[Expression of epidermal growth factor receptor (EGFR) in ovarian carcinoma stage III-IV].

Hyperexpression of epidermal growth factor receptor (EGFR) is often identified as unfavorable prognosis for different epithelial cancers. The study was concerned with an attempt of establishing a relationship between EGFR expression, on the one hand, and patient's clinico-morphological status, prognosis and efficacy of chemotherapy for stage III-IV serous ovarian carcinoma, on the other. EGFR hyperexpression predominated in advanced aggressive tumors and involved a significantly shorter period preceding tumor progression. Similarly, overall survival median in patients with EGFR hyperexpression (21+/-4 months) appeared lower than without it (42+/-8 months). In serous ovarian carcinoma stage III-IV, EGFR hyperexpression should be considered sufficient for prognosis of chemotherapy efficacy, pre-progression time and survival.

Expression of biomolecular markers (Ki-67, PCNA, Bcl-2, BAX, BclX, VEGF) in breast tumors.

We carried out a retrospective immunohistochemical study of Ki-67, PCNA, Bcl-2, BAX, BclX, and VEGF expression in tumors of two groups of breast cancer patients with favorable and unfavorable course of the disease. Considerably enhanced VEGF expression was detected in tumors of patients with early relapses of breast cancer. High VEGF expression was paralleled by high level of Ki-67 and PCNA expression in tumors. It can be hypothesized that expression of VEGF, Ki-67, and PCNA in primary tumor can be used for predicting the course of breast cancer or detecting the patients at a high risk of early relapses.

[The specificity of expression of molecular biological markers in tumors of the mammary gland].

Data are described on a retrospective immunohistochemical study of the Ki-67, PCNA, Bcl-2, BAX, BclX and VEGF expression in tumors of two groups of patients with breast cancer (BC) and with the favorable and unfavorable clinical courses. A reliably high level of VEGF expression was detected in tumors of patients with an early BC relapse. Apart from VEGF, higher levels of the Ki-67 and PCNA expression were registered in tumors of the discussed patient's category. It can be suggested that the expression parameters of VEGF, Ki-67 and PCNA in the primary tumor can be made use of in evaluating the BD prognostications or describing the features of high-risk groups with a high probability of early disease relapses.

[The role of tele-thoracoscopic parasternal lympho-dissection in the diagnosis and treatment of breast cancer].

The outcomes of treatment were studied in 190 patients with breast cancer who had undergone radical mastectomy with telethoracoscopic parasternal lymphodissection. An association of this parameter with the sizes of a primary tumor, with the extent of damage to the axillary lymph collector, with the histological pattern of a tumor, and with the site of a primary focus was analyzed. Radical removal of the parasternal lymphatic chain was proved by scintimammography in the pre- and postoperative periods. The high diagnostic value of telethoracoscopic parasternal lymphodissection was shown as a miniinvasive axilliary of radical mastectomy for breast cancer, which makes it possible to establish a stage of the disease reliably and to plan chemoradiation treatment adequately.

[Comparison of immunocytochemical and immunohistochemical methods for biomarker detection in breast cancer].

The detection of the biological parameters of the tumor before the treatment beginning becomes of more importance. The present study aimed to carry out the comparative analysis of the molecular markers expression (P53, Ki-67, Her-2/neu, Bcl-2, Bax, ER, FasL and CD95) at the cytologic and the correspondent histologic samples. The 18 tissue samples of the breast cancer were investigated. The immunocytochemical and the immunohistochemical methods of the molecular markers determination were used. Our study showed the correlation between two methods and the possibility of the use of the immunocytochemical staining as routine method of the molecular markers expression determination.

[Combination therapy of patients with locally advanced breast cancer].

The investigation involved 30 patients with locally advanced breast cancer (T3-4N1-2M0). Combination therapy comprised two courses: carboplatin 300 mg/m2, i.v., dropwise, on day 1; doxorubicin 30 mg/m2, i.v., bolus-flow, on days 1 and 8; 5-fluorouracil 350 mg/m2, i.v., bolus-flow, on days 1 and 8, and irradiation of the breast and regional metastasis area (single target dose--2 Gy, total target dose--40 Gy). Overall clinical response was 96.7% (29/30), mammography-wise--83.3% (25/30). All patients were found operable and radical mastectomy was performed in 25. Therapeutic effect stage III-IV was histologically confirmed in 40% (25/30), stage I-II--60% (15/25). Median overall and recurrence-free survival was not reached within 36 months in 24/30, relapse-free survival was been reported in 16/24 (66.6%), tumor progression--8/24 (33.4%). Three-year; host-mastectomy recurrence-free survival--68.8 +/- 16.0%.

[Epidermal growth factor receptors and their ligands in endometrial carcinoma: correlation with clinico-morphologic factors and steroid receptors].

