Routine examination by ultrasound for the detection of fetal malformations in a low risk population.

A low risk population was offered screening for malformations in the second and third trimesters of pregnancy. Of 16,763 scans of pregnancies in the second trimester, 61 malformations were identified (0.36%). In the third trimester, scanning of 10,752 pregnancies revealed 24 malformations (0.22%): in all, in a low risk population 0.58% major malformations were found. The sensitivity for noncardiac malformations of the second trimester scanning was 54.3 with a specificity of 99.9%. The malformations overlooked was a more benign spectrum than the cases diagnosed. The sensitivity for cardiac malformations was less than 20%.

[Prenatal detection of congenital heart disease].

Foetal echocardiography is a sensitive and specific method of investigation for prenatal detection of cardiac malformations. Experience has shown that brief echocardiographic screening can demonstrate, directly or indirectly, the presence of or probability of serious cardiac disease in the foetus as early as the 18th-20th weeks of pregnancy. The method of examination and its limitations are mentioned and it is recommended that screening should be carried out as part of other forms of obstetric ultrasonic scanning. Early prenatal diagnosis of severe structural cardiac malformations provide the possibility for selective termination of pregnancy, optimal pre- and perinatal observation and treatment. The possibility of prenatal diagnosis of serious cardiac malformations increases the value of obstetric ultrasonic scanning and may be included among the advantages of offering this form of investigation to all pregnant women.

[In vitro fertilization and embryo transfer. 2. The first 44 deliveries of 55 infants].

During the period 1.9.1985-21.2.1988, 143 patients commenced and completed treatment with in vitro fertilization and embryo-transfer (IVF-ET) in Rigshospitalet, Copenhagen. Forty-four out of the 143 patients were delivered of a total of 55 infants. There were 35 single deliveries, seven twin deliveries and two triplet deliveries. Seven women were admitted with threatened abortion and three women developed preeclampsia, one of these was severe. On an average, the deliveries occurred in the 38th week of pregnancy and the average weight of the singletons was 3,020 g. Ten women were delivered preterm and seven of the singletons had birth weights under 2,500 g. The frequency of caesarean section was 34% for singleton deliveries and 43% for all of the deliveries. Thirty-one boys and 24 girls were delivered, all of whom had normal karyotypes. One infant had a cleft lip but no other malformations. Three premature infants required brief respirator treatment. All of the infants were thriving on discharge. This material is still too limited to permit drawing of definite conclusions but it suggests, however, just as in other IVT-ET investigations, that the infants are born slightly earlier and weigh slightly less than average neonates, but that they are otherwise normal. In order to illustrate these problems further, the authors have commenced a more extensive prospective investigation with matched control persons.

Reactivation of peptidoglycan synthesis in ether-permeabilized Escherichia coli after inhibition by beta-lactam antibiotics.

The recovery of peptidoglycan-synthesizing activity after inhibition by beta-lactam antibiotics was investigated in ether-permeabilized cells of Escherichia coli B. Such cells synthesize sodium dodecyl sulfate-insoluble peptidoglycan when provided with UDP-linked precursors and Mg2+. The ability of beta-lactam antibiotics to inhibit the synthesis of peptidoglycan was correlated with their affinity for penicillin-binding proteins 1A and 1Bs. Penicillin-binding protein 1Bs is thought to be the major peptidoglycan synthetase in E. coli and is a major lethal target for beta-lactam antibiotics. Ether-treated bacteria were preincubated with concentrations of beta-lactams sufficient to completely inhibit peptidoglycan synthesis and then treated with beta-lactamases to inactivate free antibiotic prior to measurement of peptidoglycan synthesis. At 40 min after beta-lactamase treatment, the rate of peptidoglycan synthesis was about 74% of the control rate in cells pretreated with ampicillin, but only 15% of the control in cells pretreated with penicillin G or azlocillin. Reversal of inhibition by several other antibiotics fell between these extremes. When cross-linking of peptidoglycan was measured specifically, reversal of inhibition by ampicillin also occurred more readily than that by penicillin G. Reactivation of peptidoglycan synthesis was not due to de novo synthesis of penicillin-binding proteins since it occurred under conditions that did not allow incorporation of [14C]leucine. We conclude that there is considerable variation in the stability of the inactive acyl enzymes formed between various beta-lactams and penicillin-binding protein 1Bs, with those formed by penicillin G being relatively long-lived.

