is demonstrated. In our case, Thallium-201 didn't show myocardial ischemia with the exercise and that justified, beside the doubtful efficacy of surgery as prophylaxis of sudden death when the anomalous course is retro-aortic, to maintain a conservative attitude.

[Bacteriuria in primary biliary cirrhosis].

The high incidence of significant bacteriuria observed in 30 patients diagnosed of primary biliary cirrhosis in our department is studied. Although a significant increase in general infections has not been described in this type of liver disease, of probable autoimmune nature, the high incidence of bacteriuria in these patients has been previously reported. Possible predisposing factors are evaluated; we observed that there was a significant increase in the frequency of bacteriuria with age, but there was no relationship between the incidence of infection and the histologic stage of the disease.

[Traumatic rupture of the aorta presenting as aortic coarctation syndrome].

Aortic rupture is a frequent cause of death in chest trauma. The coincidence with other associated severe lesions and the absence of clear cardiographic signs may induce to an important delay in the diagnosis. We report a case in which the late apparition of arterial hypertension expressed as an acute aortic coarctation syndrome pointed out the diagnosis. We review the pathogenic mechanisms of arterial hypertension in traumatic aortic rupture and the utility of different diagnostic procedures.