Epidermal growth factor receptors (EGFR) were identified in the membrane fraction of tumor in 21 out of 58 (36%) endometrial cancer patients. EGFR ligands--EGF-like peptides--were found in tumor cytosols in 36% of patients. However, EGFR and EGF-like peptides were observed in 7% of tumors only; there were no receptors or ligands in 39% of patients. Both EGFR and steroid receptors were expressed in 20 tumor patients. The frequency of EGFR expression in endometrial carcinomas was practically identical to that of normal endometrium at the proliferative stage of menstrual cycle and half that of hyperplastic endometrium. Postmenopausal patients tended to show a decrease in hormonal sensitivity of carcinoma and EGFR expression frequency rate and a lower degree of endometrial carcinoma cell differentiation. Also, a significant rise in the EGFR expression frequency and mean concentration was found in conjunction with a deeper invasion of tumor into the myometrium.

Phospatidylinositol 3-kinase expression in human breast cancer.

Phospatidylinositol 3-kinase (PI 3-kinase) expression was analysed by Western blotting with monoclonal antibodies to the p85 subunit in a series of tumour and adjacent mammary gland samples collected at surgery from 33 breast cancer patients. Seventy-nine percent of the investigated pairs of the samples were characterised by an increased level of PI 3-kinase in the tumour in comparison with the adjacent mammary gland. PI 3-kinase activation was not associated with tumour steroid receptor status, histologic grade and other clinico-morphological characteristics. Furthermore, immunoblotting of epidermal growth factor receptor (EGFR) in the tumours with increased PI 3-kinase and corresponding adjacent tissues revealed no association between EGFR and PI 3-kinase activation. Thus, increased PI 3-kinase expression appears to be a widespread feature of breast cancer not associated with the main biological markers of its prognosis and hormone sensitivity.

[The effectiveness of breast-conserving surgery in T1-2N0M0 breast cancer].

A retrospective analysis of the end results of surgical treatment of 1.417 patients with T1-2N0-1M0 breast tumors is presented: local recurrence after radical surgery (1.151)-2.4% after radical resection (165)-5.5%, and after lumpectomy combined with axillary lymphadenectomy (101)-4.0%. In the T1N0M0 group, the end results of radical resection and radical mastectomy were identical. In the T1N0M0 and T2N0M0 groups, in which lumpectomy combined with axillary lymphadenectomy was performed, the end results were inferior to those radical mastectomy. There was a correlation between tumor cell detection in the blood and lymph vessels and extent of intervention: radical mastectomy-6%; radical resection-13% and lumpectomy combined with axillary lymphadenectomy-17%. The latter index tended to rise in cases of recurrence after breast-conserving surgery.

Phyllodes tumour: cytologic and histologic presentation of 22 cases, and immunohistochemical demonstration of p53.

A retrospective study of 22 cases of phyllodes tumour (PT) was undertaken to evaluate the potential value of fine needle aspiration (FNA) cytology in the diagnosis of benign and borderline PT. Histological material was available from 12 patients with typical benign PT (group 1), six patients with less typical changes (group 2) and four cases of borderline PT (group 3). Cytological presentation of PT in these cases was similar to that described by other cytologists, although abundant cellular material was obtained in only eight FNAs, naked nuclei were present in nine cases only, and atypical or suspicious cytological features were found in seven cases. Comparative analysis of p53 was made in nine patients with PT, five cases with other benign breast lesions and five with malignant lesions. p53 reaction was positive in five of nine patients with PT (all cases from groups 2 and 3), compared with two of five cases of carcinoma. p53 was negative in all patients with PT from group 1 and the five other benign cases. We suggest that cytopathologists should be careful when a myxoid stromal component is present in cytological smears.

[Expression of epidermal growth factor receptors and their ligands in malignant tumors of the breast].

Epidermal growth factor receptors (EGFR) have been investigated in the tumors of 166 breast cancer patients and concentrations of their ligands (alpha-TGF and EGF) were measured in 104 tumors by radioimmunoassay. EGFR and both growth factors were simultaneously detected in 18% and EGFR and one of the ligands were found in 11%. There were trends to higher frequency of EGFR expression in advanced tumors, in ductal and lobular infiltrative cancers with high grade of malignancy and in estrogen receptor negative breast cancers. A possible role of the parameters studied in the prediction of breast cancer and in the choice of therapy is discussed.

Comparative analysis of the sensitivity of endometrial cancer cells to epidermal growth factor and steroid hormones.

