[Hypertensive angioencephalopathies].

The results of clinical and physiological study of 2000 patients, aged 20-60 years, suffering from hypertensive encephalopathy with differential evaluation of varying cerebrovascular pathology types (according to RAMS Neurology Institute classification) and stages of their development were summarized. The complex of methods used comprised psychological personality questionnaires, neuropsychological methods, computed tomography, rheoencephalography, ultrasonic diagnosis, etc. The data was computer processed. The main diagnostic and prognostic criteria for chronic hypertensive angioencephalopathies on early stages of arterial hypertension and the principles of their prevention are outlined.

[Effect of chronic pathology of the upper cervical sympathetic ganglion on the cerebral cortex (experimental study)].

A study was made of the character of disorders of glycogen metabolism and dynamics of the glycogen synthetic properties of sensorimotor cortical neurons at different stages of dystrophic processes under chronic irritation of the anterior cervical ganglion. The authors demonstrate the local glycogen synthesis in synapse ultrastructures, which is of paramount importance for local homeostasis, ensuring high plasticity and dynamism of cortical synapses in information transmission. Decimetric radiotherapy revealed that the sclerosed sympathetic ganglion exerts a permanent tonic effect on the regulation of intracerebral vessels. It may be assumed that control of sympathetic fibers from the cervical ganglion is aimed to a definite measure at specialized regulation of energy brain supply.

[The theoretical methodological aspects of preventing nervous system diseases].

The theoretical and methodological aspects of the development of the integrated programs of preventing nervous diseases are reviewed. Among the methodological problems evidence is provided for ecological, epidemiological, preclinical, systemic, individualized and rehabilitation approaches. The concept is given of the dynamic patho- and sanogenesis of nervous diseases including neurophylo-ontogenesis, neuroreactivity and neuroadaptivity, polyfactorial nature and dynamism, staged and phasic nature of the course of nervous system conditions. Criteria for the efficacy of the rehabilitation and preventive programs are distinguished.

[Health-related and pathogenetic aspects of vascular diseases of the nervous system].

A concept of the dynamic patho- and sanogenesis of vascular diseases of the nervous system was developed on the basis of clinico-physiological studies. The concept is based on the data on the philo- and ontogenesis, reactivity and adaptability+, complexity and dynamism, stages and phases of the process. The concept suggested may serve the basis for elaborating the program of preventing cerebrovascular diseases.

[An automated system of diagnosis of cerebrovascular diseases for preventive mass screening of the population].

Employing 19 criteria the authors have elaborated simple formal diagnostic rules in tabulated and automated modes which help detect cerebrovascular diseases in the course of prophylactic check-ups of the population. The automated diagnostic variant includes the assessment of the questionnaire data, rheoencephalography and electrocardiography. The application of this variant has allowed the diagnosis of the disease in 95.4%. The system of automated diagnosis makes it possible to significantly increase the quality and efficacy of wide-scale prophylactic check-ups of the population.

[Prevention of vascular diseases of the nervous system].

On the basis of the literature data and the findings of their own studies the authors consider methodological aspects of the prevention of vascular diseases of the nervous system. Epidemiological, prenosological, systemic, individualized and rehabilitative approaches to the creation of the system for the prevention of vascular diseases of the nervous system have been determined.

[Effect of meteorologic and solar factors on the psychophysiologic responses of healthy subjects and patients with vascular lesions of the brain].

Study of the time-course of psychophysiological parameters in relation to the effect of meteoheliofactors revealed a significant correlation of the studied processes in all the examinees. The most sensitive to such an effect was higher nervous activity. The atmospheric pressure exerted the most pronounced influence on the parameters. There were two major (positive and negative) and the intermediate (inert) types of responses. Reactions of some patients either preceded or occurred some time after exposure to meteoheliofactors.

[Cardiac and hemodynamic disorders in the pathogenesis of the initial manifestations of cerebrovascular pathology].

Initial manifestations of cerebrovascular pathology (CVP) in the presence of arterial hypertension (n-26) and atherosclerosis (n-428) were combined with various cardiovascular disorders ranging from cardialgia to coronary heart disease in 67.4% of the patients. Cardial disturbances were expressed as an atypical pain syndrome in 47% and as angina of effort and postinfarction cardiosclerosis in 23.2% of the patients. Electrocardiographic changes were elicited in 52.6-62% of the patients. Bicycle ergometry revealed a decrease in all parameters of tolerance to physical exercise. Disorders of the general and cerebral hemodynamics were more pronounced in atherosclerotic patients; at the first stages of cerebral pathology they predominantly presented the normokinetic type of the hemodynamics (40.6%) while its more marked forms were associated with the hypokinetic type of the circulation (52). The early diagnosis of cardial and hemodynamic disorders is necessary for the presention of acute disturbances of the cerebral circulation.

[Genetic aspects of cerebral atherosclerosis and lipid metabolism].

The results of investigations on the incidence of cerebral atherosclerosis and the distribution of lipid levels in the general population and among the first kin relatives of probands with cerebral atherosclerosis (CA) are presented. The correlation revealed between the signs under study showed them to be genetically determined. In 47.6% of cases, predisposition to CA was determined by genes regulating the level of triglycerides and in only 25%, by genes monitoring the level of cholesterol. The values of genetic correlations between the predisposition to CA and the level of individual classes of lipids are accounted for by the presence of genes displaying a pleiotropic effect on pair signs.

[State of cerebral hemodynamics in healthy subjects and diagnosis of the subclinical manifestations of inadequate cerebral blood supply].

Results of clinical and physiological examinations of 389 practically healthy subjects and 96 persons with subclinical manifestations of insufficient blood supply of the brain are presented. The healthy probands showed an intensification of autonomodystonic manifestations with age, while in the persons with the subclinical manifestations of the insufficient blood supply no dependence of the symptoms on age was revealed. REG findings showed that in the healthy subjects the pulse blood flow diminished, the vascular wall tone increased, and the vascular wall elasticity decreased with age. In distinction from this the above symptoms appeared in the persons with the subclinical insufficiency as early as on the 3d decade of the life. The persons with these symptoms constitute a high-risk group and need prophylactic measures.
