The morphology of early trophoblast after dioxin poisoning in the Seveso area.

The placental morphology in 22 cases of induced abortion performed one to three months after the Seveso disaster is described. In 15 cases no gross histological or ultrastructural alterations were observed. In seven cases there were many prominent microprecipitates beneath the trophoblastic basement membrane at various distances from the fetal capillaries. Intermediate patterns between simple dense bodies, already well known, and complex pluri-stratified giant structures not previously reported have been described. Combined ultrastructural and x-ray investigations (EDAX analysis) have recorded calcium- and iron-related emissions from these structures. The significance of these findings and their possible relationship to dioxin poisoning has been discussed.

Skeletal alkaline phosphatase as a serum marker of bone metastases in the follow-up of patients with breast cancer.

Immunoradiometric determination of the bone isoenzyme of alkaline phosphatase with a method provided by Hybritech Inc., San Diego CA (USA) was carried out in 145 female patients, 97 of whom with radically operated breast cancer and 48 with benign mammary cysts, in order to evaluate the correlation of serum levels with the metabolic process of bone rearrangement in patients with bone metastases. This study shows that skeletal ALP, having high specificity (86.48%) and sensitivity (78.6%) for early progression (the average anticipation time compared to scintigraphic detection was 101 days) could represent a valid marker for bone metastases in association with mucinous markers in the follow-up of patients operated for breast cancer. In addition, dynamic serum determination of skeletal ALP could be a valid help in monitoring the efficacy of therapy in patients with bone progression.

[Chromo-colposcopic images during oral contraception].

The Authors report chromocolposcopic pictures with AZEA (Azure 2 + alcohol-soluble eosine + ethanol) and related histologic pictures in six women under oral contraception. Columnar epithelium develops violet or blue-violet areas histologically corresponding to endocervicitis. Aceto-white epithelium develops reddish color in case of medium-mature metaplasia, brown-violet in case of mature metaplasia, lilac in case of superficial koilocytosis on metaplasia. Native epithelium appears dark brown, or yellow-brown in case of koilocytosis.

[Relation between laboratory tests and densitometry values: characterization of the baseline profile in a perimenopausal population at anamnestic risk for osteoporosis].

The relationship between usual laboratory indexes of bone metabolism and dual photon densitometry in perimenopausal women at anamnestic risk for osteoporosis are studied. One hundred and twenty five women, with one or more risk factors for osteoporosis, have been investigated by: plasma calcium, phosphorus, osteocalcin, ALP-bone-isoenzyme, PTH, and 24 hours urinary calcium and hydroxyproline, besides dual photon densitometry of lumbar spine. Laboratory indexes are indicated as "above normal values" and, within the range of normality, "at lower limits", "average values", and "at upper limits". More than 90% of patients show laboratory findings at the upper limits. Furthermore, 46% of them show both hydroxyproline greater than 20 mg/24 h and urinary calcium greater than 200 mg/24 h; 81.7% osteocalcin greater than 7.00 ng/ml and plasma calcium greater than 4.9 mEq/l; 87.9% osteocalcin greater than 7.00 ng/ml and urinary calcium greater than 200 mg/244; 86.4% densitometric values less than 0.700 g/cm2 and osteocalcin greater than 7.00 ng/ml. Our at risk population shows a wide agreement among the laboratory indexes, to indicate an increased bone metabolism.

[Florid vulvar condylomatosis. Comparison of therapeutic schedules].

The problem of warts' therapy is of great interest for the diffusion of this pathology, for its high rate of recurrence and for its relation to CIN. So we evaluated the efficacy of different therapeutic methods for vulvar condylomata, used in 254 patients treated in our center from January '86 to July '90. 55 women underwent hot cautery, 31 laser-therapy, 42 excisional biopsy, 62 were treated with alpha interferon, 11 with beta interferon, 13 with thymopentin in association with electro-cautery or laser and in 40 cases no treatment was performed. The follow up, from 6 to 36 months, showed a remission of 88.1% in excisional biopsy, of 77.4% in laser-therapy, of 70.9% in electro-cautery, of 45.4% in beta interferon, of 45.2% in alpha interferon, of 76.9% in thymopentin and of 65% in the not treated group. No one therapy yielded optimal results, although surgical methods appeared to allow complete removal of the wart lesion with one application only, and to be better accepted by the patients because of the psychologic benefit of immediate remission notwithstanding the possibility of recurrencies. The results of the studied medical therapies appeared scanty. We conclude that, being the tissues around florid lesion purely affected by HPV, medical therapy needs further research in the hope of a more radical treatment.

