[Pituitary apoplexy].

Pituitary apoplexy is an infrequent complication of pituitary adenomas, caused by hemorrhagic or ischemic infarction in the tumor, with typical clinical presentation: severe headache of sudden onset, visual disturbances, sleep tendency or comma. Along the last ten years we have treated eight patients with pituitary apoplexy. The diagnostic was clinically established in all of them. Seven cases complained of severe headache and vomiting. Eight patients reported visual disturbances. In one case low level of consciousness and meningeal irritation were the only findings. In six cases the apoplexy was the first pituitary adenoma manifestation. MRI and CT studies demonstrated the pituitary stroke in seven patients. Surgical trans-sphenoidal decompression was performed in seven patients, requiring urgent management in only one case. All patients experienced a marked visual improvement, but there was no amelioration of endocrine preoperative disturbances in any case. We conclude that quick diagnosis, early onset of hormonal therapy and urgent or delayed trans-sphenoidal surgery, depending on clinical manifestations, constitute the principies of the appropriate treatment of pituitary apoplexy.

Effects of CDP-choline on the recovery of patients with head injury.

A single blind randomized study has been conducted in 216 patients with severe or moderate head injury, with the aim of comparing the evolution of those that received only conventional treatment with the evolution of those treated with CDP-choline. Our results indicate that CDP-choline improves the global outcome of patients. We have found a trend towards a greater improvement in motor, cognitive and psychic alterations in the patients treated with CDP-choline, as well as a shortening of the stay in the hospital ward in the patients receiving this drug that initially presented with severe head injuries.

Effects of glia-conditioned medium on primary cultures of central neurons.

The effects of glia conditioned media on the survival and differentiation of embryonic neuron cultures of the Central Nervous System is described. We established glial cultures of peritumoral areas and the culture medium was changed serially and collected as glia conditioned medium (GCM). Neuron enriched cultures plated on poly-L-lysine coated Petri dishes and after 4 days in vitro, the neuronal cultures were treated for the inhibition of glial cells. We established two groups of neuronal cultures that were respectively exposed to the GCM and to conventional culture medium. We evaluated the survival and differentiation of neuronal population in each group of cultures by contrast-phase microscopy and cellular uptake of Horse Radish Peroxidase. The cell cultures exposed to GCM showed a survival during the in vitro stages and an organization and differentiation pattern of mature neurons larger than the control cultures. This fact suggests that the glial cultures produce a diffusible neurotrophic factor that influences the neuronal response in vitro.

[Characterization of brain tumors in vitro].

The characteristics of brain tumors in vitro are analyzed in relation to their morphology, ultrastructure, immunocytochemistry and cell growth, showing that it is possible to identify the cell origin and to know several biological characteristics of the primitive tumor.
