Ultrastructural analysis of fine needle aspirates from benign breast lesions.

Thirty six fine needle aspirates from various types of benign breast lesions were examined by electron microscopy and correlated with their cytological appearances. In all cases the parenchyma consisted of clumps of cohesive cells with the ultrastructural features of epithelial cells. In many cases, particularly from fibroadenomas, the parenchyma consisted of single layers of polarised epithelial cells showing lumen formation. Similar arrays of apocrine epithelial cells were observed in 60% of fibrocystic lesions. The more solid clumps from hyperplastic lesions consisted of epithelial cells of variable shape and electron density with disorganised lumen formation. Irrespective of the type of lesion, the epithelial cells were not normally subtended by myoepithelial cells or basal lamina. The extraction process seems to result in a shearing between the epithelium and basal lamina with lysis of the myoepithelial cells. Most naked nuclei probably result from lysed myoepithelial cells.

[Local immune reaction in human intestinal spirochetosis].

The pathogenetic and clinical importance of intestinal spirochaetes in man is still unresolved. In 12 patients mainly presenting with mild diarrhoea, light and electron microscopy demonstrated massive spirochaetal infestation of the colonic mucosa (spirochaetosis). There were several hitherto unreported features: spirochaetes adhered not only to the surface epithelium of the intestine but were also present within epithelial cells and subepithelial macrophages; many partially degranulated mast cells were noted within the epithelium; there was a marked increase of IgE plasma cells within the lamina propria. In control biopsies intraepithelial mast cells were absent and IgE cells occurred only sporadically. Penetration of the microorganisms into the intestinal mucosa may be responsible for this unusual immune response. Spirochaetes, symptoms and findings disappeared after antibiotic therapy. The authors therefore suggest that intestinal spirochaetosis can cause clinical symptoms in man, and that spirochaetes should not invariably be considered harmless commensals.

Reduction of the surgical excision rate in benign breast disease using fine needle aspiration cytology with immediate reporting.

Patients attending a breast clinic in two different periods were studied. In the first period fine needle aspiration cytology (FNAC) was not available and in the second it was used on all discrete solid breast lumps and reported immediately in the clinic. With the use of FNAC the overall surgical excision rate for discrete solid lumps was reduced from 83 per cent to 41 per cent and the excision rate in patients with benign disease was reduced from 74 per cent to 23 per cent (P less than 0.001). All patients with breast cancer in the second period had malignant cytology and no patient with benign or acellular cytology has been shown, after a minimum follow-up period of 18 months, to have breast cancer. Using FNAC with immediate reporting, the number of operations performed in patients with benign breast disease can be safely reduced.

Fine needle aspiration cytology, with immediate reporting, in the outpatient diagnosis of breast disease.

One hundred and fifty one new patients attending a breast clinic over a 6 month period underwent fine needle aspiration cytology (FNAC), with immediate reporting of the smears in the clinic. Thirty nine smears (25.8%) were classified as unequivocally malignant, 10 (6.6%) as being suspicious of malignancy, 61 (40.3%) as benign and 41 (27.1%) were acellular. There were no false positive diagnoses and only one false negative cytological diagnosis of breast cancer [corrected]. Immediate reporting of results enabled the diagnosis to be discussed with the patient at the first attendance and allowed improved surgical management of both benign as well as malignant breast disease.

Primary hyperparathyroidism diagnosed at autopsy. A study of eight cases.

This paper describes the clinical and pathological findings of eight cases of primary hyperparathyroidism first diagnosed at autopsy. The eight cases were obtained from the autopsy files of the Western General Hospital, Edinburgh over a decade (1971-80). The duration of the final hospital admission was very short in the majority of cases but analysis of the clinical history, in which gastro-intestinal and neuro-psychiatric disturbances figured prominently, showed that symptoms of the disease may have been present for very much longer--perhaps years in some instances. Details of the main autopsy findings and cause of death are also presented.

The histogenesis of malignant melanoma in relation to pre-existing pigmented lesions.

One hundred and twenty-nine cases of malignant melanoma seen at Bangour General Hospital over an eleven year period were reviewed clinically and histologically. In 51 patients (39.5%) there was a history of pre-existing pigmented lesion at the site of the melanoma. In 14 cases (10.8%) histology confirmed traces of a benign melanocytic naevus. The relationships between naevus cells and malignant melanocytes are described. The patterns most frequently observed were those of intradermal naevus cells in the deeper part of an invasive melanoma and of a compound naevus undergoing malignant change in its junctional component. From these histological observations a histogenetic sequence of events is postulated. In the 37 patients with a positive history of a preceding pigmented lesion but no histological traces of a naevus, the percentage incidence of Lentigo Maligna (LM) and Superficial Spreading Melanoma (SSM), as compared with the 78 patients with a negative clinical history, was significantly higher than the incidence of nodular melanoma (NM). Although LM and SSM are thought to have separate histogenesis and aetiology, they share an important clinical and histological factor, i.e. in both, the radial growth phase is preceded by a, more or less, prolonged stage of intraepidermal horizontal growth. It is postulated that this pre-existing lesion of atypical melanocytic proliferation rather than a hypothetical benign naevus is in most cases the precursor of an invasive melanoma.

Melanoma in the South-East of Scotland: a review.

A review was carried out of 129 patients with extraocular melanoma of the skin who were treated at Bangour General Hospital during the period 1968 to 1978 in order to compare our findings with those of earlier studies in the United Kingdom. The clinical features of our series were very similar to those of earlier studies but the impression is that the incidence since the war has been rising. This was confirmed with a sub-group of our patients in which the incidence in West Lothian increased by 52 per cent from the first to the second five-year period; this was statistically significant. One disturbing fact was that only 25 per cent of patients presented for treatment in less than six months from the onset of symptoms, suggesting that the danger of a pigmented lesion is not appreciated by the public. A health education programme is long overdue.