[Climacteric symptomatology: relation with hormone levels and trophic features of the external genitalia].

During the last decade the development of a great amount of knowledge about the menopause has allowed to establish almost standardized hormonal treatments in order to prevent its symptoms and its late sequelae. Nevertheless, up to now the relationship between typical climacteric symptoms and the actual risk of late sequelae has not been assessed. The climacteric symptoms have hence been related to the involutional aspects of the external genitalia and to the levels of the sex steroids and gonadotropin hormones. An inverse relation between neuropsychical manifestations (insomnia, anxiety, depression, reduced memory, reduced libido) and 17-beta-oestradiol and progesterone levels has been shown. On the contrary, FSH and LH levels showed a direct relationship with neurovegetative symptoms. The relationship between the involutional aspects of the external genitalia and the atrophy related and the neuropsychical symptoms shows the need to evaluate these classes of symptoms, which appear to be most related to oestrogen and progesterone deprivation, in decision making toward hormonal replacement therapy.

[Treatment of uterine fibroma with goserelin].

The authors evaluated the efficacy of Goserelin, a synthetic GnRh analogue, in 15 patients affected by symptomatic uterine leiomyomata. The answer to treatment has been evaluated on clinical signs, echographically and by serial determinations of FSH, LH, 17 beta estradiol, Cortisol, total Cholesterol, HDL Cholesterol, Triglycerides, Calcemia, Hb, blood iron, MCHC. Clinical answer showed to be satisfactory, with complete subsiding of menometrorrhagia starting from the 3rd month of treatment, and not reappearing during the six months of follow-up. Ultrasound evaluation showed fibroid volume reduction of 70% at the end of the 6 months treatment, but volumes had increased up to only 22% lower than the starting ones at the end of follow-up. A better volume reduction and lesser growth during follow-up were shown for small fibroids. 17-Beta estradiol, total and HDL cholesterol shifted toward menopausal values, with a trend towards starting values during follow-up. Haemocromocytometric values showed to better even in the absence of target anti-anemic treatment, and the result showed to be kept during follow-up. The Authors conclude that such treatment can be useful in premenopausal women affected by symptomatic fibroids, when the need for treatment is likely to be limited in time.

[Correlations between trophism of the external genitalia and hormone levels during menopausal age].

In a case group of 341 perimenopausal women the following signs of reduced trophism of external genitalia have been studied: mons pubis hair rarefaction, pale mucosae, labia minora edge thinning, and labia minora involution, as well as their correlation with age, time lapse after the menopause, and plasma FSH, LH, progesterone and 17-beta-estradiol. Signs of reduced trophism appear to be present even in the younger considered ages, but a positive trend with age is shown. The passage from less than one year inveral from the menopause to one to three years interval shows the most marked increase in the appearance of involutional aspects, but at six years from the menopause one fourth of the women still show normal trophism of the external genitalia. An inverse relation with 17-beta-estradiol levels is shown for pale mucosae and labia minora edge thinning while the trend of mons pubis hair rarefaction shows no relation with the hormone level. An inverse relation is shown for progesterone levels and the trend of mons pubis hair rarefaction, while for the other signs of reduced trophism the same kind of relation as for 17-beta-estradiol was shown. FSH and LH levels show an inverse relation with "normal" trophism, a direct relation with pale mucosae and labia minora involution, and no relation with mons pubis hair rarefaction and labia minora edge thinning.

[Climacteric syndrome: comparison of several secondary therapies].

During a first one year period a random treatment for climacteric symptoms with "Estriol vaginal cream" vs "Trazodone and Estriol vaginal cream" and, after it and only in patients not complaining of dyspareunia, with "Trazodone" vs "Veralipride" has been conducted. After the first year all women complaining of dyspareunia were treated with Estriol vaginal cream. Eighty women were enrolled in the five treatment groups. After three months of treatment, a good remission of symptoms was shown, with differences in relations to treatment schedules. Dyspareunia subsided for more than 70% in women treated with Estriol vaginal cream (either by itself or in association), and Estriol vaginal cream achieved the best response from the highest number of the considered symptoms, besides being the only active treatment in insomnia. A good answer on hot flushes and "irritability, anxiety, depression" was obtained by Trazodone, while Veralipride showed to be more active on all neurovegetative symptoms (hot flushes, sweatings, tinglings, palpitations, astenia).

[Therapy of carcinoma-in-situ: comparison of methods].

