Herpes simplex virus infections. New treatment approaches make early diagnosis even more important.

The herpes simplex viruses HSV-1 and HSV-2 cause problems for millions of people worldwide. Infections range from simple cold sores and fever blisters to severe central nervous system disorders. Development of effective antiviral medications has made prompt recognition important in primary care practice. Appropriate therapy can significantly reduce both medical and psychosocial ramifications of herpes infections and can greatly improve the quality of life for many patients.

Unique wounds and wound emergencies.

Unique wounds and wound emergencies include a variety of environmental assaults including bites, stings, electrical and chemical injury, and severe reactions to medications. This article reviews the clinical manifestations and treatment options essential to the management of these environmental assaults.

Esophagitis dissecans superficialis associated with pemphigus vulgaris.

A number of dermatologic conditions may have associated esophageal manifestations. Teleologically, this may be because both the skin and the esophagus are lined by stratified squamous epithelium. Bullous dermatoses that may produce blisters, erosions, and/or stricture of the esophagus include cicatricial pemphigoid, Hailey-Hailey disease, Darier's disease, various forms of epidermolysis bullosa, bullous pemphigoid, and pemphigus vulgaris. Very rarely, bullous diseases may affect the esophagus in such a manner that there is a sloughing of the entire mucous membrane. The production of such an esophageal cast has been termed esophagitis dissecans superficialis. This is the second case recorded in the medical literature of pemphigus vulgaris associated with esophagitis dissecans superficialis. Additionally, this is the first case of pemphigus vulgaris in which the esophagus is demonstrated to be positive by direct immunofluorescence.

The correlation of disease activity and antibody titers in pemphigus.

Twenty cases of pemphigus extracted from the literature and 19 patients from UCLA with pemphigus were studied in detail regarding the possible correlation of pemphigus titer and disease activity. A statistically significant relationship between titer and disease activity, as well as change in titer and change in disease activity, was found. However, in spite of this relationship, serial titers were not found to be consistent enough to be used reliably as a guide to therapy or prognosis in pemphigus.

Trichotillomania in adults.

The cause of hair plucking is purely psychological. The extent of the hair plucking and the area from which the hair is plucked seem to have no direct correlation with the patient's underlying psychopathologic behavior. Hair plucking that begins in childhood and extends into adolescence and adulthood may represent a more complex psychopathologic condition. Therapy can consist of confronting the patient with his behavior and helping him cope with the triggering stressful external causes. Psychiatric consultation is suggested if this is not helpful. Trichotillomania by definition implies psychosis. Since this behavior is seldom associated with psychosis, we suggest the term hair plucking be used to describe this syndrome. Herein, we present two cases of trichotillomania (hair plucking), review of the relevant literature dealing with the psychopathologic nature of this disorder, and suggest methods for managing these patients.

Recent advances in the diagnosis and treatment of pemphigus.

Immunofluorescent techniques offer considerable promise in facilitating the diagnosis of pemphigus and other bullous diseases and also have provided a means for further investigative studies of these diseases. The immunofluorescent findings have in part stimulated the use of immunosuppressive agents in the management of pemphigus. Their exact status is still undetermined but early experience appears favorable.
