Minimal cardiac transit times--a sensitive parameter reflecting changes in cardiac volume-to-flow ratios.

Minimal cardiac transit times (MTTs) are differences between times of appearance of the indicator at consecutive segments of the cardiopulmonary circulation. The MTTs are shown to be directly proportional to the quotient of volume to flow. The relationship of the cardiac MTTs to ventricular ejection fraction was derived. Utilizing this technique the ejection fractions of the left and right ventricles in 72 normal individuals were calculated. Theoretical aspects of measuring cardiac MTTs were related to experience of clinical application and confirmed its usefulness.

[Diverticulosis. Anatomical aspects].

Sigmoid diverticula are pseudodiverticula, protrusions of the mucosa between the muscularis, so that they pouch out ball shaped under the serosa or in the fatty connective tissue of the sigmoid region. They occur in pairs, often symmetrically along the taenias. They increase with age; as their evidence depends on the actual contraction of the bowel, the incidence varied strongly with the method of investigation. The pseudo-diverticula can cause diverticulitis. The anatomical characteristics of the large bowel are the fortified longitudinal muscular bands, the taenias, and the haustras. By a contraction and a permanent shortening of the taenias the normally "open" haustras become closed pressure chambers. The muscular coat and the mucosal coat differ very much in size, that the mucosa folds out to the outside. Of decisive importance is, that the muscular and mucosal coat are both fixed to each other by a sliding layer. The modern popular etiological theories can be fairly summarized under the point of view of the longitudinal shortening and the sliding layer.

Coronary collateral vessels: their significance for left ventricular histologic structure.

Whether coronary collateral vessels protect the left ventricular myocardium is unknown. Light microscopic morphometry was carried out on myocardial tissue samples from 56 surgically treated patients with coronary artery disease. Transmural biopsy of the myocardium perfused by the left anterior descending coronary artery was obtained during open heart surgery. In initial reproducibility studies of biopsy samples of 17 patients a sampling error for evaluation of myocardium was defined and differences in transmural fibrosis exceeding +/- 6.2 percent were considered biologically significant. Stenosis of the left anterior descending artery was determined from preoperative angiography. Group A (control group) comprised patients with less than 75 percent area reduction of the left anterior descending coronary artery (mean +/- standard deviation 65 +/- 10 percent). Patients in group B (more than 95 percent area reduction [mean 99 +/- 2 percent] without collateral supply on arteriography) were compared with patients in group C (identical stenosis [mean 99 +/- 2 percent] but with collateral supply). Fibrosis averaged 17 percent in group A, 68 percent in group B (p less than 0.001 versus group A) and 29 percent in group C (p greater than 0.05 versus group A, p less than 0.001 versus group B). Thus, in severe coronary stenosis myocardium supplied by collateral vessels shows less fibrosis on biopsy sample than does myocardium without collateral supply.

Epithelial dysplasias in chronic pancreatitis.

The present 280 specimens of chronic pancreatitis were examined to determine the type and frequency of epithelial dysplasia of the duct system. The epitheilal dysplasias were divided into 3 degrees of severity according to cytological and histological criteria. Dysplasia was demonstrable in 40.1% of the 280 specimens. Of these 32.9% were classified as dysplasia grade I and 7.1% as dysplasia grade II. Dysplasia grade III did not occur. The epithelial proliferations were correlated with the topography, the stage of the scarring and the degree of obstruction of pancreatic secretion. An increase in dysplasia was evident in relation to the stage of the scarring and to the obstruction of secretory outflow. In correlation with the topography of the chronic pancreatitis there was the highest frequency of epithelial dysplasias in uniformly scarred glands (47.7%). Papillary and pseudopapillary hyperplasias with atypia were demonstrated in 17.9 vs. 4.5% of the cases with epithelial proliferations.

[Studies of cardiac metabolism with 123I-labelled fatty acids and 11C-methylglucose (author's transl)].

To characterize externally detectable changes in the myocardial metabolism of free fatty acids (FFA) and glucose, which are associated with ischemia and cardiomyopathy, omega-123I-heptadecanoic acid (stearic acid analogue), 75Br-phenylpentadecanoic acid, and 3-0-11C-methyl-D-glucose were used as indicators. It could be demonstrated that in the metabolism of free fatty acids at least two different patho-physiological situations may exist. Disturbances in the mechanism of the accumulation of free fatty acids lead to a decrease of the amount of the free fatty acids which are available for energy production (these disturbances can be recognized as indicator accumulation defects). Disturbances associated with the mechanism of free fatty acid catabolism lead to a decrease of the ability of the myocardial cell to utilize the free fatty acids (these disturbances can be recognized as changes in indicator elimination rates). Whereas in ischaemic heart disease, the areas with altered FFA accumulation correlate with the areas of altered FFA-elimination, no correlation was found in the case of cardiomyopathy. The 11C-methylglucose seems to be an excellent indicator for the in-vivo assessment of the function of transport system in the myocardial cell membrane.

[Does dionine (aethylmorphin) influence the lymph drainage of the eye? (author's transl)].

Drainage of a lymphotropic solution of Tc-99m sulfur colloid from the vitreous body was studied in twelve rabbits. Shortly after the application of this tracer, six of these rabbits had an injection of 0.06 ml, 0.5% dionine (Aethylmorphin) solution into the retrobulbar space. After the dionine injection, the decay of radio-activity from the eyes was significantly increased. The radio-activity in the control group approximated to 81.6% of the total applied radio-activity after six hours, whereas the radio-activity in the eyes of the dionine treated group was, on average, only 5% after the same time interval. Simultaneously, the radio-activity in the cervical lymph nodes increased significantly from approximately 0.10% in the control group to 0.67% in the dionine treated group. It is not known, as yet, whether dionine increases lymph drainage in man.

Invariants in configurational prey selection by Bufo bufo (L.).

Toads (Bufo bufo) are able to distinguish small stripes with axis orientation in the direction of motion (worm-like stripes) from those with axis orientation perpendicular to the direction of motion (anti-worm-like stripes). The configurational discrimination ability of the toad is, thus far investigated, largely invariant for (1) movement direction in the x-y plane, (2) direction of contrast--stripe against background--and (3) velocity of stripe motion.

[Experimental investigation with 198Au colloid as to lymph communications between the retrobulbar space and the thyroid gland (author's transl)].

The problem of a direct connection of lymph drainage between the retrobulbar space and the thyroid gland was examined using the lymphotropic radioactive tracer 198Au colloid. Twelve rabbits were examined. Six animals were injected with 50 muCi 198Au colloid into the right thyroid lobe and six animals into the right retrobulbar space. Distribution and total body radioactivity was measured in vivo using a scintillation camera with a computer. After 6 hrs tissue specimens of different organs were measured in a well-counter. The discharge of activity from the thyroid after 6 hrs was significantly higher than that from the retrobulbar space (63% versus 33.5%). This was probably due to a higher vascularisation and to a higher degree of lymph drainage of the thyroid. The lymph drainage of the thyroid gland as well as of the retrobulbar space occurred mainly via the lymphonodi cerv. proff. No significant activity was found in the retrobulbar space after thyroid injection or in the thyroid after retrobulbar injection. It may be concluded from these results that there are no direct lymph connections between the retrobulbar space and the thyroid gland. However, indirect communications via the deep cervical lymph nodes cannot be excluded in the event of reverse lymph flow.