This work comparatively evaluates the results of therapy for CIN (in terms of recovery persistence and relapse) in 362 women submitted to electrocauterization, laser vaporization, laser conization, diathermy loop conization, cold knife conization, from January 1980 to December 1989. Parameters as age, CIN grade, time elapsed between diagnosis and therapy, contraceptive exposition, have been considered. The results show a slight difference in the persistence and relapse rates between conization globally considered (traditional, laser, loop) and local surgery (cauterization, laser) in favour of the latter. Nevertheless the difference was too little to suggest abandoning one treatment in favour of the others. Moreover this work indicates that the results of therapy are independent from the type of contraceptive used. Furthermore, the finding of relapses during follow up only in CIN grade 2 indicates that this grade of CIN must be considered at risk, requesting adequate consideration and follow up.

[Advantages of topical therapy with polydeoxyribonucleotide in reparative processes after cauterization: experience at a center for early diagnosis of genital neoplasms].

This paper evaluates the common practice of a topic medical treatment following cautery (or other kinds of microsurgery) of the uterine cervix. Firstly, a controlled clinical trial on 116 patients treated with kanamycin sulphate alternated to polydeoxyribonucleotide vaginal suppositories, and 124 controls has been conducted. Subjective parameters (leukorrhea, bloody discharge, odorous discharge, more than three weeks vaginal discharge, itching, pain, burning) and colposcopy (Schiller test reactivity, squamo-columnar junction location, abnormal transformation zones, vascular striae, deepithelialized areas or frail epithelium) appeared more favorable in the treated group, the colposcopic finding of phlogosis being the only unfavorable finding more frequent among the treated patients. Successively, a controlled double blind clinical trial with kanamycin sulphate alternated to placebo vs PDRN at different dosages has shown an increase in number of epithelial cells in the treated patients and a better stimulation of fibroblasts by higher dosages of active substance. These results show the efficacy of the studied post cautery treatment schedule, and of PDRN in particular.

[Dynamics of the cervical mucosae during hormonal contraception by low-dose estrogens. A controlled prospective clinical study].

The Authors report the results of a prospective clinical study by means of colposcopy, planimetric measurements of iodine-light areas, cytology and target biopsy on 140 women taking low-dosage oral contraceptives and 140 controls. A basal examination and a follow up control at a mean interval of 13 months (min. 6, max. 30 months) have been performed. Colposcopy has shown no qualitative changes during follow up, neither in cases nor in controls. Planimetric measurements have shown a significant reduction of Ectropion and AnTZ in cases in comparison to controls, and more NTZ both in cases and controls. Cytology has shown at follow up a higher and significant reduction of phlogosis and viral cytopathic effect in cases. The results related to CIN, although non significant because of the small case group and the relatively short follow up, appeared more favorable in cases. In conclusion, oral contraception appears not to negatively affect the uterine cervix as far as cervical cancer is concerned but possibly to be protective. Nevertheless a longer follow up and a wider case group are needed in order to reach definitive results.

[Effectiveness of a topical medical treatment following diathermic coagulation: controlled clinical study].

A controlled clinical study has been conducted on the efficacy of a topic therapy. Kanamicine sulphate daily for 6 days and altered to polidesossiribonucleotide for the following 24 days were given after cervical cauterization for ectropion with or without AnTZ. Treated patients were 148 and untreated ones were 155. Subjective and objective parameters were evaluated. Among the former only bloody discharge (more frequent among the treated patients) and ill-smelling discharge (more frequent among the untreated patients) showed significant differences. Colposcopy instead showed better (or faster) healing of the treated group, with 83.6% vs 62% iodine dark cervices at Schiller test and 87.9% vs 59.7% SCJ at the external os or inside (P less than 0.01). Also disepitelized areas or areas with frail epithelium were less common among the treated patients.

[Colposcopic evaluation of cervical epithelium during oral contraception. A controlled clinical study].

The Authors report the results of a controlled clinical study by means of colposcopy, cytology and target biopsy on 525 oral contraceptive (O.C.) users and 519 controls. Except for age class 31-35, more iodine dark cervices are found in cases, more iodine light (AnTZ and/or ectropion) in controls. The difference, although not statistically significant, suggests a protective role of O.C. on cervical epithelia. In 34 women (15 cases and 19 controls) CIN has been diagnosed. O.C. users show a more favorable situation, except for ages over 30 exposed to O.C. for less than 24 months. The Authors hypothesize a protective role of O.C. in younger women. While women aged 30 or more still appear protected when exposed to O.C. for more than two years (possibly from younger ages), these results suggest some caution in beginning oral contraception after the 30th year of age.
