[Changes in serum levels of beta 2-microglobulin, neopterin and 2',5'-oligoadenylate synthase activity during clearance of hepatitis B virus with or without acute phase in transaminases].

We have investigated the relation between acute-like transaminase exacerbations and the induction of 2',5'-oligoadenylate (2-5A) synthetase activity, HLA-I associated beta 2-microglobulin and macrophage activated release of neopterin during hepatitis B virus clearance in 70 patients treated or not with interferon. In treated patients who had an exacerbation in ALT during HBV clearance (loss of HBeAg and HBV-DNA from serum), the activity of the enzyme 2-5A synthetase and the level of beta 2 microglobulin increased markedly (p < 0.05). In contrast, in the absence of a peak in ALT during HBV clearance following interferon administration, the levels of 2-5A synthetase activity and neopterin, but not of beta 2-microglobulin, rose significantly (p < 0.05). Neither the non-responder treated patients nor the untreated controls had significant changes in these parameters, irrespective of the transaminase levels. Thus, elimination of viremia after interferon treatment may occur by different pathways as reflected by the presence or absence of acute-like biochemical exacerbations.

[Follow-up study of delta agent infection in children with chronic hepatitis B].

We studied the frequency of HDV infection in children, in order to determine its incidence and to evaluate the possible association between the anti-HD and the replication level of HBV of the hepatic histology. A total of 68 children with chronic hepatitis B. 29 relatives of children with anti-HD. 25 children without HBV markers nor hepatic pathology and 10 asymptomatic HBsAg carriers were studied. Anti-HD appeared in 13% of children with chronic HBV infection, at low titers. AgHD was negative in serum and liver in all of these patients. Anti-HD was also positive in 25% of the relatives of children with anti-HD was also positive in 25% of the relatives of children with anti-HD, whereas in 100% of the families there was at least one member with HBV markers. Delta infection did not appear in any of the other children studied. We have not found association between the anti-HD and sex symptoms or hepatic histology. Also, there was not relation between HDB infection and the HBV replication level.

[Prevalence of hepatitis caused by A, B and D virus in children].

The aim of this study was to determine the prevalence due to hepatitis A, B and D viruses infection in children. A total of 286 children from Madrid area with ages ranging between 0 and 13 years were included. The sample was randomized with respect to the sex and age referring to the total population of Madrid. The anti-HAV was positive in 15.16% of cases, with an increasing lineal correlation with age. Any marker of HBV infection was found in 6.6% and HBsAg in 1.4%. There was an exponential correlation between the carrier state and the age, with a maximum at the first year and diminishing thereafter with age. The 21% of the cases with positive HBV-markers were HBsAg carriers. A predominant perinatal and intrafamiliar transmission of HBV was detected. Our results indicate a intermediate prevalence of HBV infection in Spain, suggesting the importance of HBsAg detection in pregnant women. None of the HBV-infected cases had anti-HD.