BACKGROUND: Epidermal growth factor (EGF) and EGF-regulated processes play an important role in steroid signal transduction. Comparative analysis of EGF and steroid receptor expression and the sensitivity of early stages of proliferation induction, such as activation of phospholipid turnover to EGF and steroids, may provide a useful new approach to characterizing the sensitivity of endometrial cancer to hormone therapy. METHODS: Progesterone (PR), estradiol (ER), and EGF receptor (EGFR) content was measured by radioligand competitive methods in surgically excised tumors from 26 patients with primary endometrial cancer. In short term cell cultures isolated from 11 of these tumors, the influence of a 10-minute treatment with 10(-8)M EGF either alone or combined with 10(-8)M progesterone on 32P-incorporation into phospholipids was studied. Phospholipids were fractionated by thin-layer chromatography and were located by autoradiography, and quantification of the labeled compounds was made by densitometric scanning of the autoradiograms. RESULTS: Epidermal growth factor receptor was found in 15 of 26 (58%) endometrial cancer samples. Eighty-two percent of the tumors studied contained PR, and 81% contained ER. No significant correlations were revealed between EGFR and ER/PR status or concentration. Epidermal growth factor stimulated 32P-incorporation by more than 120% of the control level in five of seven EGFR-positive and in one of four EGFR-negative endometrial cancer samples. An inverse relationship was revealed between EGFR content and the percentage of EGF-induced stimulation of phospholipid turnover in endometrial cancer cells (r = -0.6; P = 0.15) and between EGFR content in EGFR-positive samples and the extent of progesterone suppression of EGF-induced turnover (r = -0.77; P = 0.04). CONCLUSIONS: Determination of EGF sensitivity on a receptor and a functional level may provide important additional information about the hormonal sensitivity of endometrial cancer.

[Humoral antibodies to structural proteins of murine mammary tumor virus as a potential immunologic marker of human breast cancer].

The presence of humoral antibodies specifically recognizing structural proteins of murine mammary tumor virus was studied in indirect immuno-enzymatic reaction, in 79 patients with suspected tumors of the breast. Diagnosis was verified after surgery by histological procedures. Said antibodies were detected in 13% of patients with benign tumors, lymphomas and mammary lymphosarcomas. However, in the breast cancer group including patients with T1-2N0M0 and T1N1M0, antibodies were identified in 85.7% of sera. It is suggested that patients with breast tumors who have antibodies to murine mammary tumor virus develop a specific nosological type of tumor which arises at reproductive age and is capable of high-rate growth and low-level metastatic spreading. Feasibility of application of said procedure for early detection of breast cancer is discussed.

[Evaluation of the effectiveness of tamoxifen in malignant breast tumors: new approaches].

It has been shown that 17 beta-estradiol stimulates the cycle of phospholipid conversion in the breast tumor cells. The action of tamoxifen on antiestrogen cells is not limited by weakening the stimulating effect of 17 beta-estradiol on the exchange of intracellular phospholipids, but gives rise to a more complicated pattern of changes: inhibited incorporation of 32-P-phosphatidylcholine (PC) and activated exchange of phosphoinositides (PI). The experimental findings of 53 breast tumors have indicated that in 47.2% of cases Tamoxifen alters the PC/PI ratio and causes its 2-fold increase. Such alterations have been found to be induced by the ability of Tamoxifen to suppress the activity of protein kinase C that regulates the synthesis of PC and PI. It is suggested that the revealed capacity of Tamoxifen to change the rate of intracellular phospholipid conversion might be used for evaluating the efficiency of this agent on malignant tumors.

Organotypic growth and differentiation of human mammary gland in sponge-gel matrix supported histoculture.

Blocks of breast tissue obtained during radical mastectomies from 23 patients with mammary gland carcinomas were used for cultivation in native-state, gel-supported histocultures. We show that the human mammary gland can be successfully maintained in this system so that normal epithelial breast structures proliferate and undergo differentiation for several weeks and a well-developed system of ducts and lobules is formed. Using antibodies to individual keratins 17 and 8 we have shown for the first time that ducts and alveoles developing in vitro undergo differentiation into the lining epithelium and myoepithelium in the same way as mammary gland epithelium in vivo. Growth of epithelial structures in vitro is also accompanied by the development of continuous basal membrane.

Myoepithelial and basement membrane antigens in benign and malignant human breast tumors.

Serial cryostat sections of 160 human breast lesions and of 9 lymph-node metastases were studied by indirect immunofluorescence. We used monoclonal antibodies (MAbs) to lining-epithelium-specific keratin 8 and to myoepithelium-specific keratin 17 in combination with polyclonal and monoclonal antibodies to major basement membrane components, laminin, collagen type IV, entactin/nidogen, and large heparan sulfate proteoglycan (perlecan) core protein. Continuous basement membranes adjacent to a basal layer of keratin-17-positive myoepithelial cells were typical for normal, benign and in situ carcinomatous structures. In invasive and metastatic structures, always formed by keratin-8-positive tumor cells, basement membranes were found only rarely and with conspicuous fragmentations. This lack of basement membranes correlated with loss of myoepithelium identified by staining for keratin 17. In comedo structures of invasive ductal carcinomas and in papillary carcinomas, fibrovascular complexes with numerous blood vessels and deposition of basement membrane material were often seen in the stroma. Immunomorphological analysis of 41 cases of doubtful diagnosis at intra-operative biopsy was also performed. A combination of MAbs to keratins 8 and 17, and to basement membrane components, made it possible to distinguish between morphologically similar benign and malignant proliferations and to detect single-cell invasion of the stroma. This combination of antibodies may be recommended as an auxiliary immunomorphological tool for differential diagnosis of intra-operative breast biopsies in dubious cases.
