We describe the development and performance of a second-generation enzyme immunochromatography method for visually quantifying theophylline in whole blood without the use of instrumentation. We have developed the novel concept of an internal chemical clock reaction to combine the capillary-migration and color-generation protocol of the two-step immunochromatographic assay into a single-step, simultaneous protocol. The two assay components are (a) chromatographic paper to which glucose oxidase (EC 1.1.3.4) and monoclonal antibody to theophylline have been immobilized, and (b) an enzyme reagent consisting of glucose, dicarboxidine, ascorbate, and theophylline-labeled horseradish peroxidase (EC 1.11.1.7). The ascorbate acts as an internal clock by inhibiting premature color formation until the ascorbate has been completely consumed in the peroxidase-mediated reaction. Color is then generated rapidly, producing a clearly visible front on the paper. Performance evaluations of the 20-min one-step assay show very good precision, analytical recovery, specificity, and accuracy. This simplified protocol is reliable and convenient for therapeutic drug monitoring in the physician's office.

Thirty schizophrenic patients were treated with hemodialysis for 4-36 months. The clinical results were evaluated using two psychiatric scales: the Brief Psychiatric Rating Scale and the Inpatient Multidimensional Psychiatric Scale. In 13 schizophrenic patients psychiatric symptoms disappeared completely, and complete social reintegration followed. Eight patients showed no significant modification of schizophrenic disease after more than 30 dialysis sessions. Nine patients were not considered because their treatment was interrupted during the first month. Dialysis improved the psychotic attitude in one group of schizophrenic patients. The best results were obtained using polyacrylonitrile membranes.

The authors summarize two years trial of dialysis treatment of schizophrenia. Twenty-five schizophrenic patients were treated with hemodialysis using PAN membranes. The dialysis schedule was: dialyzer; RP 610; blood flow: 250 ml/min; dialysate flow: 500 ml/min; time: 3 hr; vascular access: arteriovenous fistula, femoral vein, antebrachial veins. Dialysis was first performed for three days, repeated after one week, two weeks, three weeks, and one month, and then performed once a month. The drug regimen was never modified or interrupted. The results were evaluated with the Overall and Lorr scale. Nine patients interrupted the treatment early and were not considered; nine patients showed disappearance of psychiatric symptomatology (Overall and Lorr index decreased to 21.8 at 2); non-significant modifications of the main schizophrenic symptoms were observed in seven patients. According to our trial, dialysis with polyacrylonitrile membranes can modify the psychotic attitude in a group of schizophrenic "dialysis responders".
Starting from a case of pure fibroma of the small intestine, which led to ileo-ileo-colic invagination, the Authors review the literature on the subject. The great rarity both of the type of neoplastic pathology found and the type of induced intestinal invagination is stressed. It is concluded by asserting that correct diagnosis, adequate selection of the type of surgery and its efficient performance allow a favourable prognosis in the overwhelming majority of cases.

Pigeons were exposed to a stimulus fading procedure in which control of responding was transferred from red and black stimuli to lines of different angular orientation. After superimposing one line on the red stimulus and the other line on the black stimulus, the intensity of the lines was gradually increased and that of the red stimulus was gradually reduced. Probes consisting of red and line stimuli presented separately were used during the course of fading to assess control exerted by each element of the compound. As the lines were faded in, they did not acquire control of responding. As red was faded out, control of responding was acquired first by the lower intensity red stimuli in combination with the line stimulus, and finally by the angular orientation of the lines. Probes also determined the point at which the line stimuli, presented alone, would maintain a high degree of stimulus control. The results demonstrated that new stimuli in fading acquire dimensional control of responding in two sequential stages. Acquisition of stimulus control in fading was explained in terms of attenuation of stimulus blocking.
