[Plasma theophylline level in patients with bronchial asthma treated with novphyllin].

The authors followed up the plasma theophylline level in 51 patients with bronchial asthma treated with novphyllin. Plasma theophylline level was determined (according to Jatlow--1975) before novphyllin intake and by 1, 2, 4 and 8 hour after it. The investigation was repeated in 18 patients on 4 th day after the systematic novphyllin treatment. The study revealed that plasma theophylline concentration gradually increased till the second hour after the intake of the preparation in all patients, whereafter it started to decrease. By 60 min since the beginning of the experiment the therapeutic plasma theophylline level (from 10 to 20 and over 20 mg/ml) was found in 31.25 per cent of the patients, by the second hour--in 50.99 per cent, by the fourth hour--in 28 per cent and by the eighth--only in 12.19 per cent. After a three-day novphyllin treatment, under the same investigation conditions, it was established that the therapeutic plasma theophylline level was observed twice more often by the fourth hour (44.44%). In patients, administered nophyllin and antibiotics in combination (tetraolean and rhondomycin), the plasma theophylline level was faster increased and was longer maintained within the therapeutic limits. The authors underlined that the therapeutic effect of the preparation begins after reaching a plasma level of 5 mg/ml, but has its maximum effect at a plasma concentration over 10 mg/ml.

[Argyrophilic integumentary structures of the miracidium, redia and cercaria of Paramphistmun daubneyi Dinnik, 1962 (Trematoda, Paramphistomidae)].

Paramphistomum microbothrium and P daubneyi are two sibling-species. The value of morphological criteria which have been used to distinguish the adults is not accepted by all authors. On the contrary, criteria based upon larval stages, except miracidia, permit an easy diagnosis. P. microbothrium parasitizes Bulinids, the rediae have a long intestine, germ-balls are inequal in size, cercariae are long-tailed and possess numerous lateral somatic bristles: about 9 by a half-body and numerous caudal bristles: about 17 pairs. P. daubneyi parasitizes Lymneids, the rediae have a short intestine, germ-balls are of equal size, cercariae are short-tailed and posses few lateral somatic bristles (about 4 by a half-body) and relatively few caudal bristles: about 6 pairs. In addition to the cercaviae, argentophilic structures of miracidia and rediae of P. daubneyi are described.

[Diagnosis and surgical treatment of endocranial compressive hemorrhage].

Authors present results of diagnosis at surgical treatment of 122 patients with compressive intracranial haemorrhages, observed and treated in the course of last five years. All patients have been admitted and treated at the Department for intensive care of patients, and have been under care of one neurosurgeon and anestetists duty and radiologists. 18 patients had chronical subdural haematoma, and 104-patients had acute compressive intracranial haemorrhages: epidural, subdural and intracerebral, one more haemorrhages in the same patient, hygromas, and sclopetar wounds with fast evolution. Out of 104 patients, 25 died (mortality rate 24%), and 26 patients died out of 122 patients (mortality rate being 21,3%). Authors enphesize the value of eho-encefalography and CAT of brain in the manengement of compressive inte cranial haemorrhages in the state of peace and in the state of war.

[Results of reconstructive bypass surgery of obliterative arteriosclerosis in the aortoiliac segment].

The present study included the results of 154 cases aortoiliac arteriosclerotic obstruction surgically treated at the Clinic for Surgical Diseases--Skopje Yugoslavia. The investigated operative clinical material was divided in to 3 groups. The I group consisted of endarterectomy in 89 patients. The II group had undergone resection of arterosclerotic obstruction in 26 cases. While in the III group a aortofemoral by-pass operation was carried out in 39 cases. In addition, a control group of 16 patients with aortoiliacal arteriosclerotic obstruction were medically treated. Preoperatively all patients were treated by Bergmann, s infusions. In our comparative study we found that the best satisfactory results were obtained by applying each of these methods on selected cases. There were 2 operative deaths.

In vivo transfection and/or cross-priming of dendritic cells following DNA and adenoviral immunizations for immunotherapy of cancer--changes in peripheral mononuclear subsets and intracellular IL-4 and IFN-gamma lymphokine profile.

In order to provoke an immune response, a tumor vaccine should not only maximize antigen-specific signals, but should also provide the necessary "co-stimulatory" environment. One approach is to genetically manipulate tumor cells to either secrete lymphokines (GM-CSF, IL-12, IL-15) or express membrane bound molecules (CD80, CD86). Furthermore, patient dendritic cells can be loaded with tumor-associated antigens or peptides derived from them and used for immunotherapy. Genetic modification of dendritic cells can also lead to presentation of tumor-associated antigens. Transfection of dendritic cells with DNA encoding for such antigens can be done in vitro, but transfection efficiency has been uniformly low. Alternatively, dendritic cells can also be modulated directly in vivo either by "naked" DNA immunization or by injecting replication-deficient viral vectors that carry the tumor specific DNA. Naked DNA immunization offers several potential advantages over viral mediated transduction. Among these are the inexpensive production and the inherent safety of plasmid vectors, as well as the lack of immune responses against the carrier. The use of viral vectors enhances the immunogenicity of the vaccine due to the adjuvant properties of some of the viral products. Recent studies have suggested that the best strategy for achieving an intense immune response may be priming with naked DNA followed by boosting with a viral vector. We have successfully completed a phase I and phase II clinical trials on immunotherapy of prostate cancer using naked DNA and adenoviral immunizations against the prostate-specific membrane antigen (PSMA) and phase I clinical trial on colorectal cancer using naked DNA immunization against the carcinoembryonic antigen (CEA). The vaccination was tolerated well and no side effects have been observed so far. The therapy has proven to be effective in a number of patients treated solely by immunizations. The success of the treatment clearly depends on the stage of the disease proving to be most efficient in patients with minimal disease or no metastases. A panel of changes in the phenotype of peripheral blood lymphocytes and the expression of intra-T-cell lymphokines seems to correlate with clinical improvement.

[Integumentary argentophilic structures in larval stages (miracidium, redia, cercaria) of Fasciola hepatica (Trematoda, Fasciolidae). Comparison with F. gigantica (author's transl)].

Integumentary argentophilic structures of the miracidium and cercaria are described and with those of F. gigantica compared. In the miracidia, the pattern of the organites of the terebratorium and the arrangement of the papillae between the first and second tier of epidermal plates are similar for the two species. However, the disposition of the medio-ventral plate of the second tier allows the miracidia of the two related species to be distinguished statistically. In the cercariae, the chaetotaxy differs between F. hepatica and F. gigantica in two cephalic regions and on the tail (presence of UD papillae in F. hepatica, absence in F. gigantica). The chaetotaxy of a daughter-redia of F. hepatica is also described. There is an important concentration of papillae around the buccal aperture, whereas at the pharyngeal and post pharyngeal levels, papillae are more spaced on the side supposed to be ventral, and scarce on the side considered dorsal.

Chaetotaxy of Psilotrema spiculigerum (Mühling, 1898) Odhner, 1913 (Trematoda: Psilostomatidae) cercariae.

The chaetotaxy of P. spiculigerum mature cercariae was studied. Unlike from the known in the literature up to now, the chaetotaxy of the cercariae was complemented as follows: a - the number and arrangement of the papillae on the dorsal, ventral and lateral surface of the body were first described; b - the arrangement of the papillae on the cephalic region and tail was corrected.