Plasma fibronectin concentration in normal pregnancy and pre-eclampsia.

Plasma fibronectin concentration was determined by electroimmunoassay in 111 normal pregnant women and in 23 pregnant with evidence of pre-eclampsia. The plasma fibronectin concentration during the period of normal pregnancy showed a significant increase. In mild or severe pre-eclampsia patients, the plasma fibronectin concentration was significantly higher than that measured at normal 38-41 weeks of gestation. The present findings suggest that measurement of plasma fibronectin might be of diagnostic value in pre-eclampsia.

Induction of labor and cervical ripening by intracervical prostaglandin E2.

A randomized double-blind trial was conducted over 48 hours comparing the effectiveness of prostaglandin E2 gel administered intracervically with that of demoxytocin buccal tablets for induction of labor in 103 patients with unripe cervical status (Bishop score 5 or less). A statistically significant difference was found in success frequency between the two groups, both on the first day (54.7 and 34.0%, respectively) and on the second day (82.0 and 61.9%, respectively; P less than .05). Without being matched, variables influencing the course of labor in the two groups were comparable. There was no statistically significant difference in the induction-delivery interval between the two groups during the first and second days of the trial. In both the prostaglandin E2 and the demoxytocin groups, patients who had not gone into labor during the first day showed a statistically significant increment in the Bishop score on the morning of the second day (2.4 and 1.3, respectively; P less than .01). The frequency of instrumental deliveries and cesarean section was the same in both groups; neither hypertonic uterine contractions nor side effects were observed in any patient. It is concluded that prostaglandin E2 gel administered intracervically is particularly well suited for the induction of labor in patients with unripe cervical status because of its combined contraction-inducing and cervical-ripening properties.

A randomized trial of external cephalic version with tocolysis in late pregnancy.

The effect of external version under tocolysis with intravenous ritodrine during the 37th week of gestation was studied in a prospective randomized trial comprising 130 consecutive women with a fetus in breech presentation. Version was successful in 41% of the patients, all of whom had a cephalic vaginal delivery. Of the 56 women in the control group in whom version was not attempted; 8 (14%) converted to the vertex presentation spontaneously. Failure of version was significantly correlated with primiparity, location of the placenta on the anterior uterine wall, and maternal weight. The caesarean section rate was significantly lower in the version group (27%) than in the control group (46%) (P less than 0.05). The overall caesarean section rate for a fetus in breech presentation was 50%. There were no serious complications associated with version and the condition of the infants at birth was better in the version group than in the control group. We advise external version under tocolysis in late pregnancy to reduce the frequency of breech presentation in labour.

Submucous paracervical blockade compared with intramuscular meperidine as analgesia during labor: a double-blind study.

A double-blind study was carried out to compare the effect of submucous paracervical blockade using 12 mL 0.25% bupivacaine (55 women) to the effect of intramuscular injection of 75 mg meperidine (62 women) during the first stage of labor. All 117 were normal primiparous pregnancies. Seventy-eight percent of the women in the paracervical blockade group achieved full or acceptable pain relief against 31% in the meperidine group (P less than .01). Transient fetal bradycardia occurred in two cases in the paracervical blockade group and one in the meperidine group; all infants were born in good condition. Fetal distress, defined as an umbilical artery pH of 7.15 or less and/or a one-minute Apgar score of 7 or less was more frequent in the meperidine group (16 infants) than in the paracervical blockade group (six infants) (P less than .05). Submucous paracervical blockade is superior to intramuscular meperidine as pain relief during labor. Furthermore, meperidine results in more infants with asphyxia as compared with paracervical blockade.

Acardiac twins: pathophysiology, diagnosis, outcome and treatment. Six cases and review of the literature.

Twin gestation is often a hazardous pregnancy and especially the monochorionic twin pregnancy significantly contributes to fetal morbidity and mortality. Among the serious complications with twins, the twin-twin transfusion syndrome complicates 5-35% of monozygotic twin pregnancies with monochorionic placentation. Acardiac twinning, earlier known as chorioangiopagus parasiticus, is the most extreme manifestation of this condition. An acardiac twin is a rare complication of multifetal pregnancy, in the literature reported at an incidence of 1% of monochorionic twin pregnancies, i.e. 1 of 35,000 pregnancies. In the following paper we review the literature on the subject and report 6 cases, 5 twins and 1 triplet, that were diagnosed at our department during the period of 1993-1997 and treated conservatively. Only 1 child survived.
